The value of dynamic radiographic myelography in addition to magnetic resonance imaging in detection lumbar spinal canal stenosis: A prospective study.

OBJECTIVE: MRI is regarded as the study of choice in the diagnosis of lumbar spinal stenosis. In some cases, the supine MRI leads to a misdiagnosis in the extent of lumbar spinal stenosis. Dynamic myelography can detect lumbar spinal stenosis in these cases of where the MRI may not be as sensitive. To compare the sensitivities of dynamic radiographic myelography and supine MRI in lumbar canal stenosis (LCS) patients and to determine whether dynamic radiographic myelography is a valuable diagnostic exam in the work-up of lumbar canal stenosis. PATIENTS & METHODS: Over two years, the imaging da

"Do Not Follow the Tail": A Practical Approach to Remove a Sheared Lumbar Catheter Fragment Avoiding Its Migration into the Spinal Canal.

BACKGROUND: Catheter shearing is one of the most common complications of various neurosurgical modalities that use an intrathecal lumbar catheter. The sheared catheter fragment often can spontaneously migrate into the spinal canal; however, in some cases, the end of the fragment will remain outside the spinal canal. In this situation, the consulting neurosurgeons may try to retrieve the catheter fragment by approaching it directly through the catheter tract. This simple maneuver, however, can cause the fragment to slip into the spinal canal before it is secured, as we experienced recently in 2…

A unique occurrence of a cerebral atypical teratoid/rhabdoid tumor in an infant and a spinal canal primitive neuroectodermal tumor in her father.

This report describes the clinical, pathological, immunohistochemical and genetic data of two rare malignant neoplasms of the central nervous system (CNS)--a cerebral atypical teratoid/rhabdoid tumor (AT/RT) in a 5-month-old girl and a spinal canal primitive neuroectodermal tumor (PNET) in her father. Despite aggressive treatment, both tumors were fatal, displaying extensive local recurrence and diffuse neoplastic dissemination. The paraffin-embedded tumor tissue samples were analyzed using a dual-color FISH with a locus specific LSI22q (BCR) probe. In the AT/RT tissue, a loss of BCR locus was…

[Proposition of an anatomic and functional reserve into the spinal canal, as an interference factor on the mechanical degenerative low back pain and sciatica physiopathology].

OBJECTIVE: To test the hypothesis of an anatomic and functional reserve in the spinal canal that explains the radiological changes in lumbar spine of asymptomatic individuals, the differences in modalities of clinical presentation and the discrepancies in the prevalence of mechanical and degenerative low back pain and sciaticas. CASES AND METHODS: Two groups of 27 persons were submitted to computed axial tomography. In one group, the patients presented low back pain and/or sciaticas. The other formed by matched controls, showed no signs or symptoms. The bony canal area, dural sac area, interf…

B-scan ultrasonic measurement of the lumbar spinal canal as a predictor of industrial back pain complaints and extended work loss.

B-scan ultrasonic measurements of lumbar spinal canal diameter were examined as predictors of industrial back pain complaints and extended work loss. Baseline data were collected on 3,020 Washington State aircraft manufacturing workers, and over a mean 3.7-year follow-up period 352 subjects reported industrial back pain complaints. Mean canal measurements of subjects with industrial back pain complaints were smaller at all spinal levels than in subjects without complaints. The mean differences between the groups, however, were extremely small (0.07 mm to 0.51 mm), and not all levels were stati…

Vertebral canal and spinal cord mensuration: a comparative study of its effect on lumbosacral myelography in the dachshund and German shepherd dog.

One hundred noncontrast spinal radiographic studies followed by myelography were evaluated to compare vertebral canal size and spinal cord location and size in a chondrodystrophic breed (50 Dachshunds) with those variables in a nonchondrodystrophic breed (50 German Shepherd Dogs). Measurements were made of the sagittal diameter (height) of the vertebral canal, sagittal diameter (height) of the spinal cord, and transverse diameter (width) of the spinal cord in the lumbar and sacral regions. Differences were detected in the craniocaudal location of maximal vertebral canal height and maximal spin

The lumbar spinal canal in children. Part II: the interpedicular distance and its relation to the sagittal diameter and transverse pedicular width.

The lumbar interpedicular distance in 100 children aged 3-15 years have been determined radiologically before and after correction for magnification in the different age groups. The increase by age was approximately 2,5 mm in the upper four lumbar levels and 3,3 mm in L5. The difference between the vertebral levels was almost similar in the different age groups. A rather weak correlation between the interpedicular distance and the sagittal diameter of the spinal canal was found, strongest at the fourth lumbar level. No correlation was found between the interpedicular distance and the size of t…

Presenting features of neuroblastoma with spinal canal invasion. A prospective study of the International Society of Pediatric Oncology Europe - Neuroblastoma (SIOPEN).

INTRODUCTION: Between 5 and 15% of children with neuroblastoma (NB) present with or develop spinal canal invasion (SCI). The majority of these children have symptoms of epidural compression of spinal cord and/or spinal nerves. Treatment of NB-SCI is considered an emergency but its modalities are not yet well-established. Independently of treatment, NB-SCI may result in significant long-term disabilities. We report on the first prospective study of NB-SCI focused on presenting characteristics of both symptomatic and asymptomatic patients and correlation between SCI-related symptoms and imaging …

Physiologic Osseous Remodeling of the Anterior Wall of the Spinal Canal after Anterior Cervical Corpectomy and Fusion: A Retrospective Observational Study.

BACKGROUND: Anterior cervical corpectomy and fusion (ACCF) has been widely used in the treatment of cervical spondylotic myelopathy (CSM), ossification of posterior longitudinal ligament (OPLL), cervical trauma, and other cervical diseases, but few studies have reported the osseous and physiologic remodeling of the anterior wall of the spinal canal following ACCF. In this study, we analyze that remodeling process and its influence on titanium mesh cage (TMC) subsidence. METHODS: We performed a clinical and radiologic analysis of consecutive patients treated with ACCF. Growth rates (GRs) refle…

Efficacy of Nerve Root Block for the Treatment of Lumbar Spinal Canal Stenosis in Adults Older Than 80 Years of Age.

BACKGROUND: Patients with advanced lumbar spinal canal stenosis (LCS) often prefer non-operative treatment owing to decreased physiological function and comorbidities. Although the therapeutic value of selective nerve root block (SNRB) for LCS is confirmed, there are few reports of its effectiveness in the elderly. We investigated the efficacy of SNRB for LCS in patients over 80 years of age. METHODS: The subjects were 112 patients aged over 80 years (mean age: 84 years; 45 men and 67 women ) with medication-resistant LCS without cauda equina syndrome who underwent SNRB. Cases with acute-onse…

Reissner's fiber and the wall of the central canal in the lumbo-sacral region of the bovine spinal cord. Comparative immunocytochemical and ultrastructural study.

Reissner's fiber (RF) of the subcommissural organ (SCO), the central canal and its bordering structures, and the filum terminale were investigated in the bovine spinal cord by use of transmission electron microscopy, histochemical methods and light-microscopic immunocytochemistry. The primary antisera were raised against the bovine RF, or the SCO proper. Comparative immunocytochemical studies were also performed on the lumbo-sacral region of the rat, rabbit, dog and pig. At all levels of the bovine spinal cord, RF was strongly immunoreactive with both antisera. From cervical to upper sacral le

Case report 833: Primary spinal echinococcosis (Echinococcus granulosus) of lumbosacral spine with destruction of the left pedicles of L3-5 and extension of a large paraspinal cystic mass into the spinal canal.

CT is helpful in recognizing the occurrence of hydatid cysts. The increased spatial resolution allows better visualization of details of bone destruction and spinal canal involvement. Paraspinal cystic disease may also show typical and/or characteristic signs of hydatid disease even if serological findings are falsely negative or inconclusive. In the postoperative follow-up, CT may be helpful in assessing residual hydatid cysts and/or in detecting recurrence at an earlier stage.
