The article presents radiological signs of basal processes (arachnoiditis, arachnoencephalitis) associated with paranasal sinusitis. gamma-Topography of the brain in inflammatory processes is distinguished by such typical radiological signs as a pathological accumulation of a radiopharmaceutical which is more often diffuse in nature. According to the findings of radionuclide cysternography the inflammatory process is characterized by a block of the subarachnoidal space of the frontobasal portions and an impairment of the CSF dynamics.

The relation between the occurrence of vasospasm and the site of ruptured aneurysm was investigated. Vasospasm tended to occur initially on arteries close to the aneurysm, later extending to other locations. Contralateral spasm was more frequent the closer the site of the aneurysm to the mid-sagittal line. The spasm observed angiographically varies in relation to the site of the aneurysm; the site of the hematoma in the subarachnoid space was supposed to be the cause of such variation.

Temporal bone histopathology is reported on a case of acute suppurative otitis media, acute suppurative labyrinthitis, and meningitis with death of the patient 20 days after stapes surgery. Purulent exudate invaded the vestibule by the oval window through a displaced footplate. The main pathway of infection to the menings was extension along the branches of the vesticular nerve to the internal auditory meatus and to the subarachnoid space.

BACKGROUND: A patient presented with a spinal subarachnoid hemorrhage (SAH) and subdural hematoma (SDH) attributed to a spinal schwannoma at the T12-L1 level.
CASE DESCRIPTION: A 67-year-old male acutely presented with severe back pain and L1 paraparesis/sensory loss, with urinary incontinence. CT/MR studies showed a spinal SAH and SDH within a likely T12-L1 schwannoma. At surgery, the hemorrhage within the tumor was continuous through the lower pole of the tumor into the subarachnoid and subdural spaces; tumor was dissected away from the surrounding tissues and totally removed. The postopera

In this video publication a standardized mouse model of subarachnoid hemorrhage (SAH) is presented. Bleeding is induced by endovascular Circle of Willis perforation (CWp) and proven by intracranial pressure (ICP) monitoring. Thereby a homogenous blood distribution in subarachnoid spaces surrounding the arterial circulation and cerebellar fissures is achieved. Animal physiology is maintained by intubation, mechanical ventilation, and continuous on-line monitoring of various physiological and cardiovascular parameters: body temperature, systemic blood pressure, heart rate, and hemoglobin saturat…

A 51-year-old man presented with a 6-week history of progressive headache, confusion and ataxic gate. The symptoms were not preceded by trauma or lumbar puncture. A CT-scan of the brain revealed bilateral subdural fluid accumulation and hyperdensities in the subarachnoid space. In view of the signs of a subarachnoid haemorrhage, angiography was performed but showed no indications of an aneurysm. An MRI-scan of the head revealed abnormalities in line with intracranial hypotension. CT-myelography of the whole spine revealed a cerebrospinal fluid leak at the level of the fifth and sixth thoracic …

We describe a Cockapoo with a subarachnoid diverticulum (Type III), at the level of L6-7. Magnetic resonance imaging identified a circumferential dilatation of the dural sac, extending from the cranial endplate of L6 to the midbody of L7, containing T2-weighted hyperintense and T1-weighted hypointense material that suppressed on fluid-attenuated inversion recovery sequences, consistent with cerebrospinal fluid. An exploratory dorsal laminectomy confirmed a subarachnoid diverticulum (Type III), and a durotomy was performed. After surgical decompression, full clinical resolution was observed. Th

We present a case of subarachnoid neurocysticercosis caused by Taenia crassiceps in an elderly woman in Slovenia with no underlying disease or immunosuppressive treatment. The parasite was identified by 12S rDNA PCR and sequencing. Despite prolonged therapy with albendazole and praziquantel, the disease recurred after treatment was discontinued.

OBJECTIVES: Idiopathic intracranial hypertension (IIH) is a disease defined by elevated intracranial pressure (ICP) without an established etiology. Arachnoid granulations (AG) are conduits for CSF resorption from the subarachnoid space to the venous system. AG have been implicated to play a central role in maintaining CSF homeostasis. We tested the hypothesis that patients with fewer visible AG on MRI are more likely to present with IIH.
METHODS: In this institutional review board (Institutional Review Board)-approved retrospective chart review study, 65 patients with a clinical diagnosis of…

Cerebrospinal fluid (CSF) production and absorption concept significantly changed in the early 2010s from "third circulation theory" and "classical bulk flow theory" to a whole new one as follows: First, CSF is mainly produced from interstitial fluid excreted from the brain parenchyma, and CSF produced from the choroid plexus plays an important role in maintaining brain homeostasis. Second, CSF is not absorbed in the venous sinus via the arachnoid granules, but mainly in the dural lymphatic vessels. Finally, the ventricles and subarachnoid spaces have several compensatory direct CSF pathways a…

OBJECTIVE: Quadrigeminal cistern arachnoid cysts (QACs) are congenital lesions that can cause pineal region compression and obstructive hydrocephalus when sufficiently large. Management of these cysts is controversial and rates of reintervention are high. Given the limited data on the management of QACs, the authors retrospectively reviewed 20 years of cases managed at their institution and performed a literature review on this topic.
METHODS: The authors performed a retrospective analysis of patients treated for QAC at their institution between 2001 and 2021. They also performed a literature

Idiopathic syringomyelia (IS) and refractory syringomyelia (RS) are types of syringomyelia that often pose a management challenge and are associated with long-term clinical sequela. They are usually an epiphenomenon reflecting an underlying pathology where the treatment of the primary cause should be the aim for any surgical intervention. In the case of IS, the initial step is agreeing on the definition of the terms idiopathic and syringomyelia. After a rigorous exhaustive clinic-radiological workup, only IS patients with progressive neurology are treated, usually unblocking subarachnoid cereb

The distal dural ring (DDR) is a conserved intracranial anatomic structure marking the boundary point at which the internal carotid artery (ICA) exits the cavernous sinus (CS) and enters the subarachnoid space. Although the CS has been well described in a range of anatomic studies, to our knowledge no prior study has analyzed the histologic relationship between the ICA and DDR. Correspondingly, our objective was to assess the relationship of the DDR to the ICA and determine whether the DDR can be dissected from the ICA and thus divided, or can only be circumferentially trimmed around the arter…

Proteus syndrome (PS), a rare hamartomatous disorder, manifests itself in asymmetric and disproportionate overgrowth of multiple body tissues. Because of complexity of the disorder, the anesthetic problems encountered during patients' perioperative management are very varied. We discuss the case of a 14-year-old adolescent boy diagnosed with PS who underwent corrective osteotomy of right knee joint under subarachnoid block. The salient points the anesthetists need to be aware of while caring for patients with PS are highlighted.

The authors compared high-signal-intensity flow-related artifacts present with a conventional two-dimensional (2D) fluid-attenuated inversion recovery (FLAIR) sequence with those seen with a single-slab, three-dimensional (3D) FLAIR sequence. Four readers graded the subarachnoid space and intraventricular artifacts, the pulsation artifacts, and the conspicuity of cranial nerves in the posterior fossa. For all comparisons, differences between 2D and 3D images were highly statistically significant, with 3D imaging being superior in all cases.

A 70-year-old man developed pain and functional weakness in the lower limbs with dysesthesia and urinary retention after subarachnoid anesthesia with 0.5% hyperbaric bupivacaine. Neurological and electrophysiological tests and follow-up, as well as diagnostic images (CAT and MR), ruled out spinal cord lesions. The diagnosis was cauda equina syndrome. Cauda equina syndrome is a neurological complication of subarachnoid anesthesia. Associated with use of microcatheters for continuous subarachnoid anesthesia and 5% hyperbaric lidocaine, cauda equina syndrome is rare after a single injection of bu

Cerebrospinal fluid (CSF) leaks may occur following spinal surgery and are commonly associated with infection. These leaks can be successfully evaluated with retrograde myeloscintigraphy using the same techniques applied in diagnosing CSF otorrhea and rhinorrhea. This report describes the utility of this procedure in differentiating a process due solely to postoperative infection from one that involves both an inflammatory site and a fistulous tract with the subarachnoid space.
