Neurophysiologic markers of primary motor cortex for laryngeal muscles and premotor cortex in caudal opercular part of inferior frontal gyrus investigated in motor speech disorder: a navigated transcranial magnetic stimulation (TMS) study.

Transcranial magnetic stimulation studies have so far reported the results of mapping the primary motor cortex (M1) for hand and tongue muscles in stuttering disorder. This study was designed to evaluate the feasibility of repetitive navigated transcranial magnetic stimulation (rTMS) for locating the M1 for laryngeal muscle and premotor cortical area in the caudal opercular part of inferior frontal gyrus, corresponding to Broca's area in stuttering subjects by applying new methodology for mapping these motor speech areas. Sixteen stuttering and eleven control subjects underwent rTMS motor spee…

[Fluency shaping with surface electromyography: a pilot study].

BACKGROUND: the use of a technological resource in fluency promotion. AIM: to verify the effectiveness of a stuttering treatment based on the use of surface electromyography (SEMG) exclusively. METHOD: participants were four stuttering adults of both genders. Assessment, pre and post-treatment, consisted of a speech gathering session and the analyses of the rest tension and of the reaction time for speech. Treatment consisted of twelve twenty minute training sessions monitored by SEMG. RESULTS: there was a statistically significant reduction in the number of stuttering-like disfluencies (p …

Lidcombe Program telehealth treatment for children 6-12 years of age: A Phase II trial.

BACKGROUND: For children older than 6 years who stutter, there is a gap in clinical research. This is an issue for speech-language pathologists because the tractability of stuttering decreases and the risk of long-term psychological consequences increase with age. PURPOSE: To report a Phase II trial of a telehealth version of the Lidcombe Program with school-age children. METHODS: Participants were 37 children who stuttered, 6-12 years of age, from Australia, New Zealand, Hong Kong, and Singapore. Parents were trained by video telehealth how to deliver the Lidcombe Program to their child. Pr…

Treatment for preschool age children who stutter: Protocol of a randomised, non-inferiority parallel group pragmatic trial with Mini-KIDS, social cognitive behaviour treatment and the Lidcombe Program-TreatPaCS.

Stuttering is a speech disorder in which the flow of speech is disrupted by involuntary repetitions of sounds, syllables, words or phrases, stretched sounds or silent pauses in which the person is unable to produce sounds and sound transitions. Treatment success is the highest if stuttering is treated before the age of 6 years, before it develops into "persistent" stuttering. Stuttering treatment programs that focus directly on the speech of the child, like the Lidcombe Program, have shown to be effective in this age group. Mini-KIDS is also a treatment that focuses directly on the speech of t…

Priapism and sickle-cell anemia: diagnosis and nonsurgical therapy.

INTRODUCTION: Priapism describes a persistent erection lasting longer than 4 hours. Ischemic priapism and stuttering priapism are phenotypic manifestations of sickle-cell disease (SCD). AIMS: To define the types of priapism associated with SCD, to address pathogenesis, and to recommend best practices. SOURCES: Literature review and published clinical guidelines. SUMMARY OF FINDINGS: Priapism is a full or partial erection that persists more than 4 hours. There are three kinds of priapism: ischemic priapism (veno-occlusive, low flow), stuttering priapism (recurrent ischemic priapism), and non…

Long-term effectiveness of the SpeechEasy fluency-enhancement device.

UNLABELLED: The SpeechEasy has been found to be an effective device for reduction of stuttering frequency for many people who stutter (PWS); published studies typically have compared stuttering reduction at initial fitting of the device to results achieved up to one year later. This study examines long-term effectiveness by examining whether effects of the SpeechEasy were maintained for longer periods, from 13 to 59 months. Results indicated no significant change for seven device users from post-fitting to the time of the study (t=-.074, p=.943); however, findings varied greatly on a case-by-c…

Disfluency in Tourette syndrome: observational study in three cases.

It is often stated that stuttering is a common speech disorder in individuals with Tourette syndrome (TS). It has also been suggested, however, that the fluency failures observed in people with TS do not completely conform to the classic pattern of stuttering. The present paper describes the results of an analysis of the speech patterns of three individuals with TS. A picture emerged that bears some resemblance to stuttering, cluttering, and palilalia but that is also different from each of these disfluency types.

Spouses as adjuncts in fluency therapy.

Couples with one partner who stutters completed questionnaires about the fluent partners' awareness of their spouses' stuttering; their general knowledge about stuttering; how fluent spouses had helped their partners; and the nature and benefits of spousal participation in therapy. Findings strongly suggested that spouses may be helpful in therapy. Considerations and suggestions for involving spouses are made on the basis of these findings and those of surveys completed by speech-language pathologists (SLPs) who include spouses in fluency therapy.

Predicting treatment time with the Lidcombe Program: replication and meta-analysis.

BACKGROUND: The benefits of treating stuttering close to onset have become obvious in recent years, and the Lidcombe Program has emerged as an effective and safe treatment method for children in their preschool years. The benefits of implementing the programme with young children, however, need to be weighed against the knowledge that many children recover from stuttering without treatment. In light of this, speech-language therapists need to know how long treatment is likely to take and whether treatment time can be predicted. In particular, they need to know if adopting a 'watchful waiting' …

The "Iowa Way" revisited.

UNLABELLED: This paper attempts to bridge the gap between past and present stuttering therapy approaches. Specifically, the ideas and methods of Wendell Johnson and Dean Williams are compared to current approaches used in treating mental illness and communication disorders in which there is an emphasis on focusing on what is "right" with the individual and spending less time focusing on the "disorder". There is particular emphasis on tying Johnson's and Williams' overall philosophy to modern psychological approaches rooted in "Positive Psychology", to highlight how these early ideas in stutter…

The Lidcombe Program After 35 Years: Empirical, Theoretical, and Social Contexts.

PURPOSE: Thirty-five years ago, the Lidcombe Program was introduced as a new evidence-based treatment for early stuttering. This milestone presents an opportunity to examine the Lidcombe Program and its relevance today. METHOD: Four Lidcombe Program developers, together with 17 members of the Lidcombe Program Trainers Consortium, reflected on the program's historic origins, early reception, current status, and future direction. CONCLUSIONS: This review of the program explores its origins in the context of causal theories of stuttering and its place in the modern clinical context. We point ou…

Protection from harm: the experience of adults after therapy with prolonged-speech.

BACKGROUND: It is well documented that adults can control stuttering if they use certain novel speech patterns referred to generically as prolonged-speech (PS). These speech patterns were refined in the 1960s and developed into behavioural treatment programmes. The bulk of available PS treatment research has focused on speech parameters thought to reflect favourable treatment outcome. Considering this, and that post-treatment relapse is known to be common, clinicians and researchers could be usefully informed by knowledge about the experiences of those who receive these treatments. Subsequentl…

Two-year follow-up of the Lidcombe Program in ten Swedish-speaking children.

PURPOSE: The purpose of this semicontrolled prospective study was to report on the outcomes of 10 Swedish-speaking children treated with the Lidcombe Program and how the parents viewed the treatment. METHOD: Ten children at an age from 2:9 to 5:8 (years:months) referred to two clinics for communication disorders were consecutively included in the study. The children had been stuttering for 0:9 to 3:3 with a mean stuttering frequency of 10.1% syllables stuttered (%SS; 0.8-33.9) before treatment. Parents and teachers filled out questionnaires concerning the children's speech and the parents als…

Handedness and creativity in a sample of homosexual men.

A form concerning handedness, dyslexia, stuttering (self-report), and twinning was included in a study of sexual habits of homosexual men. Questions about interests in music, art, and creative writing were also included. A questionnaire was sent out to male homosexuals. Out of 391 returned forms, 363 were suitable for data processing. Analysis showed 13.9% reported writing with the left hand, 6.5% with stuttering, and 8.6% with dyslexia. The twin incidence was 1.2%. The creativity questions indicated a high music and painting activity. The results confirmed previous studies, showing a left shi…

An investigation into the relationship between parent satisfaction and child fluency in the Lidcombe Program: Clinic versus telehealth delivery.

Purpose: Parents have a vital role in the delivery of the Lidcombe Program when treating stuttering in young children. Research into parent experiences and perceptions throughout the Lidcombe Program is currently limited. Qualitative literature indicates that parent experiences and child progress are closely linked with program duration. This preliminary quantitative study examined parent satisfaction with child fluency at three time points. Data from clinic and telehealth delivery of the Lidcombe Program were used to further explore between-group non-inferiority. Method: The data were drawn f…

Speech disfluencies in children with Down Syndrome.

PURPOSE: Speech and language development in individuals with Down syndrome is often delayed and/or disordered and speech disfluencies appear to be more common. These disfluencies have been labeled over time as stuttering, cluttering or both. FINDINGS: were usually generated from studies with adults or a mixed age group, quite often using different methodologies, making it difficult to compare findings. Therefore, the purpose of this study was to analyze and describe the speech disfluencies of a group, only consisting of children with Down Syndrome between 3 and 13 years of age. METHOD: Parti…
