OBJECTIVES: The hospital setting is often perceived as slow to change. While employee-driven approaches offer a promising alternative to traditional top-down methods, guidance is limited. This study provides a description and formative evaluation of an employee-driven working group (WG) approach to tailor ward-specific measures to improve care in the dying phase. The aim is to evaluate the WG process and offer practical insights for transferability to other hospitals.
METHODS: Formative mixed-methods evaluation of a WG process to tailor ward-specific evidence-informed measures on 10 wards out

INTRODUCTION: Support staff within social care settings have expressed a need for resources to facilitate end-of-life care planning with people with intellectual disabilities. This study aimed to co-design a preliminary toolkit of end-of-life care planning approaches and resources that can be implemented in adult social care services for people with intellectual disabilities.
METHODS: An adapted Experience-Based Co-Design process was applied to develop a toolkit for end-of-life care planning with people with intellectual disabilities. A co-design group (the 'All Together Group') met six times

A 72-year-old man with metastatic pancreatic cancer was admitted to the ICU with increased oxygen demand and confusion, likely related to pulmonary metastases. In the presence of his son, the health care agent, and the team, the patient requested to be do not attempt resuscitation and do not intubate status before losing decision-making capacity. When the patient's brother and another son heard of the code status change, they insisted on a return to Full Code. Although the youngest son (the health care agent) was present for the patient's request to be do not attempt resuscitation/do not intub

BACKGROUND: Previous data indicate major differences between countries and settings regarding the intention when administering sedative drugs at the end of life and the perception, which drugs are sedating. Therefore, we aimed to explore the concept of 'sedative drugs' and the intentions of German healthcare professionals in general palliative care when administering sedative drugs at the end of life.
METHODS: Semi-structured qualitative interviews with physicians and nurses (n = 49). Recruitment took place via contact persons in five hospital departments (haematology/oncology (n = 2), neurol

Trial tested effect of advance care planning on family/surrogates' understanding of patients' end-of-life treatment preferences longitudinally. A multisite, assessor-blinded, intent-to-treat, parallel-group, randomized controlled clinical trial in five hospital-based HIV clinics enrolled 449 participants aged 22 to 77 years during October 2013-March 2017. Patients living with HIV/family dyads were randomized at 2:1 ratio to 2 weekly ~ 60-min sessions either ACP (n = 155 dyads)-(1) ACP facilitated conversation, (2) Advance directive completion; or Control (n = 68 dyads)-(1) Developmental/relati

Malnutrition and the broad spectrum of cancer cachexia frequently occur in patients with malignant disease of all tumour stages and impact on survival and quality of life of patients. Structured screening for the risk of malnutrition with validated tools and nutritional assessment are the prerequisite for adequate nutritional support in cancer patients. In patients receiving tumour directed therapy, the patients diet should meet the requirements to give optimal support, while later on comfort feeding is part of symptom focused palliation. The basis of nutritional support in a malnourished pati

Donation after cardiac death (DCD) remains controversial in some pediatric institutions. An evidence-based, consensus-building approach to setting institutional policy about DCD can address the controversy openly and identify common ground. To resolve an extended internal debate regarding DCD policy at Children's Hospital Boston, a multidisciplinary task force was commissioned to engage in fact finding and deliberations about clinical and ethical issues in pediatric DCD, and attempt to reach consensus regarding the development of a protocol for pediatric DCD. Issues examined included values an

PURPOSE: The contribution of bioethics to clinical care at the end of life (EOL) deserves critical scrutiny. We argue that researchers have rarely questioned the normative power of autonomy-based bioethics practices. Research on the ethical dimensions of EOL decision making has focused on an idealized discourse of patient "choice" that requires patients to embrace their dying to receive excellent palliative care.
DESIGN AND METHODS: Our critique is based on a comprehensive review of empirical research exploring bioethics practices at the EOL. In addition we will provide a brief review of our

Four patients with advanced cancer were referred to the Pain Team because of refractory pain symptoms. They received intravenous barbiturates prior to death. All attempts at palliative treatment had been exhausted. The symptoms were described by patients and family as unendurable. After adding intravenous thiopental, relief and tranquility were achieved. All four patients died peacefully. The need to sedate to a state of unconsciousness or semi-consciousness to relieve symptoms in dying cancer patients is extremely rare. After all viable alternatives have been explored, titrated barbiturate in

Patients with terminal cancer may have a series of severe and dehumanizing physical and psychologic symptoms. To improve symptom control in the final days and hours of life, we administer intravenous barbiturates continuously to provide heavy sedation or continuous somnolence. Titrated dosage is then reduced to a minimum, after a desired steady-state has been achieved. Improved symptom control is provided, and the patient's dignity is maintained until death.

BACKGROUND: Palliative care is required for all life-threatening diagnoses, aiming to prevent and reduce patients' experienced total pain, hence improving the quality of life for both patients and their relatives. Providing palliative care at home and facilitating home deaths is challenging. Norwegian authorities recognise insufficient competence among healthcare professionals as a factor within this area.
AIM: To elucidate how healthcare professionals experience their utilisation of individual competencies when providing palliative end-of-life care at home.
METHOD: A qualitative study was p

BACKGROUND: In late stages, dementia is a palliative diagnosis and the majority of those affected are cared for in nursing homes, but some manage in ordinary housing. The aim was to study how these two groups differ as regards clinical characteristics, healthcare utilization and costs.
METHODS: A registry study (Region Stockholm) was performed on deceased patients (≥ 18 years) with a dementia diagnosis during their last year of life, for the years 2015-2023. Pairwise comparisons between nursing homes and ordinary housing were made and multivariable binary logistic regression models were creat

BACKGROUND: As the aging population grows, the care provided to patients with dementia at the end of life represents a critical area of geriatric and palliative care. This study aimed to describe the care provided to hospitalized patients with dementia who died during their hospital stay.
METHODS: A retrospective cohort study was conducted at a teaching hospital in Sydney, Australia. The study included patients with dementia who died during hospitalization. Data were collected on demographic characteristics, clinical management, and documentation of key care processes, including advance care

CONTEXT: Palliative care has expanded to include noncancer populations, yet disparities in hospice access and end-of-life (EOL) care quality between cancer and noncancer patients remain.
OBJECTIVES: To examine 10-year trends and determinants of hospice use and aggressive EOL care among cancer and hospice-eligible noncancer decedents in Taiwan.
METHODS: This retrospective study included 15,546 adult inpatients who died or were terminally discharged from a tertiary center between 2010 and 2019. Patients were classified as cancer or hospice-eligible noncancer based on Taiwan's National Health I

BACKGROUND: Palliative care interventions are underutilized in infants due to a knowledge deficit among providers surrounding neonatal end-of-life care and a pervasive lack of guidelines in neonatal intensive care units (NICUs). The aim of this study was to increase end-of-life symptom management by 50% and improve staff attitude scores and awareness of end-of-life cases by 20% by August 2024.
METHODS: The Model for Improvement was used to examine the baseline of mortalities and test interventions to increase analgesia administration and improve staff attitude scores and awareness during end-

PURPOSE: Hospice is underutilized by patients with hematologic malignancies (HM), and when patients are referred, they are typically more ill, hospitalized, and with shorter length of stay (LOS) than patients with solid tumors (ST), limiting research about home hospice care experiences of patients with HM. In this mixed-methods study, we examined the hospice experiences of patients with HM who died at residential care homes (RCHs), home-based settings in which volunteer caregivers and hospice staff provide end-of-life (EOL) care under the social hospice model.

The global burden of human death after experiencing serious health-related suffering (SHRS) requiring palliative care (PC) and end-of-life care (EOLC) is increasing. The largest increase in SHRS, death situations, PC, and EOLC needs continues to be in developing countries with limited access to PC. It is critical that nursing human resources in countries with limited access to PC are well trained and have adequate knowledge, attitudes, and skills to provide PC and EOLC. This study aimed to compare the PC knowledge and attitudes toward EOLC of future nursing human resources in Oman, India, and

Intensive care medicine symbolizes a form of medicine that seeks to prevent or overcome illness and life-threatening situations in order to enable a life that is acceptable to the patient. Despite the curative goal, dying and death are unavoidable. For ethically founded intensive care medicine, treatment goals must be agreed upon. The use of all resources is justified by their alignment with these goals and their desire for the patient. The indication and patient consent must be continually reviewed. If the goals are no longer achievable or desired by the patient, dying must be permitted. Then
