This report aims to describe the first case of muscular and collagenous choristoma in a dog. A 10-yr-old female mixed-breed dog presented with lateral recumbence, vocalization, positional vertical nystagmus, divergent strabismus, anisocoria, and status epilepticus. The clinical condition evolved to stupor and ultimately, death. Necropsy revealed a white mass causing an irregular increase in the volume of the cerebellar vermis. In histological analysis, a well circumscribed, unencapsulated mass was observed in the cerebellum, consisting of fibers of striated skeletal muscle and collagen fibers,…

The thalamus is a complex structure with over 40 named nuclei. Ischemic lesions of the thalamus exhibit a panorama of phenomena ranging from facial numbness to ocular and visual field disturbances to hemiplegia, behavioral disorders, and stupor. It is a dense neuronal hub with a bewildering variety of connections and functions. We present an intriguing case of intermittent vertical diplopia due to an artery of Percheron ischemic infarct of the bilateral paramedian thalami. We seize upon this opportunity to simplify the thalamic nuclei sub-divisions and their vascular supply. In this process, w…

At the 6-month follow-up, the patient had complete recovery with a good neurological outcome.

Psychoses caused by an intoxication with atropine or scopolamine are rarely published. Nevertheless atropine and scopolamine were being used in the ancient civilisations and are still in use today. The intoxication is characterised by dose-dependent and substance-dependent syndrome with specific central and peripheral symptoms. Atropine and scopolamine cause a central and peripheral anticholinergic blockade of the muscarine receptor. Psychiatric symptoms include restlessness, excitement, hallucinations, euphoria, disorientation but also stupor, coma and respiratory depression. History, pathoph…

A 21-year-old male, who had undergone a ventriculoatrial shunt for hydrocephalus 5 years previously, became stuporous. A roentgenogram revealed that the distal segment of the broken atrial catheter had migrated and become lodged in the heart. Because the fragment had not adhered to the myocardium, it was easily retrieved by the transvenous approach with a retriever catheter. If the migrated catheter does not adhere to the myocardium, transvenous catheter retrieval is absolutely necessary. If, however, the migrated catheter adheres to the myocardium, an open thoracotomy would be required for re…

A study of 170 patients with juvenile rheumatoid arthritis and a review of the literature indicate that this disease can significantly affect the central nervous system. Signs of CNS dysfunction were observed in 13 children. During the acute toxic stages the EEG is abnormal in many cases. Other manifestations of toxic encephalopathy such as irritability, drowsiness, stupor, convulsions and marked meningismus may be evident in severe cases. Meningitis is often suspected but ruled out by the finding of normal CSF. Steroids can rapidly improve the condition of these children. If ;unexplained' sei…

The Labours of Hercules, written by Agatha Christie, contains twelve short stories, with 'atropine' playing a crucial role in the seventh story, The Cretan Bull (1939). Atropine easily crosses the blood-brain barrier (BBB), leading to central nervous system (CNS) side effects such as delirium, hallucinations, disorientation, memory problems, coma or stupor, and convulsive seizures when overdosed. In this episode, a concentrated form of atropine obtained from prescribed eye drops was misused to induce a hereditary incurable neurodegenerative disorder, similar to Huntington's Chorea, in the clie…

To choose differentiated therapy, 62 patients suffering from refractory schizophrenia were administered bromocriptine in a dose of 7.5-15 mg/day after they had been placed on placebo. According to the bromocriptine test, the treatment was changed in 53 patients. After the occurrence of the picture of hallucinatory stupor remission was attained with the aid of less doses of neuroleptics given before. In patients who responded to bromocriptine by anxious excitation, the health status noticeably improved after administration of anxiolytics and lithium carbonate. Those patients in whom bromocripti…

OBJECTIVES: To describe the occurrence of delirium in a cohort of older medical intensive care unit (ICU) patients and its short-term durati … tent delirium in the post-ICU period. Overall, 83 of 118 (70%) had delirium during hospitalization. Stupor or coma occurred in 44% of the patients overall, and 89% of survivors of stupor/coma progressed to delirium. Patients with dementia were 40% more likely to be delirious (relative risk = 1.4, 95% confidence interval = 1.1-1.7), even after controlling …

BACKGROUND: Acute changes in cerebral function after elective coronary bypass surgery is a difficult clinical problem. We carried out a multicenter study to determine the incidence and predictors of -- and the use of resources associated with -- perioperative adverse neurologic events, including cerebral injury.
METHODS: In a prospective study, we evaluated 2108 patients from 24 U.S. institutions for two general categories of neurologic outcome: type I (focal injury, or stupor or coma at discharge) and type II (deterioration in intellectual function, memory deficit, or seizures).
RESULTS: Ad…

Zolpidem (Stilnox) is a non-benzodiazepine hypnotic drug of the imidazopyridine class intended for treatment of insomnia in humans. A 16-year-old neutered cat, weighing 3.8 kg, was presented with sudden onset of stupor, disorientation, severe ataxia, vomiting and hypersalivation. Symptomatic treatment was given when ingestion of 1.25 mg/kg zolpidem (half of a 10-mg tablet) was confirmed, because no information on the efficacy and safety of the use of flumazenil in the treatment of zolpidem poisoning in cats has been published to date. As zolpidem is prescribed with increasing frequency in huma…

Catatonia may be monosymptomatic at presentation, with stupor or coma as the cardinal and only manifestation. A case of catatonic coma with profound bradycardia is presented to help clinicians recognize this entity and include catatonia in the differential diagnosis of coma as a rare etiology after other more common causes of coma have been excluded. Cases of catatonic coma are nevertheless important to identify because treatment with intravenous benzodiazepines is simple and highly effective. The authors argue that the descriptive term "catatonic coma" is preferable over terms implying psycho…

Neuroleptics are among those pharmacological agents that can cause a nonpsychogenic catatonic state. Neuroleptic malignant syndrome (NMS) is marked by a change in state of consciousness, ranging from withdrawal through stupor to coma. In addition, it is characterized by autonomic dysfunction, hyperthermia, mutism, and rigidity. It is included in the differential diagnosis of the catatonic syndrome. Evidence is reviewed to suggest that agents responsible for improving NMS act on the dopamine (DA) gamma aminobutyric acid (GABA) connections in the mesostriatal and mesolimbic systems and also in t…

Catatonia is a neuropsychiatric syndrome characterized by stupor, mutism, negativism, posturing, mannerisms, stereotypies, waxy flexibility, catalepsy, grimacing, echolalia, and echopraxia. This syndrome has a plethora of etiologies that arise from neurological and medical conditions, psychiatric illnesses, and withdrawal of psychoactive substances. Adding to this, it has numerous differentials, including an acute confusional state, which often adds to the diagnostic dilemma. The incidence of catatonia is between 5 and 20%. A comprehensive workup of the patient is required to rule out organic …
