We report a case of a 69-year-old man who developed tetraparesis and muscular pain under the therapy of prednisolone for several months. Diagnosis was sepsis due to pyomyositis with multiple septic pulmonary staphylococcus aureus abscesses. Antibiotic therapy with piperacillin and tazobactam resulted in a decrease of the inflammatory factors and improvement of the tetraparesis. Pyomyositis, common in tropical areas, is a suppurative infection of striated muscle. Immunodeficiency has been implicated in the development of pyomyositis in temperate climates.

This report highlights a case of severe hyponatraemia secondary to excessive sweating and salt [corrected] poor fluid consumption and low salt diet in hot conditions. The case was complicated by the presence of marked hypokalaemia caused by secondary hyperaldosteronism confirmed, for the first time, by the presence of grossly elevated serum renin and aldosterone concentrations. With the rise in global temperature affecting even temperate climates doctors, especially in acute and general medicine, may be faced with this condition more often.

Attention-deficit hyperactivity disorder (ADHD) is probably the most common chronic undiagnosed psychiatric disorder in adults. It is characterized by inattention and distractibility, restlessness, labile mood, quick temper, overactivity, disorganization, and impulsivity. It is always preceded by the childhood diagnosis, a disorder that is rarely inquired about, and usually overlooked. Treatment can be dramatically effective. Approximately two thirds of adult ADHD patients experience moderate-to-marked improvement with psychoeducational management and stimulant drug therapy.

The lack of therapeutic progress, coupled with limitations in early breast cancer diagnostic surveillance, suggest the need for innovative control programs for this disease. We have provided certian practical criteria for prophylactic subcutaneous mastectomy based on family history, presence of precancerous diseases of the breast, and other breast cancer risk factors, and the attitudes, feelings, and emotional aspects of the patient, all of which temper decisions for the type of cancer surveillance program and/or prophylactic surgery.

Strongyloidiasis is a ubiquitous parasitic infection mostly endemic in tropical and subtropical regions, although sporadic autochthonous cases or sparse pockets have been reported in some areas of temperate regions. We report here the first, to our knowledge, autochthonous case diagnosed in the Arcachon basin in the coastal Aquitaine region of South-Western France. We failed to identify any recognized risk factor for transmission in this case of autochthonous infection occurring in a wealthy major tourist destination.

The clinical manifestations of equine pituitary pars intermedia dysfunction (PPID) in temperate climates are well described. The classic presentation is that of an older animal with hirsutism, laminitis, poor muscle tone, pendulous abdomen and weight loss. This case series highlights the additional clinical signs of anhidrosis and heat stress with secondary exercise intolerance that were seen as primary presenting problems in equids with PPID in the hot, humid conditions of a tropical climate. The clinical signs resolved with medical treatment for PPID.

Babesiosis is an emerging, tick-transmitted, zoonotic disease caused by hematotropic parasites of the genus Babesia. Most cases of Babesial infections in humans have been acquired in temperate regions of the United States, Europe, France and England. A few cases of Babesiosis have been described in other parts of the world, including China, Taiwan, Egypt, South Africa, and Mexico.1,2 We report the first case of human Babesiosis, in a normosplenic, previously healthy individual from India.

Histoplasmosis is a deep mycosis which occurs in America and Africa. It is extremely rare in France. The case presented involves a male subject who had spent time in the French colonies of Africa and in Brazil. He presented with undiagnosed ulceration of the cheek, similar to that seen with cancer. Diagnosis was made on histology on one hand and on mycological examination on the other. This case serves to remind us that tropical and sub-tropical pathology may be encountered in temperate zones due to population migration.

Even in temperate zones of the earth such as Middle Europe quite a number of men have to work in very hot dry climates, particularly in salt mines. Problems of evaluation stress under these condition are discussed. Climatic chamber experiments at ambient temperatures mainly between 45 and 65 degrees C were performed in order to find out various possibilities to reduce heat strain. New methods are discussed for reducing extremely elevated body core temperatures.

The microbiology of 12,174 symptomatic ears presenting over an eight-year period to one hospital was examined in detail. 1,061 cases of fungal infection were found. The incidence and pattern of these fungi and the accompanying bacteria, in cases of mixed infection, do not appear to have been affected by the widespread use of powerful topically-applied antibiotic/steroid preparations over more than two decades. Otomycosis remains an uncommon, often overlooked but important cause of morbidity in a temperate climate.

Vast travelling nowadays into warm subtropical and tropical regions demands more knowledge about infectious diseases outside our temperate climate. A leishmaniasis infection in Europeans is no rarity today because of its vast distribution in Africa. Cutaneous leishmaniasis is now considered a spectral and polar disease not unlike leprosy, caused by the leishmania parasite but also influenced by the host's immune response and intolerance. Therefore, diagnosis and treatment of a cutaneous leishmaniasis request an immunopathohistological view of the case.

Between August and 28 October 2024, 199 autochthonous cases of dengue virus serotype 2 were notified in the city of Fano, central Italy. We describe the ongoing epidemiological and microbiological investigation and public health measures implemented to contain the outbreak. The high transmissibility and the extension of the outbreak suggest that dengue should be expected in temperate regions during favourable seasons, highlighting the need for heightened awareness among healthcare providers and the public to ensure timely detection and response.

Pyomyositis is a rare condition in temperate climates. We present a case of Methicillin Resistant Staphylococcus aureus pyomyositis of the shoulder complicated by multifocal lung infiltrations, treated successfully with antibiotic therapy. After excluding shoulder septic arthritis, a low threshold of suspicion for the diagnosis of shoulder pyomyositis should be applied to patients with persistent fever, pain, and decreased range of shoulder motion. A prompt diagnosis and a rapid rise in antibiotic therapy are important to avoid local and systemic complications.

We present a case which highlights several areas of concern relating to the prevention and management of varicella in Ireland. We review the pathophysiology of this virus and highlight its greater potential for morbidity in certain groups, most particularly adult males. The experience and opinions with regard to varicella vaccination in the US and other temperate countries is reviewed along with evidence of changing epidemiology of varicella infection. The National Immunisation Advisory Committee (NIAC) guidelines are reviewed in the context of our experience.

Exfoliative toxin A (ETA) causes staphylococcal scalded-skin syndrome in children. The gene for ETA was believed to be coded by the chromosomal DNA. We isolated temperate phages from an ETA-producing strain, ZM, using a restriction minus strain, 1039, as an indicator. One of the prophages, designated phi-ZM-1 mediated lysogenic conversion of ETA. The polymerase chain reaction assay of the eta gene revealed that phage phi-ZM-1 carries the structural gene for ETA.

Pharmacotherapy, in conjunction with psychosocial interventions, is emerging as a valuable tool for alcohol dependence treatment. OBJECTIVE: On the basis of several controlled clinical investigations the cost-effectiveness of acamprosate as adjuvant therapy of alcohol-dependent patients has yet been evaluated. These optimal conditions cannot be found in the daily ambulant practice and results in asking which of the alternatives, "standard plus acamprosate" or "standard without acamprosate", is more cost-effective in maintaining abstinence in alcohol dependent patients under realistic conditions.
