Therapeutic Response to Treatment of a Papillomatous Ocular Surface Squamous Neoplasia With Intramuscular Human Papillomavirus Vaccine.

PURPOSE: The purpose of this study was to describe the response of a papillomatous ocular surface squamous neoplasia (OSSN) to the intramuscular (IM) 9-valent human papillomavirus (HPV) vaccine after failed medical and surgical interventions. METHODS: A 79-year-old White man with a conjunctival lesion underwent a biopsy which revealed OSSN and positivity for high-risk HPV. Initially treated with medical therapy and surgical excisions, the patient developed a recurrence and refused further surgery. He was given 4 doses of IM HPV vaccine at the 6-week interval. RESULTS: A dramatic reduction in

The Case of Billy Best: 25 Years Later.

Billy Best was diagnosed with Hodgkin lymphoma in 1994 at age 16 and became well-known when he ran away from home to avoid receiving further chemotherapy. His story became national news when, with the support of his adopted parents, he returned home and opted to use complementary and alternative medicine (CAM) instead of standard chemotherapy and radiation for his cancer treatment. Now 25 years since Billy Best entered the public eye, his story is one that is frequently referenced in pediatrics, bioethics, and other related fields. Here, the authors examine the evolution of various features of

Erosive rodent ulcer of the ear secondary to neglect.

Basal cell carcinomas (BCCs) are slow-growing, locally invasive lesions that rarely metastasise, however, if left untreated can progress to extensive destruction of local structures giving rise to the alternative name 'the rodent ulcer'. Here we present a case of BCC of the ear progressing to destruction of the pinna and mastoid, involving the facial nerve and breaching dura. This case presentation shows not only the seldom-seen natural progression of the rodent ulcer with impressive imaging and clinical photographs but also highlights a broader topic of capacity and consent in patients displa

"My son can not attend the school because 5 classmates are unvaccinated". On the question of compulsory vaccinations and the risk for immune-compromised children into the schools: the case of paediatric cancer patients.

Since 2017, 10 vaccines are compulsory for newborns in Italy and unvaccinated children are not admitted to kindergartens. Recently the Italian Government announced the perspective of reforming the law about the compulsory vaccination. A debated started about the presence, in the same class of the schools, of unvaccinated and immunocompromised children. Cancer is the one of the most important cause of immunodepression among children: after the chemoterapy, there is a period of 13-23 months in which the cancer survivors have to come back at the school and at to the "normal life" (even for psycho

Incorporating trial data into bipolar disorder management.

Evidence-based medicine integrates data from clinical trials into clinical care while efficacy studies assess simplified interventions in simplified populations. The findings of such studies are useful in obtaining Food and Drug Administration approval of medications but often fail to generalize to the more complex populations and interventions commonly seen in clinical practice. Recently, an increased emphasis has been placed on effectiveness studies (also referred to as pragmatic or practical studies) designed to yield findings that can be generalized to the clinical management of patients w

Improving adherence in patients with alcohol dependence: a new role for pharmacists.

PURPOSE: To review medication adherence rates with various pharmacotherapies and the role of pharmacists in improving medication adherence in patients with alcohol dependence. SUMMARY: Nonadherence diminishes the effectiveness of treatment for all chronic diseases, including alcohol dependence. Rates of adherence to pharmacotherapies for alcohol dependence range widely, both within and among agents. Factors undermining adherence include high pill burden and adverse effects, both of which occur with common treatments for alcohol dependence. There is a clear need for interventions that improve

Progressive gastrointestinal histoplasmosis leading to colonic obstruction two years after initial presentation.

A 37-yr-old man from Ecuador presented with abdominal pain, diarrhea, and weight loss. Endoscopy revealed duodenal histoplasmosis. The patient improved with antifungal therapy but was readmitted 2 yr later with diarrhea and fever. Colonoscopy revealed histoplasmosis lesions, including a constricting transverse colon lesion. The patient refused surgery and died of colonic perforation. We discuss the diagnosis and management of gastrointestinal histoplasmosis in this report.

Spontaneous regression of recurrent hepatocellular carcinoma after TAE: possible mechanisms of immune mediation.

We report findings in a 76-year-old man who underwent a lateral segmentectomy of the liver for hepatocellular carcinoma in July 1996. In July 1997, transarterial embolization (TAE) was performed for recurrent tumors in the remnant liver. Augmentation of the tumors and an increase in protein induced by vitamin K absence or antagonist (PIVKA)-II level were noted in October 1997, and, although we recommended TAE again, the patient and his family refused further treatment. Subsequently, the patient was only observed, and, except for a small lesion that was probably a scar, no tumors were noted on

Recent advances in peptic ulcer disease: Helicobacter pylori infection and its treatment.

The identification of the gastric bacterium Helicobacter pylori was a significant advancement in the treatment of peptic ulcer disease. H. pylori infects the gastric mucosa and its eradication is associated with the prevention of ulcer recurrence. The significance of this finding is that bacterial infections can be treated and cured, offering hope to individuals with peptic ulcers. Characteristics of H. pylori and its putative role in ulcer formation are discussed. The recent challenge has been to identify a drug regimen that will effectively eradicate this organism inexpensively and convenien

Discharges of children from hospital against medical advice.

A retrospective study on discharges of children from hospital against medical advice or at own risk (AOR) discharges was conducted at our department from March 1981 to February 1990. There were altogether 890 patients giving an average incidence of 2%/year. The racial composition comprised 62.5% Chinese, 28.5% Malay, 7.3% Indian and 1.7% others. The common reasons for AOR discharge includes: (a) Inconvenience of having the child hospitalised (18.4%). (b) Preference of being treated by the general practitioner (15%). (c) Parents think child is well (14%). (d) Preference of being treated by priv

The untransfusable patient.

Red cell transfusions represent the main treatment of acute blood loss anemia; however, there are patients who are unable or refuse to receive blood products. Further, in all patients, it is essential to only transfuse when the benefits of transfusion outweigh the risks. In those patients who are unable to receive a blood transfusion, it is important to have a multidisciplinary treatment plan. Further, for those patients requiring surgery, it is important to consider the pre-, intra-, and postoperative periods. Many of the principles and strategies outlined in this article can be applied to pa

Extensive tracheal resection in lung cancer and tuberculosis: a case report.

BACKGROUND: Tracheal bifurcation resection remains the greatest challenge in airway reconstruction, especially with extensive lesions. Additionally, lung cancer and pulmonary tuberculosis comorbidity complicate the chemoradiotherapy treatment due to the TB reactivation. This case describes tracheal resection in a patient with both tuberculosis (TB) and lung cancer. CASE PRESENTATION: The patient was diagnosed with right lung tuberculosis and upper lobe cancer with trachea invasion complicated by hemoptysis. A right pneumonectomy with circular trachea bifurcation resection was performed. Radio
