Sternal semi-closure using a bioresorbable osteosynthesis device: a new method for delayed sternal closure.

PURPOSE: To evaluate the safety and efficacy of our new delayed sternal closure (DSC) method, involving sternal semi-closure using a bioreso … patients undergoing early sternal semi-closure a few days after surgery. In the new DSC group, the sternum was fixed with Super Fixsorb MX40, followed by complete skin closure. RESULTS: Respiratory and hemodynamic conditions, such as systolic blood pressure, cardiac index, tidal volume, and regional oxygen saturation, were significantly more stable in t …

Use of a plant-based polysaccharide hemostat for the treatment of sternal bleeding after median sternotomy.

BACKGROUND: In cardiac surgery profuse or persistent sternal bleeding after sternotomy is routinely controlled with bone wax. However, bone wax should be avoided, especially in high-risk patients for nonunion of the sternum and infections. Purpose of this study was to evaluate an alternative technique to control bleeding after medium sternotomy using a plant based absorbable polysaccharide hemostat. METHODS: A consecutive series of 38 patients requiring median sternotomy for coronary artery bypass surgery (21 OPCAB, 17 CABG) had sternal bleeding control with the polysaccharide hemostat, STARS…

Bone imaging of sternocostoclavicular hyperostosis in palmoplantar pustulosis.

Sternocostoclavicular hyperostosis is a rare syndrome that is characterized by hyperostosis and soft tissue ossification between the clavicl … observed in the sternoclavicular, sternocostal, and manubriosternal joints, in the ribs, and in the sternum. Whole body imaging revealed radionuclide accumulations unexpectedly in other bones such as the vertebrae, femur, tibia, or sacroiliac joints in five of 11 cases. Radiographs were available in nine patients. Three chest radiographs were negative, an …

Two easy ways to ensure safe sternotomy and sternal closure.

Appropriate sternotomy and sternal closure are the most important factors in mechanical stability of the sternum and prevention of several postoperative complications. Easy techniques for identifying the sternal midline to facilitate opening and for obtaining reinforced closure are described. These techniques require minimal additional time. They are particularly indicated in patients at risk of sternotomy-related complications, and helpful to young surgeons in training.

Cantrell syndrome. Case report of an adult.

Cantrell syndrome is characterized by defects that involve the diaphragm, abdominal wall, pericardium, heart, and lower region of the sternum. It is a rare entity, usually diagnosed at birth and accompanied by high mortality due to the complexity and gravity of the anomalies. In this report, we present a 32-year-old male patient, who was diagnosed in infancy but who reached adult age asymptomatic.

Oesophageal atresia--severe tracheomalacia and its correction by aortopexy.

Tracheomalacia is an important cause of recurrent respiratory complications following correction of cogenital oesophageal atresia. It constitutes a well-defined syndrome, which requires clinical recognition and surgical correction in the most severely affected infants. Five cases are described. Special investigations confirmed tracheal collapse. Permanent relief of symptoms was achieved by suspension of the aortic arch to the sternum.

Use of a Capture-Guidance Surgical Instrument to Minimize Muscle Stripping During Nuss Repair of Pectus Excavatum.

Background: Pectus excavatum is an anomaly of chest wall development in which anterior ribs curve inward and the sternum is displaced toward the vertebral column. The Nuss procedure is a minimally invasive technique in which one or more metal bars are implanted to brace the sternum in a corrected position. Over time, the chest wall remodels into an anatomically corrected shape and the bar(s) are removed at a later date. During the procedure, passage of an introducer instrument and then the repair bar(s) may shear the intercostal muscles from the adjacent ribs. This creates larger than necessar…

Minimally invasive subxiphoid-right, videothoracoscopic technique of thymectomy for thymoma and rethymectomy.

Minimally invasive subxiphoid-right, videothoracoscopic (VATS) technique of thymectomy for thymoma is presented. This approach enables complete removal of the whole thymus, with visualization of the lower poles of the thyroid. The whole dissection is performed through the 4-7 cm subxiphoid approach with single 5-mm port insertion into the right chest cavity for the thoracoscope and, subsequently for the chest tube. The sternum is elevated with two hooks connected to the sternal frame. The lower hook is inserted through the subxiphoid incision and the superior hook is inserted percutaneo…

The vacuum bell for treatment of pectus excavatum: an alternative to surgical correction?

OBJECTIVE: Pectus excavatum (PE) is the most common chest wall malformation and one of the most frequent major congenital anomalies. The sur … ch are selected according to the individual patients age. When creating the vacuum, the lift of the sternum is obvious and remains for a different time period. The device should be used for a minimum of 30 min (2 per day), and may be used up to a maximum of several hours daily. Presently, a 12-15-month course of treatment is recommended. In addition, the …

Superior sternal cleft: repair in the newborn.

Superior sternal cleft is a rare congenital anomaly that should be repaired in the newborn while the bony thorax is still compliant. A method of approximating the U-shaped sternal defect that is applicable to the majority of patients seen as newborns is described.
