Tenosynovial giant cell tumour (pigmented villonodular synovitis-)-like changes in periprosthetic interface membranes.

Tenosynovial giant cell tumour (TSGCT; synonym, pigmented villonodular synovitis (PVNS)) is a rare low-grade mesenchymal neoplasm of either intra-articular or extra-articular origin. The etiopathogenesis of TSGCT is still uncertain, but recent studies showed a translocation involving colony-stimulating factor 1 (CSF-1) gene in a subset of cases. Histological features mimicking TSGCT can sometimes be encountered in periprosthetic interface membranes. To investigate the frequency and morphologic spectrum of this phenomenon, we conducted a systematic analysis of 477 periprosthetic interface membr…

Quantitative assessment of synovitis in Legg-Calvé-Perthes disease using gadolinium-enhanced MRI.

A quantitative method to assess hip synovitis in Legg-Calvé-Perthes disease (LCPD) is not currently available. To develop this method, the areas of synovial enhancement on gadolinium-enhanced MRI (Gd-MRI) were measured by two independent observers. The volume of synovial enhancement was significantly increased in the initial and the fragmentation stages of LCPD (Waldenström stages I and II), with a persistence of synovitis into the reossification stage (stage III). The Gd-MRI method had high interobserver and intraobserver agreements and may serve as a useful method to monitor the effect of va…

Simultaneous bilateral posterior interosseous nerve palsy caused by rheumatoid synovitis of the elbows.

A 60-year-old woman with rheumatoid arthritis (RA) developed bilateral posterior interosseous nerve palsy. Her left side recovered after conservative treatment. However, her right side, which had been affected previously, did not recover and required surgery. Although conservative therapy, including administration of biologic agents, should be conducted for tight control of RA activity, we recommend surgical treatment when there is recurrence of the disease.

Review article: Open versus arthroscopic synovectomy for pigmented villonodular synovitis of the knee.

Studies published in MEDLINE between January 2010 and October 2013 were reviewed to compare open with arthroscopic synovectomy for pigmented villonodular synovitis of the knee in terms of rates of recurrence, complication, and osteoarthritis. 10 studies specifically on this topic were analysed; all were retrospective case reports, and thus level of evidence was low. Respectively after open and arthroscopic synovectomy, the overall recurrence rates were 26.7% and 24.6%, the overall complication rates were 5.7% and 3.2%, and the overall osteoarthritis rates were 20% and 17.1%. Arthroscopic and o…

Synovial fluid differential cell count in wear debris synovitis after total knee replacement.

BACKGROUND: Determining the cause of synovitis following total knee arthroplasty (TKA) can be challenging. The differential diagnoses include infection, hemarthrosis, instability, crystalline disease, wear debris or idiopathic causes. Wear particle synovitis can mimic periprosthetic infection with symptoms of pain and effusion. Radiographs and physical exam are often inconclusive in differentiating the two. Synovial fluid analysis is routinely used in evaluating periprosthetic infections. We examined the association between synovial white blood cell count and differentials, and polyethylene we…

Diagnostic outcomes associated with ankle synovitis in early inflammatory arthritis: a cohort study.

OBJECTIVES: To examine the diagnostic outcomes associated with clinical ankle synovitis in an early inflammatory arthritis cohort. METHODS: Data from the Birmingham early inflammatory arthritis cohort (BEACON) were used to obtain information about baseline disease and demographic variables and diagnostic outcomes at 18 month follow-up. The prevalence of clinical ankle synovitis (defined as joint swelling on examination) was calculated. Relative risk (RR) and 95% confidence interval (95% CI) were used to estimate whether clinical ankle synovitis at baseline predicts diagnostic outcomes indepen…

Pigmented villonodular synovitis: a retrospective study of seventy five cases (eighty one joints).

PURPOSE: Pigmented villonodular synovitis (PVNS) is a relatively rare, benign proliferation lesion of the synovium of large joints, but there is not much information available about the disease's aetiology, clinical history, differential diagnosis, treatment, and long-term effects. We conducted a study to analyse these aspects of PVNS. METHODS: We reviewed all clinical data for 75 patients with PVNS (81 joints) who were treated either by synovectomy alone or synovectomy plus arthroplasty. RESULTS: In all cases, the diagnosis of PVNS was confirmed by pathological examination. The ratio of mal…

Remitting seronegative symmetrical synovitis with pitting edema syndrome in a chronic hemodialysis patient.

A 75-year-old male who was undergoing chronic hemodialysis developed abrupt-onset pitting edema and pain in the dorsum of both hands and feet. Biochemical analysis disclosed increased C-reactive protein, and negative rheumatoid factor and antinuclear antibody. Radiological examination showed no bony erosion. Computed tomography and gallium scintigraphy revealed no active infection or neoplasms. The clinical diagnosis was remitting seronegative symmetrical synovitis with pitting edema (RS(3)PE) syndrome. The pitting edema and inflammatory response quickly subsided after low-dose prednisolone th…

Synovitis induced by joint lavage with hypertonic saline solutions in healthy dairy calves.

The objective of this study was to evaluate the effect of a single joint lavage with 7.2% or 15% hypertonic saline solutions (HSS) on the tarsocrural joints of healthy calves. The tarsi of 10 calves were randomly lavaged with 7.2% HSS, 15% HSS, or isotonic saline. Synovial fluid samples were collected aseptically on days 1 (before joint lavage), 2, 3, 4, and 8 for complete cytological analysis. Lameness, joint swelling, and pain were recorded daily. Calves were euthanized on day 8 for gross and histological analyses of synovial membranes and articular cartilage. Synovitis was evaluated using a…

Intra-articular chromic phosphate (³²P) in the treatment of diffuse pigmented villonodular synovitis.

PURPOSE: Pigmented villonodular synovitis (PVNS) is an uncommon proliferative lesion of synovial tissue. In diffuse PVNS, recurrence rates are high after resection alone. Adjuvant external beam radiation therapy contributes to improved local control. Limited data exist for intra-articular radioisotope therapy after surgical resection. We report institutional experience with intra-articular chromic phosphate ((32)P). METHODS AND MATERIALS: Records were reviewed from the Department of Radiation Oncology at Indiana University. Nine cases of PVNS treated with (32)P were identified (mean age=40). …

Simultaneous pigmented villonodular synovitis and synovial chondromatosis of the temporomandibular joint: case report.

Pigmented villonodular synovitis and synovial chondromatosis are benign nonneoplastic proliferations originating in the synovium of unknown cause. Involvement of the temporomandibular joint (TMJ) is rare. Only 3 cases of simultaneous pigmented villonodular synovitis and synovial chondromatosis involving the TMJ have been reported. The authors report the case of a 21-year-old female with simultaneous involvement of pigmented villonodular synovitis and synovial chondromatosis in the left TMJ. Clinical examination and magnetic resonance imaging did not reveal the synovial lesions, and the diagnos…

Acne fulminans with synovitis-acne-pustulosis-hyperostosis-osteitis (SAPHO) syndrome treated with infliximab.

Acne fulminans is a syndrome of sudden onset hemorrhagic and ulcerative acne involving the back, chest, and face combined with systemic symptoms. It can be the dermatologic manifestation of the synovitis-acne-pustulosis-hyperostosis-osteitis (SAPHO) syndrome. Current therapy for acne fulminans consists of wound care, topical and systemic corticosteroids, isotretinoin, and nonsteroidal anti-inflammatory drugs (NSAIDs). Infliximab, a recently developed monoclonal antibody against tumor necrosis factor-alfa, has shown efficacy in the treatment of psoriatic arthritis and ankylosing spondylitis bot…

Intracarpal synovitis related to Dacron interposition after trapeziectomy: a report of three cases.

To prevent the mechanical consequences of trapeziectomy, interposition devices are promoted, such a Dacron spacer. We report three cases of osteolysis and synovitis related to the use of such a device. This phenomenon occurred 4 months after insertion and required revision surgery after 9 months of follow-up because of pain and extensive osteolysis of the carpal bones. Revision consisted of an extended synovectomy, removal of the broken Dacron followed by a ligamentoplasty using the Flexor Carpi Radialis. Two years after revision surgery the result was satisfactory, with no recurrence of pain …
