Pain Duration and Intensity Are Related to Coexisting Pain and Comorbidities Present in Temporomandibular Disorder Pain Patients.

AIMS: To investigate the relationships between three pain parameters (duration, intensity, and frequency), the number of pain sites and comorbidities, and the risk of having coexisting pain and/or comorbidities in patients with temporomandibular disorder (TMD) pain. METHODS: The sample consisted of 198 outpatients attending the Dental Hospital of Chulalongkorn University. TMD pain was determined using the Diagnostic Criteria for TMD. Pain lasting 3 months or longer was defined as chronic pain. Pain intensity was reflected using a 0- to 10-point numeric rating scale, and pain frequency was ass

Benefits of implementing pain-related disability and psychological assessment in dental practice for patients with temporomandibular pain and other oral health conditions.

BACKGROUND: Evidence in the field of dentistry has demonstrated the importance of pain-related disability and psychological assessment in the development of chronic symptoms. The Diagnostic Criteria for Temporomandibular Disorders offer a brief assessment for the diagnostic process in patients with orofacial pain (Axis II). The authors describe relevant outcomes that may guide general oral health care practitioners toward tailored treatment decisions and improved treatment outcomes and provide recommendations for the primary care setting. METHODS: The authors conducted a review of the literat

Depression, somatization and anxiety in female patients with temporomandibular disorders (TMD).

The aim of this research was to determine the possible differences in degrees of depression, somatization and anxiety between the acute and chronic female patients with temporomandibular disorders (TMD), and whether these differences exist in healthy female patients. Ninety female patients were involved in this research; 60 of them were TMD patients of the Dental Polyclinic, while other 30 females came for a routine recall visit and had no problem related to TMD. Patients were aged 22 to 67 years, the average age being 38.5 +/- 12 years. All patients were asked to fill in the RDC/TMD protocol

Salivary cortisol levels in young adults with temporomandibular disorders.

AIM: Causes of temporomandibular disorders (TMDs) are multifactorial, involving both physical and psychological factors. The aim of this study was to evaluate the salivary cortisol levels in young adults with TMDs. METHODS: The Research Diagnostic Criteria (RDC)/TMDs were used to establish the diagnosis of TMDs regarding the physical (axis I) and psychological factors (depression and somatization; axis II). Young adults of both genders composed the TMDs males (21.7+/-2.21 years, n=10) and females (22.7+/-1.95 years n=10) groups, and 20 healthy matched controls composed the control male (22.5+

Definition, diagnosis, and forensic implications of postconcussional syndrome.

Injuries from blows to the head often are manifested only as subjective complaints. Postconcussional syndrome thus can be feigned for financial or psychological gain. The authors review the pathology of brain trauma, symptoms of postconcussional syndrome, and criteria for diagnosis. In addition to somatic deficits, psychological and cognitive problems are common. The likelihood and severity of postconcussional syndrome are greater for women. Malingering may be suspected in cases involving litigation, and tests to detect it are available. Treatment for postconcussional syndrome depends on the s

Somatized depression as a subgroup of fibromyalgia syndrome.

An increase of depressive symptoms in fibromyalgia patients has been noted in a number of studies. The etiologic significance of this finding remains, however, controversial. We suggest that a subgroup of patients with this symptom combination may be pragmatically classified as suffering from somatized depression. Clinical indicators such as a family history of depressive disorders, circadian disturbances, pronounced loss of appetite or libido, and chronic psychosocial stressors should be assessed and, if present, prompt the initiation of psychiatric evaluation and treatment including pharmaco

Diagnosis-specific mortality. Primary unipolar depression and Briquet's syndrome (somatization disorder).

A 42-year follow-up of 76 women with Briquet's syndrome revealed no evidence of excess mortality. A carefully matched group of patients with primary unipolar depression had excess mortality in comparison both with the Briquet's syndrome group and with the Iowa population. Although one of the 30 patients with Briquet's syndrome with available death certificates died by suicide, the rate of death due to unnatural causes in the Briquet's syndrome group was not greater than that predicted by relevant population figures. These findings suggest that some psychiatric illnesses involve excess mortalit

Colonised and neurasthenic: from the appropriation of a word to the reality of a malaise de civilisation in urban French Vietnam.

Neurasthenia remains an important health problem in certain Asian populations, both in Asia as well as in a diasporic context. An anachronistic disease for Western observers, it has become an exotic culture-bound syndrome as well as a somatoform disorder too often hiding much more serious issues of depression. This article approaches this 'problematic' health issue from a historian's point of view and offers a colonial genealogy that will discuss neurasthenia's outline in French Vietnam. By retracing and analysing the different mentions, definitions, and uses of the term neurasthenia in the in

Psychiatric diagnosis and psychological symptoms in impotence.

A sample of 137 males who presented with a principal complaint of impotence were assessed regarding manifest psychopathology through DSM-III psychiatric diagnosis and the SCL-90-R, a self-report symptom inventory. Contrasts in self-reported symptoms among dysfunctional groups assigned different DSM-III diagnoses proved to be very significant, as were contrasts with patients free of current psychiatric diagnoses. When the sample was separated into the three groups of organic etiology, psychogenic etiology with psychiatric diagnosis, and psychogenic etiology without psychiatric diagnosis, few si

Psychopathological correlates of anxiety sensitivity: evidence from clinical interviews and self-report measures.

Most research on anxiety sensitivity (AS) and its relation to psychopathology has examined the Anxiety Sensitivity Index (ASI) in various clinical samples. The present study was the first to investigate psychopathological correlates of AS using self-report measures, the anxiety, somatoform, and substance use disorders sections of the Structured Clinical Interview for the DSM-IV (SCID), and open-ended interview questions about the subjective meaning and origins of AS. A college student sample (N=317) was used for the correlational analysis, and subsamples of high-AS (n=44) and low-AS (n=41) par

A nationwide healthcare program for functional somatic disorders (FSD) in Denmark.

BACKGROUND: Functional Somatic Disorders (FSD) are prevalent, costly for society and cause significant suffering for patients. The healthcare system has previously failed to provide adequate care for these patients, leading to prolonged and fragmented diagnostic processes. Major obstacles have been the lack of a clear classification of FSD and lack of prioritization of diagnosis and treatment of FSD by other medical specialties. METHODS: In 2018, the Danish Health Authority issued national recommendations to improve the quality of care for FSD nationwide across all levels of the healthcare sy

Functional Disorders in Neurology: Case Studies.

Functional, often called psychogenic, disorders are common in neurological practice. We illustrate clinical issues and highlight some recent research findings using six case studies of functional neurological disorders. We discuss dizziness as a functional disorder, describing the relatively new consensus term Persistent Posturo-Perceptual Dizziness (PPPD), axial jerking/myoclonus as a functional movement disorder, functional speech symptoms, post-concussion disorder with functional cognitive symptoms and finally advances in treatment of dissociative seizures and functional motor disorders.
