Pressure injury to the chest wall caused by vascular graft and endograft after thoracic and thoraco-abdominal aortic repair.

Thoracic and thoraco-abdominal aortic repair remains a challenging issue in vascular surgery, and long-term complications are well described. We report 2 cases of octogenarians with unusual long-term complications after open and endovascular thoracic aortic repair of chest wall pressure injury from the vascular prosthesis. In the first case, a computed tomographic scan of an 80-year-old man showed a pressure injury of the chest wall and rib erosion caused by a Dacron graft. The second case was an 81-year-old woman who presented with external bleeding from the left posterior chest wall. A compu

Long Thoracic Nerve Injury Caused by Overhead Weight Lifting Leading to Scapular Dyskinesis and Medial Scapular Winging.

Scapular winging represents a rare phenomenon that most commonly results from nerve damage to either the long thoracic nerve, spinal accessory nerve, or, less commonly, the dorsal scapular nerve. This injury results in an abnormal scapulohumeral interaction during kinetic motion known as scapular dyskinesis. In this case report, the patient presented with scapular dyskinesis and medial scapular winging caused by overhead weight-lifting exercises, and a long thoracic nerve injury was diagnosed. Physicians are encouraged to consider long thoracic nerve damage in a patient with a history of repet

Thoracic Duct Chylous Fistula Following Severe Electric Injury Combined with Sulfuric Acid Burns: A Case Report.

BACKGROUND As patients with thoracic duct injuries often suffer from severe local soft tissue defects, integrated surgical treatment is needed to achieve damage repair and wound closure. However, thoracic duct chylous fistula is rare in burn patients, although it typically involves severe soft tissue damage in the neck or chest. CASE REPORT A 32-year-old male patient fell after accidentally contacting an electric current (380 V) and knocked over a barrel of sulfuric acid. The sulfuric acid continuously poured onto his left neck and chest, causing combined electrical and sulfuric acid burn inju

Idiopathic thoracic spinal cord herniation: retrospective analysis supporting a mechanism of diskogenic dural injury and subsequent tamponade.

BACKGROUND AND PURPOSE: tSCH in the absence of spinal trauma or surgery is a rare disorder for which numerous mechanisms have been proposed. Here, we have conducted an analysis of images in all published reports of idiopathic tSCH and identified evidence supporting a pathogenesis in which anterior dural erosion at thoracic levels generates a CSF leak that pushes adjacent spinal tissue to tamponade the dural defect, causing progressive myelopathy. Additionally, we describe a case of tSCH in which postural headache was a significant symptom before myelopathy. This finding suggests that tSCH path

Severity of locomotor and cardiovascular derangements after experimental high-thoracic spinal cord injury is anesthesia dependent in rats.

Anesthetics affect outcomes from central nervous system (CNS) injuries differently. This is the first study to show how two commonly used anesthetics affect continuously recorded hemodynamic parameters and locomotor recovery during a 2-week period after two levels of contusion spinal cord injury (SCI) in rats. We hypothesized that the level of cardiovascular depression and recovery of locomotor function would be dependent upon the anesthetic used during SCI. Thirty-two adult female rats were subjected to a sham, 25-mm or 50-mm SCI at T3-4 under pentobarbital or isoflurane anesthesia. Mean arte

Multiple thoracic vertebral compression fractures caused by non-accidental injury: case report with radiological-pathological correlation.

We report a 21-month-old boy with multiple contiguous thoracic vertebral compression fractures involving eight vertebral bodies, attributable to non-accidental injury. No subluxation was associated, however, there was extensive injury to the upper cervical and lower lumbar regions of the spinal cord. Anterosuperior beaking, thought to represent a previous injury, was evident in a mid-lumbar vertebra. Clinical examination revealed bilateral retinal hemorrhages and retinoschisis. Death occurred as a result of severe brain edema with bilateral subdural and subarachnoid hemorrhages. Radiological-p

Early ischemic preconditioning without hypotension prevents spinal cord injury caused by descending thoracic aortic occlusion.

OBJECTIVE: Postoperative neurologic deficits after thoracic aortic reconstruction vary widely. Our previous study showed that delayed ischemic preconditioning could prevent spinal cord injury caused by occlusion of the descending thoracic aorta in pigs. We investigated early ischemic preconditioning in the same model. METHODS: Twenty-eight pigs were divided into 4 groups: group 1 (n = 6) underwent a sham operation, group 2 (n = 6) underwent aortic occlusion for 20 minutes, group 3 (n = 8) underwent aortic occlusion for 35 minutes, and group 4 (n = 8) underwent aortic occlusion for 20 minutes

Update: Fatal and severe liver injuries associated with rifampin and pyrazinamide for latent tuberculosis infection, and revisions in American Thoracic Society/CDC recommendations--United States, 2001.

During February 12-August 24, 2001, a total of 21 cases of liver injury associated with a 2-month rifampin-pyrazinamide (RIF-PZA) regimen for the treatment of latent tuberculosis infection (LTBI) was reported to CDC. These 21 cases are in addition to two previously reported RIF-PZA-associated cases. Cases of liver injury have occurred each year since 1999. CDC also received reports of 10 cases associated with other LTBI treatment regimens; however, risk for liver injury cannot be compared among treatment regimens in part because the number of patients treated for LTBI with each treatment regim

A meta-analysis of the diagnostic accuracy of chest ultrasound for the diagnosis of occult penetrating cardiac injuries in hemodynamically stable patients with penetrating thoracic trauma.

BACKGROUND: We performed a systematic review (SR) and meta-analysis (MA) to determine the diagnostic accuracy of chest ultrasound (US) compared with a pericardial window (PW) for the diagnosis of occult penetrating cardiac injuries in hemodynamically stable patients with penetrating thoracic trauma. METHODS: A literature search in five databases identified relevant articles for inclusion in this SR and MA. Studies were eligible if they evaluated the diagnostic accuracy of chest US, compared with a PW, for the diagnosis of occult penetrating cardiac injuries in hemodynamically stable patients

Increasing Trends of Pediatric Thoracic and Lumbar Spine Fractures in the United States from 2004 to 2023: A 20-year National Injury Review Depicting Shifts in Mechanisms of Injury.

BACKGROUND: Fractures of the thoracic and lumbar spine are uncommon injuries in children and adolescents, but they carry substantial morbidity and a real risk of death. Compared with the adult spine, the pediatric spine has unique anatomic and biomechanical properties that yield different fracture patterns than the adult population. The mechanisms of injury and degree of skeletal maturity constitute important predictors of the outcomes of pediatric thoracic and lumbar fractures; however, there remains a paucity of meaningful epidemiologic data with specific attention to the pediatric spine. Acute cardio-centric haemodynamic management preserves chronic cardiovascular function in pigs with high-thoracic spinal cord injury.

Chronic high-level spinal cord injury (SCI) leads to a complex phenotype of long-term cardio-autonomic dysregulation. In the initial hours post-injury, cardio-centric management with dobutamine (DOB) can optimize cardiovascular haemodynamics and mitigate secondary injury compared with standard vasopressor-based management (i.e. norepinephrine, NE) in a porcine model of T2 SCI; however, any potential long-term benefits remain to be determined. We therefore sought to assess whether acute DOB treatment could mitigate cardiovascular dysfunction in the chronic setting of high-level SCI. Seventeen Y

Physiologic phenotypes in blunt thoracic aortic injury: implications for risk stratification and surgical decision-making using machine learning.

INTRODUCTION: Blunt thoracic aortic injury (BTAI) is rare but highly lethal. Despite advances such as thoracic endovascular aortic repair (TEVAR), management remains challenging due to heterogeneous physiologic presentations. Injury grade alone may not capture early derangements influencing risk. In other acute syndromes, machine-learning-derived physiologic phenotypes have informed prognosis. We applied this approach to BTAI to identify phenotypes, examine outcomes, and use explainable AI (XAI) to characterize their physiologic drivers. METHODS: We analyzed 1,375 patients from the Aortic Tra

Intravenous lidocaine reduces systemic inflammation but not myocardial injury following thoracic surgery for lung cancer: a randomized controlled trial.

BACKGROUND: Elevated high-sensitivity troponin T levels shortly after noncardiac surgery are closely linked to myocardial injury, a key factor in 30-day postoperative mortality. Intravenous lidocaine, known for its potent anti-inflammatory and membrane-stabilizing properties, has shown cardioprotective potential in other surgical settings, but its efficacy in noncardiac thoracic surgery remains unclear. OBJECTIVES: This study was a double-blind, placebo-controlled randomized trial. Participants were randomly allocated to the lidocaine or placebo group with a 1:1 ratio. DESIGN: Single-centre, d

Aberrant Left Vertebral and Aberrant Right Subclavian Arteries in Blunt Thoracic Aortic Injury Patients: A Multicenter Registry Study.

BACKGROUND: Congenital aortic arch variants may complicate thoracic endovascular aortic repair (TEVAR) for blunt thoracic aortic injury (BTAI), particularly when left subclavian artery (LSA) coverage is required. While bovine arch anatomy has been studied, the clinical relevance of rarer variants such as aberrant left vertebral (ALV) artery and aberrant right subclavian (ARS) artery remains unclear. We sought to determine the prevalence of ALV and ARS in BTAI patients and assess their association with injury severity and adjunctive LSA revascularization during TEVAR. METHODS: We performed a r

Beyond injury severity: monocyte-associated small extracellular vesicles distinguish respiratory-support trajectories following thoracic trauma.

Thoracic trauma is a major contributor to morbidity in severely injured patients and is frequently associated with prolonged mechanical ventilation, tracheostomy, and extended intensive care unit (ICU) treatment. Early identification of patients at risk for unfavorable respiratory-support trajectories remains challenging. Small extracellular vesicles (sEVs) are rapidly released in response to cellular activation and injury and may provide biological information complementary to established clinical parameters. In our study we analyzed CD9⁺CD14⁺ and CD9⁺CD61⁺ sEV subpopulations in 45 patients w

The differences in injury patterns and outcomes of thoracic trauma between rural and urban level two trauma centers in a single country.

PURPOSE: Equal level trauma centers in the same country might have significant differences regarding their demographics and types of trauma. Understanding geographic variations in injury patterns are essential for optimal care. Here we describe the differences in injury patterns and associated outcomes of thoracic trauma patients between rural and urban level-II trauma centers in a single country. METHOD: A retrospective analysis of patients with thoracic trauma over a three-year period in a rural level-II and an urban level-II in Israel were compared. Demographics, mechanism of injury, preho
