Cervical spine stenosis in chondrodysplasia punctata.

Chondrodysplasia punctata (CDP) is a rare skeletal dysplasia characterized by stippled epiphyses during infancy. The frequency is probably underdiagnosed because of the large heterogeneity in this group. Many genotypic variations exist. Although cervical instability is commonly seen in many skeletal dysplasias, cervical spine stenosis associated with CDP is very rare. We report a boy with phenotypic features of brachytelephalangic chondrodysplasia punctata (BCDP) who had severe cervical spine stenosis successfully corrected by vertebrectomies of C6 and C7 with a fibular strut graft. We discuss…

[Diffusion weighted MRI of spine tumors].

PURPOSE: To evaluate the contribution of diffusion weighted MR imaging in malignant spine pathology. Materials and methods. Between February 2004 and January 2005, 49 patients (43 to 86 years old) were included. Three groups were made: osteoporotic collapses (n = 13), malignant collapses (n = 15) and malignant spine lesions (n = 21). The MRI (Symphony 1.5T) allowed SENSE imaging. After conventional MRI examination (T1, T2 fat sat, T1 with Gadolinium), all patients underwent diffusion weighted imaging (Spin Echo) with variable b values: 0, 250, 500, 750 and 1000. The diffusion sequence lasted 2…

Dysferlinopathy associated with rigid spine syndrome.

Dysferlinopathy and rigid spine syndrome occurring in a 50-year-old man is reported. The patient noticed stiffness of knee and ankle joints, which gradually extended to neck, wrist and elbow joints leading to difficulty in anterior flexion. Muscular weakness and wasting of the lower extremities had developed since age 40, accompanied by a limitation of anterior bending of the spine. Elevated serum CK was noticed. Muscle CT revealed atrophy with moderate fatty replacement of muscles in the neck, shoulder and pelvic girdle, and marked replacement in the para-vertebral muscles, posterior compartm…

Cyberknife radiosurgery for metastatic spine tumors.

Metastatic spine tumors affect a large number of patients each year, resulting in significant pain, destruction of the spinal column causing mechanical instability, and neurologic deficits. Standard therapeutic options include surgery and fractionated external beam radiotherapy. The first option can be associated with significant morbidity and limited local tumor control. Conversely, radiotherapy may provide less than optimal pain relief and tumor control, because the total dose is limited by the tolerance of adjacent tissues, such as the spinal cord. The emerging technique of spinal radiosurge…

Anthropometrical analysis of cervical spine injuries.

UNLABELLED: To date the principal focus of the mechanism of cervical spine fracture has been directed towards head/neck circumference and vertebral geometric dimensions. However the role of other measurements, including chest circumference and neck length, in a standard cervical fracture population has not yet been studied in detail. Cervical fractures often involve flexion/extension type mechanisms of injury, with the head and cervical spine flexing/extending, using the thorax as an end point of contact. Thus, the thorax may play an important role in neck injuries. STUDY DESIGN: We prospecti…

Tracheostomy after anterior cervical spine fixation.

BACKGROUND: Patients with cervical spine injury may require both anterior cervical spine fusion and tracheostomy, particularly in the setting of associated cervical spinal cord injury (SCI). Despite the close proximity of the two surgical incisions, we postulated that tracheostomy could be safely performed after anterior spine fixation. In addition, we postulated that the severity of motor deficits in patients with cervical spine injury would correlate with the need for tracheostomy. METHODS: A retrospective review was undertaken of all adult trauma patients diagnosed with cervical spine frac…

Brucellar spine infection--four case reports.

Brucellosis can be difficult to diagnose because of the nonspecific and variable clinical picture. This systemic disease is still an important public health problem in the Mediterranean Basin. These four cases of brucellar spine infection originated from rural areas around Aydin, Turkey. The systemic complaints of fever, profuse sweating, malaise, polyarthromyalgia, and weight loss indicated the final diagnosis of brucellosis and presumptive diagnoses were made based on agglutination testing for brucellosis. Computed tomography and magnetic resonance imaging indicated signs of bone infection a…

[Vertebral body replacement in spine surgery].

Autografts and allogeneous bone grafts as well as cages are used for the reconstruction of the anterior column after corpectomy. Recently, e … l data on the treatment of fractures, metastasis and infection of the cervical, thoracic and lumbar spine demonstrated no significant difference between expandable and non-expandable cages. However, the increased stress-shielding effect of expandable cages compared to non-expandable cages might result in a deterioration of the long-term clinical outcome …

Blood loss in pediatric spine surgery.

This article reviews the extent of blood loss in spine surgery for scoliosis corrections in the pediatric age group. An extensive literature review presents blood loss values in surgery for adolescent idiopathic scoliosis, cerebral palsy, Duchenne muscular dystrophy, spinal muscular atrophy, and myelomeningocoele. The underlying disorder plays a major role in determining the extent of blood loss. Blood loss is considerably higher in those patients with a neuromuscular scoliosis compared with adolescent idiopathic scoliosis. Within the neuromuscular group those with Duchenne muscular dystrophy …

Putting some spine into alternative splicing.

Spinal muscular atrophy is a neurodegenerative disease caused by mutations of the SMN1 gene. The homologous SMN2 gene is unable to complement SMN1 because of a crucial mutation in an exonic splicing enhancer, leading to alternative splicing and exclusion of exon 7. Two recent papers show that the defect in splicing of exon 7 of SMN2 is specifically corrected by small synthetic effectors. These new and specific approaches have potential in the treatment of diseases caused by defective splicing.

[Cervical spine manipulation: risks--benefit--assessment].

Cervical manipulation is a widely used method indicated in non-specific mechanical neck pain and cervicogenic headache. Cervical manipulation can cause severe neurologic complications which are both rare and generally unpredictable, which can be compared with the accidents occurring with other treatments (nonsteroidal anti-inflammatory drugs...). To decrease this risk, we propose five recommendations developed by consensus: --unwanted effects, however minor (e.g. nausea or vertigo), of prior manipulation should be searched for routinely and taken as contraindications for future spinal manipula

Pedicular stress fracture in lumbar spine.

The purpose of this article is to report two cases of pedicular stress fracture of the lumbar spine, which is an uncommon cause of low back and leg pain. The relevant literature is reviewed and features of the cases that differ from those already reported are highlighted. One of our cases of pediculolysis is the first in the literature for that is caused by rotational instability induced by laminectomy. The remodeling of the fractured pedicle was striking when compared with its normal counterpart. To the authors' knowledge, this is the first report of a pediculolysis with T2-weighted imaging f…

[Imaging evaluation of cervical spine trauma].

Evaluation and treatment of patients with suspected cervical vertebral trauma requires close cooperation between multiple medical disciplines. Recognition of the significant signs of cervical spine injury enables accurate interpretation of the radiograph, and if necessary adequate selection of the best imaging method for further evaluation. When judiciously selected, these imaging techniques result in the expeditious gathering of the diagnostic information required for the management of the traumatized patient.

Rigid spine syndrome. Two case-reports.

UNLABELLED: Rigid spine syndrome is characterized by massive spinal rigidity, usually most marked in the cervical region. Stiffness of the peripheral joints is sometimes present. We report two cases. Patient 1 was a 12-year-old boy diagnosed at three years of age with Duchenne's muscular dystrophy because of delayed onset of walking. Contracture of the Achilles tendons, flexion contracture of the elbows, and loss of motion of the cervical spine were the main findings during the current evaluation. Radiographs of the affected joints were normal. An electrocardiogram showed an incomplete left bu…

Cervical spine abnormalities in professional singers.

A systematic X-ray study of the cervical spine of three populations of subjects (professional singers, beginning singers, and non-singers) who exhibited no vertebral column pathology or postural disorders provided evidence of abnormalities in the shape of the cervical spine in all of the professional singers studied. The disorders observed included hypolordosis, and more frequently, cervical rectitude or hypocyphosis without functional hindrance. The findings can be explained in terms of deformations undergone by the cervical spine after many years of intensive singing. The resulting functiona…

Cervical spine models for biomechanical research.

Biomechanical models have been used for the understanding of the basic normal function and dysfunction of the cervical spine and for testing implants and devices. Biomechanical models can be broadly categorized into four groups: 1) Physical models, made of nonanatomic material (e.g., plastic blocks), are often used for the testing of spinal instrumentation when only the device is to be evaluated. 2) In vitro models consisting of a cadaveric spine specimen are useful in providing basic understanding of the functioning of the spine. Human specimens are more suitable for these models than are ani…
