There's No Place Like Home: Delirium as a Barrier in Geriatric Trauma.

INTRODUCTION: Delirium is associated with a three-fold increase in frequency of 6-mo mortality following intensive care unit admission. Outside of mortality, it has been linked with severe morbidity including long-term cognitive decline, loss of autonomy, and increased risk of institutionalization. There is a paucity of literature regarding delirium and geriatric trauma patients. The aim of our study is to determine predictive factors of delirium in geriatric trauma patients. METHODS: This is a retrospective review of all geriatric (>65 y) trauma patients with a documented frailty score at a

Influence of preoperative comprehensive education on anxiety, depression, pain, and sleep in elderly patients operated under general anesthesia.

BACKGROUND: Owing to the particularities of their physical characteristics, older patients undergoing surgery under general anesthesia experience great surgical traumas. Thus, exploring more refined and individualized nursing approaches is an urgent need to mitigate the negative effects of surgery on such patients. AIM: To analyze the influence of preoperative comprehensive education on anxiety, depression, pain, and sleep in older patients who underwent surgery under general anesthesia. METHODS: In total, 163 older adults who underwent surgery under general anesthesia between June 2022 and

Effect of age on progression through temporary prostheses after below-knee amputation.

Forty six patients that underwent below knee amputation for diabetes, trauma/osteomyelitis or peripheral vascular disease were studied. Healing was estimated by the rate of progression through temporary prostheses as determined weekly by the amputee rehabilitation team (physiatrist, orthopedist, physical therapist, nurse, and prosthetist). For all groups combined, mean days +/- standard error from amputation to above knee cast with pylon and foot (AKPy) were 22.9 +/- 1.9, to below knee cast with pylon and foot (BKPy) 41.6 +/- 2.8 and to laminated temporary prostheses 66.5 +/- 4.5. A regression

Implementation of a standardized screening program for risk of posttraumatic stress disorder among youth hospitalized with injury.

Children with pediatric injury and their parents are at risk for developing posttraumatic stress disorder (PTSD). Although challenging to implement, standardized screening for risk of developing PTSD can identify families most at risk. The current retrospective, chart review study evaluated the implementation of a clinical program that integrated screening for risk of PTSD into standard care for youth admitted to a Level I pediatric trauma center due to injury. Advanced practice nurses administered the Screening Tool for Early Predictors of PTSD (STEPP), a brief screen that evaluates risk of d

Detection of acute alcohol intoxication and chronic alcohol dependence by trauma center staff.

BACKGROUND: Trauma patients with acute alcohol intoxication or chronic alcohol dependence are at greater risk for morbidity and mortality. We hypothesized that relying on clinical suspicion to detect acute alcohol intoxication and chronic alcohol dependence in trauma patients is inaccurate, influenced by injury factors, and biased by race, gender, age, and socioeconomic status. METHODS: Trauma patients were screened with a blood alcohol concentration and with the Short Michigan Alcohol Screening Test and CAGE questionnaire. Before screening, physicians and emergency department nurses were ask

On-scene helicopter transport of patients with multiple injuries--comparison of a German and an American system.

Hospital-based helicopter services from a German (GER) and an American (AMR) university-affiliated trauma center were reviewed. All patients with multiple injuries transported via helicopter from the scene to the trauma centers during a 1-year period were included. The patients were comparable regarding mechanism of injury, age, flight times, mean ISS, ISS distribution, and number of severe injuries per body region (patients with AIS score > 3 for head, thorax, and abdomen). Overall mortality was 21 of 221 (9.5%) for GER and 21 of 186 (11.3%) for AMR (NS). Survivor-based TRISS analysis yielded

Nuclear to cytoplasmic compartment shift of the p33ING1b tumour suppressor protein is associated with malignancy in melanocytic lesions.

CONTEXT: Communities in northern Canada face many mental health concerns related to isolation, historical and intergenerational trauma effects, and economic issues. Access to mental health services is problematic due to geographic, cultural and economic issues. Reduced mental health services have resulted in more responsibility and stress for the remaining formal mental health practitioners (including counsellors, psychologists, social workers and nurses) and on informal mental health support, such as lay counsellors, Elders, family members, and community-identified helpers. Endotracheal intubations in rural pediatric trauma patients.

BACKGROUND/PURPOSE: Evidence from urban trauma centers questions the efficacy of pediatric field endotracheal intubations (ETIs). It is recognized that in the rural environment, discovery, transport delays, and a paucity of pediatric expertise contribute to higher pediatric trauma mortality rates compared with urban environments. The purpose of our study was to determine the effectiveness of field ETI in rural pediatric trauma patients. METHODS: ETI attempts (field, referring hospital, trauma center [TC]) in trauma patients less than 19 years old were included. Prehospital and TC charts, incl

Traumatic spinal cord injury in West Virginia: Disparities by insurance and discharge disposition from an acute care hospital.

Context: Medicaid has been linked to worse outcomes in a variety of diagnoses such as lung cancer, uterine cancer, and cardiac valve procedures. It has furthermore been linked to the reduced health-related quality of life outcomes after traumatic injuries when compared to other insurance groups. In spinal cord injury (SCI), the care provided in the subacute setting may vary based upon payor status, which may have implications on outcomes and cost of care.Design: A retrospective review utilizing the institutional trauma databank was performed for all adult patients with spinal cord injury since

"I got to catch my own baby": a qualitative study of out of hospital birth.

BACKGROUND: About 1.6% of planned births in the United States occur out of hospitals. Studies indicate that planned out-of-hospital birth (OOHB) is safe and satisfying for women; however, there is great variation among ethnic groups, and Black women are underrepresented. A recent phenomenon is the choice to have an unassisted birth (UAB) with no midwife or other professional maternity care attendant. The purpose of this study is to fill a gap in understanding reasons for choosing OOHB or UAB for two clinically important sub-groups of women: Black women, and women who have experienced childhood

Delirium Diagnosis Timing Among Trauma and Acute Care Surgery Ward Patients.

BACKGROUND: Delirium is common among hospitalized trauma and acute care surgery patients and may be under-recognized on surgical wards. Understanding which patient factors are associated with earlier versus later documented delirium diagnosis may inform targeted surveillance and prompt recognition. OBJECTIVE: To identify factors associated with the timing of documented delirium diagnosis among adult trauma and acute care surgery patients admitted to a surgical ward. METHODS: This single-center, retrospective cohort study included adults admitted to a trauma and acute care surgery ward (Janua

Healthcare providers' perspectives on integrating trauma-informed care for intimate partner violence in HIV/STI clinical settings in Trinidad and Tobago.

Intimate Partner Violence (IPV) remains a significant public health concern, particularly for persons living with HIV (PLHIV) and other sexually transmitted infections (STIs). The intersection of IPV and HIV/STIs creates complex care needs requiring trauma-informed, contextually grounded responses. This study explored healthcare practitioners' perspectives on existing practices and opportunities to strengthen IPV-related care in public HIV/STI clinical settings in Trinidad and Tobago. Structured weekly consultations were conducted over seven weeks with 13 healthcare practitioners-including phy

Acute Traumatic Pain in the Emergency Department.

Trauma is a major cause of mortality throughout the world. Traumatic pain-acute, sudden, or chronic-is defined as "an unpleasant sensory and emotional experience associated with actual or potential tissue damage". Patients' perceptions of pain assessment and management have become an important criterion and relevant outcome measure for healthcare institutions. Several studies show that 60-70% of ER patients experience pain, and more than half of them express a feeling of sorrow, which can be moderate or severe, at triage. The few studies that have analyzed how pain is assessed and managed in t
