A proposal for standardization of speech and hearing evaluations to assess velopharyngeal function.

The article comprises the proposal made by the Cleft Palate Committee of IALP to standardize the parameters of the speech and hearing evaluation in individuals with cleft palate/velopharyngeal incompetence (CP/VPI). The suggested parameters are the following: nasal resonance, nasal escape, articulation and its disorders, facial grimacing, speech intelligibility, patient teachability, expressive language and voice. The authors recommend a five-point scale description system, which can be used also for assessment of hearing, velopharyngeal competence and other attributes of the CP/VPI individual…

Effect of maximum power output and noise reduction on speech recognition in noise.

BACKGROUND: The maximum power output (MPO) of a hearing aid was typically discussed in the context of avoiding loudness discomfort. However, an MPO that is too low, as in the cases to avoid discomfort for people with a severe loudness tolerance problem and hearing losses that exceed the fitting range of the hearing aids, could negatively affect sound quality and speech intelligibility in noise. PURPOSE: The current study was designed to demonstrate the degradation in speech intelligibility in noise on the HINT (Hearing in Noise Test) when the MPO of the wearers' hearing aids was lowered by 10…

Characteristics of Speech Auditory Brainstem Response in Preschool Children With Attention-Deficit/Hyperactivity Disorder.

OBJECTIVE: This study aimed to investigate the characteristics of auditory processing (AP) in preschool children with attention-deficit/hyperactivity disorder (ADHD) using the speech auditory brainstem response (speech-ABR), which provides insights into the AP of speech signals in the central auditory nervous system (CANS). METHOD: A total of 84 preschool children diagnosed with ADHD, aged 4-6 years, were matched with 84 typically developing (TD) children based on gender and age. All children underwent speech-ABR testing, cognitive assessment using the Wechsler Preschool and Primary Scale of

Listening Difficulties in Children: Behavior and Brain Activation Produced by Dichotic Listening of CV Syllables.

Listening difficulties (LiD) are common in children with and without hearing loss. Impaired interactions between the two ears have been proposed as an important component of LiD when there is no hearing loss, also known as auditory processing disorder (APD). We examined the ability of 6-13 year old (y.o.) children with normal audiometric thresholds to identify and selectively attend to dichotically presented CV syllables using the Bergen Dichotic Listening Test (BDLT; www.dichoticlistening.com). Children were recruited as typically developing (TD; n = 39) or having LiD (n = 35) based primarily

Hearing Aids Reshape Neural Processing of Emotional Speech Without Improving Emotion Perception.

Hearing aids have improved speech intelligibility but not speech emotion perception in older adults with hearing loss. Hearing loss has also been linked to altered brain responses to emotional non-speech sounds, potentially contributing to the speech emotion perception deficits even under aided listening. While fMRI requires hearing aid removal due to metal incompatibility, fNIRS is silent and hearing-aid compatible, creating a novel opportunity to identify neural processes during aided listening. We leveraged fNIRS to examine how hearing aids affect brain function in experienced hearing-aid u…

Spatiotemporal movement variability in ALS: Speaking rate effects on tongue, lower lip, and jaw motor control.

PURPOSE: Although it is frequently presumed that bulbar muscle degeneration in Amyotrophic Lateral Sclerosis (ALS) is associated with progressive loss of speech motor control, empirical evidence is limited. Furthermore, because speaking rate slows with disease progression and rate manipulations are used to improve intelligibility in ALS, this study sought to (i) determine between and within-group differences in articulatory motor control as a result of speaking rate changes and (ii) identify the strength of association between articulatory motor control and speech impairment severity. METHOD:

Perceptual Pattern of Cleft-Related Speech: A Task-fMRI Study on Typical Mandarin-Speaking Adults.

Congenital cleft lip and palate is one of the common deformities in the craniomaxillofacial region. The current study aimed to explore the perceptual pattern of cleft-related speech produced by Mandarin-speaking patients with repaired cleft palate using the task-based functional magnetic resonance imaging (task-fMRI) technique. Three blocks of speech stimuli, including hypernasal speech, the glottal stop, and typical speech, were played to 30 typical adult listeners with no history of cleft palate speech exploration. Using a randomized block design paradigm, the participants were instructed to

Motor speech patterns in Huntington disease.

OBJECTIVE: Dysarthric speech of persons with Huntington disease (HD) is typically described as hyperkinetic; however, studies suggest that dysarthria can vary and resemble patterns in other neurologic conditions. To test the hypothesis that distinct motor speech subgroups can be identified within a larger cohort of patients with HD, we performed a cluster analysis on speech perceptual characteristics of patient audio recordings. METHODS: Audio recordings of 48 patients with mild to moderate dysarthria due to HD were presented to 6 trained raters. Raters provided scores for various speech feat

Analysis of Presurgical Language in Children with Posterior Fossa Tumours Relative to Postoperative Speech Outcomes: Findings from the European CMS Study.

Posterior Fossa Syndrome is a common complication in children following posterior fossa tumour surgery, typically marked by transient postoperative speech impairment (POSI; i.e., mutism or reduced speech). Previous studies suggest that children who develop POSI show different language profiles postoperatively compared to those who do not. It remains unclear to what extent these language difficulties exist preoperatively and whether preoperative language difficulties are related to postoperative speech status. This study provides the first comprehensive analysis of preoperative language samples

Evaluating Speech-in-Noise Deficits in Conductive Hearing Loss: The Underestimated Role of Low-Frequency Air-Bone Gaps.

UNLABELLED: <p>Introduction: This study aimed to investigate the impact of conductive hearing loss (CHL) on speech perception in noisy environments, with a particular focus on the role of low-frequency air-bone gaps. While CHL is traditionally considered a peripheral auditory disorder, emerging evidence suggests that it may also affect central auditory processing, especially under complex acoustic conditions. METHODS: Thirty individuals diagnosed with CHL and thirty age-matched participants with normal hearing were recruited from the Audiology Unit of Gazi University. All participants underwe
