Carbon fiber reinforced implants have emerged as an alternative to traditional stainless steel and titanium implants because of the improved visibility on advanced imaging and excellent biomechanical strength. This can be advantageous in patients with concern about fracture healing, tumor recurrence, or additional imaging requirements. However, limited long-term data are available to present complications from the use of these implants. We present the case of a failure of a carbon fiber reinforced intramedullary femoral nail used in a closed distal femur fracture.

Boutonniere deformity associated with a dorsal avulsion fracture of the central slip overlying the proximal interphalangeal joint results in loss of extension of the joint and hyperextension of the distal interphalangeal joint. This article reports a surgical technique for treatment of the injury in 21 digits, which involves application of loop stainless steel wire. We also present the long-term results using the technique on the digits. Loop wire fixation is a successful surgical technique for the treatment of displaced central slip avulsion fracture.

Balloon mitral valvotomy (BMV) is a safe and effective therapy for rheumatic mitral stenosis. During the procedure, the coiled portion of 0.025 in. stainless steel BMV guidewire broke off within the giant left atrium (LA). Attempts to retrieve the guidewire with various snares failed as it was moving freely within the voluminous LA. We describe a technique wherein the broken guidewire was retrieved with a snare made of 0.014 in. percutaneous transluminal coronary angioplasty guidewire. The mechanism and prevention of this complication and management options are discussed.

The LifeSite Hemodialysis System (Vasca, Inc., Tewksbury, MA) is totally subcutaneous, incorporating two silicone catheters positioned in the central venous system and connected to a stainless steel-titanium valve implanted in a subcutaneous pocket. A randomized trial and nonrandomized clinical experience have demonstrated lower rates of infection and longer device survival when 70% isopropyl alcohol is used as the antimicrobial solution with the LifeSite.
