Clinical translation of carbon nanomaterials: the clinical translation paradox, regulatory barriers, and future pathways.

Carbon-based nanomaterials (CNMs) have generated intense research interest owing to their exceptional physicochemical versatility and broad biomedical potential. However, despite two decades of rapid innovation, the clinical translation of CNMs remains strikingly limited, owing to a persistent mismatch between performance-driven material innovation and predictability-driven clinical and regulatory demands-an enduring clinical translation paradox that urgently requires systematic resolution. This review examines four representative CNM classes-carbon dots, carbon nanotubes, carbon nanofibers, a

Biological functions of BAF57, its role in disease pathogenesis, and treatment: From molecular mechanisms to clinical translation.

Brahma-associated factor 57 (BAF57), known as SWI/SNF matrix-associated actin-dependent regulator of chromatin subfamily E member 1 (SMARCE1), is a main subunit ubiquitously existing in mammalian SWI/SNF ATP-dependent chromatin remodeling complexes. BAF57 regulates diverse biological processes, including gene transcription, DNA recombination, repair, and replication. It plays a critical role in cell cycle control, cellular growth and development, skeletal muscle differentiation, and immune modulation. Beyond its fundamental biological functions, BAF57 was identified as a potential biomarker fo

Plasminogen Activator Inhibitor-1 as a Therapeutic Target for Healthy Longevity, Immunosenescence, and Age-Related Disease: Translational Development of the Small-Molecule Inhibitor TM5614.

Plasminogen activator inhibitor-1 (PAI-1), encoded by SERPINE1, is the principal physiological inhibitor of tissue-type and urokinase-type plasminogen activators and a central regulator of fibrinolysis. Beyond its canonical hemostatic role, PAI-1 has emerged as a pleiotropic mediator of tissue remodeling, fibrosis, metabolic dysfunction, cancer progression, cellular senescence, and age-associated immune dysregulation. A central argument of this review is that PAI-1 should be understood not only as a downstream biomarker of aging-associated pathology, but also as an active effector linking sene

[Immunology of aging in head and neck cancer : From the tumor microenvironment to translational approaches and clinical response].

The immune system undergoes profound age-related changes that manifest as impaired immune function (immunosenescence) and chronic low-grade inflammation (inflammaging), both of which may influence the tumor biology of head and neck squamous cell carcinoma. While phenotypic analyses of peripheral and intratumoral immune cells have so far shown few age-associated differences, omics studies have identified senescence-related signatures linked to poor prognosis and reduced immune activity. Emerging translational data suggest that senolytic therapies may overcome mechanisms of immunosenescence-driv

Aptamers in cancer therapy: Why has clinical translation lagged behind preclinical promise?

Aptamers have attracted substantial interest as cancer therapeutics due to their high target specificity, chemical programmability, and versatility in drug and RNA delivery; however, despite extensive preclinical development, clinical translation in oncology has remained extremely limited, with no aptamer-based therapies approved for cancer indications to date. This gap reflects not a failure of binding affinity, but a fundamental misalignment between aptamer selection and evaluation paradigms and the biological complexity of human tumors. Most aptamers are selected against purified proteins o

Mesenchymal Stromal Cell Rejuvenation Strategies to Enhance Clinical Translation in Cell Therapy.

Cell therapies represent a promising frontier in modern medicine. Mesenchymal stromal cells (MSCs) constitute a valuable source due to their minimal ethical concerns, low immunogenicity, negligible tumorigenicity in vivo, and robust paracrine activity mediated by the secretion of anti-inflammatory and angiogenic factors. Despite these advantages, MSCs undergo rapid replicative senescence accompanied by a progressive loss of stemness markers and functional potency. In this context, cellular rejuvenation has been emerging as a pivotal strategy to restore their original biological features. These

MicroRNAs in penile cancer: challenges, opportunities, and translational perspectives.

MicroRNAs (miRNAs) are small non-coding RNAs that regulate gene expression, and dysregulated miRNA expression has been implicated in multiple cancer hallmarks. In penile cancer (PeCa), a rare but aggressive malignancy, evidence suggests that miRNAs contribute to tumor development and disease progression. This review aims to summarize the current understanding of miRNA involvement in PeCa, highlighting their potential as diagnostic, prognostic, and therapeutic biomarkers. A comprehensive review of published studies on miRNA expression in PeCa was performed, with an emphasis on molecular mechani

miR-29b as an Anti-Fibrotic Therapeutic: Mechanisms, Disease Biology and Translational Opportunities.

Fibrosis emerges when normally self-limiting tissue repair fails to resolve and overlapping phases of injury, stromal activation, extracellular matrix (ECM) deposition and remodelling become sustained. MicroRNAs (miRNAs) shape this transition by coordinating signalling, cell-state and matrix programmes. Functionally, pro-fibrotic fibro-miRs amplify fibrogenic pathways, whereas anti-fibrotic miRNAs restrain fibroblast activation and ECM production; the miR-29 family is a principal member of the latter group. This review examines miR-29 family organisation, the regulation of miR-29b by transform

Toward precision anti-biofilm therapy for Candida: a translational perspective.

Candida biofilm-associated infections remain difficult to eradicate, particularly on indwelling devices and in other high-risk settings where persistence, relapse, and antifungal failure are common. Natural compounds and nano-enabled delivery have shown promising antibiofilm effects in preclinical studies, but the evidence base remains fragmented and largely nonclinical. This Perspective proposes Precision Anti-Biofilm Therapy (PABT) as a staged translational framework that separates what is clinically actionable now from what remains investigational. Tier 1 centers on current bedside practice

Immunopathogenesis and Experimental Models of Alopecia Areata: From Mechanistic Insights to Translational Platforms.

Alopecia areata (AA) is a common autoimmune, non-scarring hair loss disorder with substantial clinical heterogeneity and psychosocial burden. Although recent therapeutic advances, particularly Janus kinase (JAK) inhibitors, have transformed management, the pathogenic mechanisms underlying disease initiation, progression, and relapse remain incompletely understood. Current evidence supports the collapse of hair follicle immune privilege as a central event in AA, followed by the activation of cytotoxic T-cell-dominant responses and amplification through IFN-γ/IL-15-driven inflammatory circuits.
