The treatment of extensor tendon injuries of the hand is based on Verdan's division of the different sections into eight zones. Taking the anatomic characteristics of those zones into consideration, good functional results can be achieved in most cases by differentiating between indications for operative or conservative treatment. The most frequent extensor tendon injury of the hand is the closed tendon rupture over the DIP joint. In almost all cases it can be completely cured by immobilization for at least 8 weeks. Open extensor tendon injuries on the other hand require surgical therapy. This

OBJECTIVE: To explore the clinical application of allogeneic tendon handled by ultra-deep-low-temperature.
METHODS: From March 1995 to June 2002, tendons rupture and tendons defect were repaired by ultra-deep-low-temperature allogeneic tendon grafts in 96 cases, including 51 cases of flexor or extensor tendon defect of hand, 36 cases of coracoclavicular ligament rupture, 4 cases of cruciate ligament rupture, 2 cases of calcaneus tendon rupture and 3 cases of extensor tendon defect of foot. The interval between injury and operation was 1 day to 5 months. The length of defect was 4-12 cm with a…

Wrist injuries in the gymnast are due to the transformation of the upper extremity into a weight bearing entity. Gymnast wrist pain presents a difficult diagnostic and therapeutic challenge. Here, we present a new case of extensor tendon impingement in an elite gymnast. To our knowledge, there is no similar report in the literature.

Posterior tibial tendon dysfunction is the most common cause of acquired flatfoot in adults. The posterior tibial tendon is the primary dynamic stabilizer of the medial longitudinal arch of the foot. Various histological alterations in the tendon substance disrupt the normal linear structure of the collagen fibers. As a result the tendon weakens, elongates, tears and its function is compromised. This in turn places the static stabilizers of the arch under excessive stress, which eventually leads to their collapse and formation of pes planovalgus deformity. Many etiologies have been proposed as

We present a case of synovial chondromatosis originating from the quadriceps tendon sheath, which caused a complete quadriceps tendon rupture. The patient was treated using marginal excision. The ruptured quadriceps tendon was repaired. This is the first description of a quadriceps tendon rupture associated with synovial chondromatosis.

Frequently used zone 2 flexor tendon repair splints are reviewed and classified according to the American Society of Hand Therapists' Splint Classification System. These splints both restrict and mobilise digital motion and fall into two main groups: (1) splints that incorporate the wrist and digital joints as primary joints to allow predetermined increments of early passive or active motion at both the wrist and digital joints; and (2) splints that include the wrist as a secondary joint and the digital joints as primary joints, allowing early passive or active motion at digital joints but not

Closed rupture of the tendon of tibialis anterior is an infrequently reported entity. A thorough review of the literature was performed with 49 reported cases of this condition identified. Those patients who were treated conservatively experienced late sequelae, including continued slapping of the foot, mild to moderate flatfoot deformity, and ankle arthrosis. Low morbidity associated with surgical reconstruction should preclude only the most sedentary of patients from operative treatment.

Posterior tibial tendon dysfunction is the most common cause of acquired flatfoot deformity in adults. Although this term suggests pathology involving only the posterior tibial tendon, the disorder includes a spectrum of pathologic changes involving associated tendon, ligament, and joint structures of the ankle, hindfoot, and midfoot. Early recognition and treatment is the key to prevention of the debilitating, long-term consequences of this disorder. Conservative care is possible in the earliest stages, whereas surgical reconstruction and eventually arthrodeses become necessary in the latter

Dislocation of the posterior tibial tendon has been reported rarely in the English literature. We report a case of traumatic dislocation of the posterior tibial tendon. The diagnosis was delayed for months and was ultimately made by magnetic resonance imaging, which accurately determined the location of the tendon. At the time of surgical exploration, no predisposing anatomic abnormalities were noted. A new pseudoretinaculum had developed that prevented recurrent subluxation or dislocation. Surgical relocation of the tendon and repair of the retinaculum resulted in a good outcome.

Until recently, extensor tendon injuries were often discounted as an important hand problem. However, studies have shown that not all extensor lacerations fare well and that loss of flexion can be problematic. Newer postoperative protocols emphasizing tendon gliding have improved results, and better repair techniques and postoperative rehabilitation regimens are under investigation. This article reviews the evaluation of acute open and closed extensor tendon injuries, their conservative and surgical treatment, and postoperative rehabilitation options.

An anatomic cadaver study was performed and subsequently, in a prospective study, diagnostic and therapeutic tendoscopy (tendon sheath endoscopy) was performed in 16 consecutive patients with a history of persistent posteromedial ankle pain for at least 6 months. All patients had pain on palpation over the posterior tibial tendon, a positive tibial tendon resistance test, and local swelling. The indications were diagnostic procedure after surgery in 5 patients, diagnostic procedure after fracture in 5, diagnostic after trauma in 1, chronic tenosynovitis in 2, screw removal in 1, and posterior

In densely scarred digits, tendon reconstruction fails because of paucity of blood supply. This inevitably leads to adhesion formation and failure of gliding. Staged tendon grafting with silicone spaces can improve results but does not ameliorate the essential problem of reduced vascularity and may indeed worsen it. Tendon grafts can be implanted with their own living sheaths as free or pedicled vascularised transfers from the foot or forearm to either extensor or flexor sites in the hand. These mesenteric tendons with their fascial or synovial sheaths intact and vascularised by adjacent vascu…

In order to determine the diagnostic value of the electromyographically tested tendon reflexes (ETR) in lumbosacral radiculopathies, we compared the latency and amplitude of the knee (EKTR) and Achilles tendon reflex (EATR) to their clinical testing and to the EMG and H reflex values in 200 non selected patients (126 men, 74 women) aged 24-68 years (48.6 +/- 9.4). The control group was composed of 50 healthy volunteers (28 men, 22 women) aged 25-60 yrs (44 +/- 8.5). The combined electrophysiological reflex examination was abnormal in 45% of the affected limbs, being the most frequent pathologi

Extensor tendon injuries are often initially evaluated and diagnosed in the Emergency Department. These injuries may be painfully obvious or frightfully subtle, but if the emergency physician has a firm understanding of the anatomy involved and he or she embarks on a careful and complete history and physical examination, the diagnosis should be made. There are various considerations in the ultimate treatment and outcome for the patient, and the patient's best interests are served when the emergency physician provides the initial appropriate treatment and proper disposition.

Achilles tendon ruptures can be treated nonsurgically in the nonathletic or low-end recreational athletic patient, particularly those more than 50 years of age, provided the treating physician does not delay in the diagnosis and treatment (preferably less than 48 hrs and possibly less than 1 week). The patient should be advised of the higher incidence of re-rupture of the tendon when treated nonsurgically. Surgical treatment is recommended for patients who are young and athletic. This is particularly true because the major criticism of surgical treatment has been the complication rate, which h

Overuse injuries of the Achilles tendon are common in patients engaging in recreational athletics. Achilles tendon overuse injuries exist as a spectrum of diseases ranging from inflammation of the paratendinous tissue (paratenonitis), to structural degeneration of the tendon (tendinosis), and finally tendon rupture. Factors known to predispose patients to Achilles tendinitis include inadequate stretching, training errors, mechanical malalignment of the lower extremities, rigid training surfaces, and occasionally systemic disease. Treatment of the patient with paratenonitis and tendinosis is in
