BACKGROUND: The seventh nerve palsy causes loss of function in the affected orbicularis oculi muscle. There is exposure and dessication of the corneal surface and obvious aesthetic changes in facial symmetry.

Seventy-nine patients of bladder neck suspension using an extraperitoneal variation of laparoscopic surgery were performed for the treatment of stress urinary incontinence. Using a balloon dissector the anterior vesical pelvic space is secured. The bladder neck suspension similar to the Burch operation was performed through a laparoscopic procedure. Single doses of 1 gm 5-aminosalicylic acid (5-ASA) suspension was administered to 24 healthy volunteers during both fasting and fed conditions. Bladder neck suspension under local anesthesia is a new technique for the treatment of stress incontinence in females which can be done under private clinic conditions. The procedure is a modification of Stamey-Pereyra operation. It has already been applied in 34 patients with a success rate of 80% with low morbidity. A higher success rate was achieved in patients who had no gynaecological or surgical operations previously. The purpose of the technique that we used on these patients is to restore anatomic position without causing urethral obstruction.

A series of 20 women with stress urinary incontinence underwent bladder neck suspension according to the method in which there are no suprapubic or vaginal incisions and no buttresses. Follow-up was between 9 weeks and 8 months. Twelve patients (60%) were cured, 2 (10%) were significantly improved and 6 (30%) were not improved; 3 patients had pre-operative detrusor instability; when they were excluded the failure rate fell to 23%. Four young girls with myelodysplasia who remained incontinent despite intermittent catheterization and pharmacological manipulation underwent suprapubic bladder neck suspension along with bladder augmentation. Twelve patients with neurovesical bladder dysfunction and genuine stress incontinence were treated by the Stamey operation. The technique is described together with some minor modifications that we have found useful. Results have been encouraging and 11 of the 12 patients have been cured of incontinence.
