Role of oral health professional in pediatric obstructive sleep apnea.

Sleep disordered breathing (SDB) in children is common. The impact of SDB on the growth and development of child may have detrimental effects on health, neuropsychological development, quality of life, and economic potential; therefore, SDB in children should be recognized as a public health problem as in the adult population. The coexistence of obesity and obstructive sleep apnea (OSA) not only appears to yield increased morbidity rates and poorer responses to therapy, but also is altogether associated with a distinct and recognizable clinical phenotype. Therapeutic options have somewhat expa

[Efficacy of velopharyngoplasty for treating obstructive sleep apnea hypopnea syndrome].

OBJECTIVE: To investigate the indication, efficacy and influencing factors of velopharyngoplasty for treating of obstructive sleep apnea hypopnea syndrome (OSAHS). METHOD: Thirty OSAHS patients were included in this study, all cases had velopharyngoplasty. The follow-up period was between 6 to 11 months; and a series of measurements parameters was analysed for thirty cases. RESULT: When compared preoperatively and postoperatively, Apnea and hypopnea index (AHI) decreased from 54.6+/-18.3 to 25.3+/-9.5; and the Epworth sleep scale decreased from 16.5+/-4.8 to 5.2+/-4.1. Alternatives to positive airway pressure for obstructive sleep apnea syndrome.

Owing to the prevalence, the considerable risk of accidents and cardiovascular consequences, optimal treatment of obstructive sleep apnea syndrome is of crucial importance. However, many patients look for alternative conservative or surgical therapies to continuous positive airway pressure to avoid discomfort and local side-effects owing to the mask. Scientific data are lacking for most of the alternative methods. However, it has been proven that intra-oral appliances can reduce mild-to-moderate respiratory disturbances; the maxillo-mandibular osteotomy is efficient in the short- and long-term

Effect of uvulopalatopharyngoplasty on endothelial function in obstructive sleep apnea.

OBJECTIVES: This study evaluates the effects of uvulopalatopharyngoplasty (UPPP) on serum levels of nitric oxide derivatives (NO(x)) and endothelial function by endothelium dependent flow-mediated dilation (FMD) in obstructive sleep apnea syndrome (OSAS). STUDY DESIGN: Prospective study. SUBJECTS AND METHODS: Fifteen healthy subjects and 30 subjects with moderately severe to severe OSAS who desired UPPP were prospectively enrolled. FMD was measured by high-resolution B-mode ultrasonography; serum level of NO(x) from peripheral blood samples was also measured. All subjects participated in sle

Allergic and nonallergic rhinitis: the threat for obstructive sleep apnea.

BACKGROUND: Although allergic rhinitis (AR) is accepted as a risk factor for obstructive sleep apnea syndrome (OSAS), the role of nonallergic rhinitis (NAR) is unknown. OBJECTIVE: To compare OSAS in patients with AR vs NAR. METHODS: We performed an observational study in 48 adults with AR and NAR that included a review of rhinitis and sleep symptoms, skin prick test results, self-administered questionnaire (Epworth Sleepiness Scale and 36-Item Short Form Health Survey) findings, and all-night polysomnography records. RESULTS: The most frequent sleep symptom was snoring. Patients with AR had

Madelung's disease as a rare cause of obstructive sleep apnea.

Madelung's disease, or multiple symmetric lipomatosis, is a rare disease, characterized by accumulation of unencapsulated fat, generally located symmetrically around the neck and shoulders. Here, we present the case of a patient with diffuse lipomatosis accompanied by obstructive sleep apnea due to cervical involvement and facial deformity, which made it necessary to use nasal pillows for ventilation. The patient was hospitalized with a diagnosis of pneumonia and required noninvasive ventilation due to severe hypercapnia. A brief review of the literature was made, and we describe and discuss t

Obstructive sleep apnea syndrome and nocturnal epilepsy with tonic seizures.

Some ambiguous symptoms may delay or lead to an erroneous diagnosis. We present a case of pure sleep, generalized tonic seizures in a patient with concomitant sleep apnea syndrome. The prolonged apneic periods with tonic muscle contracture lasting minutes and occurring exclusively at night with ensuing confusional state posed diagnostic difficulties because of the negative EEG at the beginning of the workup and the absence of other epilepsy symptoms (i.e. clonic phase, tongue biting, enuresis, seizures while awake). Numerous apneas on polysomnography led to the diagnosis of sleep apnea syndrom

Adjustable tongue advancement for obstructive sleep apnea: a pilot study.

OBJECTIVES: Surgical treatment of obstructive sleep apnea (OSA) caused by hypopharyngeal collapse of the upper airway can be considered in patients who are intolerant to continuous positive airway pressure (CPAP). The present procedures addressing the hypopharynx are invasive and have substantial morbidity and limited efficacy. METHODS: Ten patients (mean age, 44 years) with moderate to severe OSA, ie, an apnea-hypopnea index (AHI) between 15 and 50, with CPAP intolerance were included in a prospective, nonrandomized, multicenter study to evaluate the feasibility, safety, and efficacy of a no

Operative timing for absent pulmonary valve with obstructive sleep apnea.

Congenital absence of the pulmonary valve appears to have a prolonged fate, despite substantial regurgitation, thus the optimal timing of surgical correction remains unclear. A 53-year-old man with isolated pulmonary regurgitation accompanied by obstructive sleep apnea developed progressive heart failure after reopening of the foramen ovale. Closure of the interatrial shunt and pulmonary valve replacement with a 25-mm mechanical prosthesis relieved his refractory left heart failure.

Obesity and obstructive sleep apnea: pathogenic mechanisms and therapeutic approaches.

Obstructive sleep apnea is a common disorder whose prevalence is linked to an epidemic of obesity in Western society. Sleep apnea is due to recurrent episodes of upper airway obstruction during sleep that are caused by elevations in upper airway collapsibility during sleep. Collapsibility can be increased by underlying anatomic alterations and/or disturbances in upper airway neuromuscular control, both of which play key roles in the pathogenesis of obstructive sleep apnea. Obesity and particularly central adiposity are potent risk factors for sleep apnea. They can increase pharyngeal collapsib

[Exacerbation of obstructive sleep apnea syndrome at altitude 3,850 m].

Recently more people visit extremely high-altitude places for sightseeing or business. As a result, patients with obstructive sleep apnea (OSAS) also are likely to stay at such places. However the changes of severity of OSAS at extremely high-altitude are not certain. A 44-year-old man was given a diagnosis of OSAS in 2004 and had been using continuous positive airway pressure CPAP at night. On a visit to Nepal the severity of his OSAS was evaluated at Kyanjing Gompa (3,850 m) and at other places. At Kyanjing Gompa, his apnea-hypopnea index (AHI) was 41.8/hr on one night and 52.2/hr on another

Obstructive sleep apnea caused by carotid body tumor: case report.

Obstructive sleep apnea (OSA) results from a structural compromise of the upper airway combined with decrease in muscle tone during sleep. Overt upper airway pathology is rare, however a variety of pharyngeal tumors have been well described as a cause of OSA. We describe a case of a mass originating in the carotid body resulting in severe OSA with hypersomnia resistant to positive pressure ventilation.

Atherosclerosis and obstructive sleep apnea in patients with ischemic stroke.

BACKGROUND: Of late, obstructive sleep apnea (OSA) has been suggested to be a risk factor for atherosclerotic artery disease. In the present study, we analyzed the prevalence of atherosclerosis in stroke patients with and without OSA. PATIENTS AND METHODS: Two hundred and fourteen consecutive patients with ischemic stroke were included in this study. Patients were screened for classical cerebrovascular risk factors. All patients received a cardiorespiratory polygraphy to determine the apnea-hypopnea index. Duplex ultrasonography was used to evaluate the carotid bulb and the internal carotid a

Placebo and modafinil effect on sleepiness in obstructive sleep apnea.

INTRODUCTION: Previous studies have evaluated the effect of modafinil on residual excessive daytime sleepiness (EDS) in patients with obstructive sleep apnea syndrome (OSAS) under effective CPAP treatment. Even though those trials also used placebo groups, we suppose that the placebo effect might influence the patients' response to modafinil. METHODS: Twenty sleepy patients with OSAS under CPAP treatment were selected. All of them had Epworth Sleepiness Scale (ESS) >10. Following baseline evaluation (T1), all subjects were instructed to take placebo for 7 days. After this single-blind placebo

Auditory processing assessment in children with obstructive sleep apnea syndrome.

INTRODUCTION: The obstructive sleep apnea syndrome (OSAS) is a respiratory disorder that occurs during sleep and it is relatively common in children. AIM: The goal of this paper is to verify if there is a relationship between the obstructive sleep apnea syndrome (OSAS) and auditory processing. METHODS: In order to do that, three groups of children ranging in age from 5 to 11 were studied, including a normal group. Twenty subjects who made up the study group were submitted to ear, nose and throat (ENT) exams and to polysomnography (PSG), and were divided in two groups: GROUP I (RO) comprised

A retropharyngeal lipoma causing obstructive sleep apnea in a child.

BACKGROUND: Lipomas are common benign tumors, but they rarely occur in the retropharyngeal space and even more rarely occur in children. CASE REPORT: We report the case of an 11-year-old girl with a 5-year history of progressively worsening nasal obstruction, snoring, and excessive daytime sleepiness. Physical examination revealed a child with a body mass index of 16.9 kg/m2; otolaryngologic examination and imaging studies showed a large retropharyngeal mass. Polysomnography indicated an apnea-hypopnea index of 13.9 events per hour of sleep and a minimum oxygen saturation of 84%, with 20 epis
