[Human African trypanosomiasis in children. A pediatrics service experience in Libreville, Gabon].

During a period of six years (1/1/89-12/31/94), seven children with trypanosomiasis were admitted to the Department of Pediatrics of Owendo Pediatric Hospital-Libreville, Gabon. They were 5 boys and 2 girls, aged 4-17 years, five of them under 15 years. The main reasons of hospitalization were somnolence (4 cases), psychical disorders (5 cases), neurological disorders (4 cases), asthenia (3 cases), loss of weight (3 cases) and fever (3 cases). Increased sedimentation rate (5 cases) and hypergammaglobulinemia (6 cases) were the most important biological disturbances. Serodiagnosis (CATT, indire…

Cost-effectiveness analysis of alternative treatments of African gambiense trypanosomiasis in Uganda.

African trypanosomiasis, or sleeping sickness, is a tropical disease caused by trypanosome parasites transmitted by tsetse flies. The focus of this paper is on the cost-effectiveness of alternative drug treatments for patients in the late stage of the disease. Melarsoprol has been used for many decades. More recently, eflornithine has been developed. It has fewer side effects and improves the overall cure rate. It is much more expensive than melarsoprol, however. The objective of the present cost-effectiveness is to identify the costs and benefits that would be involved in switching from melar…

Delayed recovery of adrenocortical and testicular function after chemotherapy of human trypanosomiasis.

The following indicators of pituitary, adreno-cortical and testicular function were measured in 58 male African trypanosomiasis patients from Western Kenya; plasma cortisol, luteinizing hormone (LH) and testosterone levels. The measurements were carried out by specific radioimmunoassay methods in early and late stage infected patients on admission to hospital and in both groups of patients after one month of chemotherapy. Packed cell volume (PCV) and hemoglobin levels were also measured in all the patients to determine the extent of their anaemia and success of recovery. High parasitaemia, ana…

Measurement of risk in endemic areas of human African trypanosomiasis in Côte d'Ivoire.

An index of epidemiological risk was developed for the foci of human African trypanosomiasis (HAT) in the forest zone of Côte d'Ivoire, based on the following characteristics of Glossina palpalis palpalis populations: daily survival rate, apparent density of teneral males and females, and frequency of human-fly contact. The index agreed well with HAT prevalence. It varied according to ethnic groups and with seasonal changes in agricultural activities and fell rapidly to zero following the start of an anti-vector control campaign. Further studies in different biogeographical zones are desirable…

[2 epidemics of arsenical encephalopathy in the treatment of trypanosomiasis, Uganda, 1992-1993].

Since 1988, the french non-governmental organisation Médecins Sans Frontières is running a control program of human african trypanosomiasis in the district of Moyo, North-Uganda. Between 1988 and 1993, more than 7,000 patients were diagnosed and treated. Since 1988, it has been noted that incidence of melarsoprol reaction had increased systematically between June and October of each year, indicating strong seasonal variation. In 1992 and 1993, two outbreaks of arsenical reactive encephalopathy (ARE) occurred in the sleeping sickness center of Adjumani. The incidence of ARE suddenly exceeded 10…

The use of azathioprine to ameliorate post-treatment encephalopathy associated with African trypanosomiasis.

The treatment of human African sleeping sickness is complicated by a post-treatment meningoencephalitis that may be fatal. Using a mouse model this study assesses the use of the non-steroidal anti-inflammatory drug, azathioprine, in the management of this post-treatment reaction. Female NIH mice treated with the trypanocidal compound diminazene aceturate (40 mg/kg), 28 days after infection, developed a similar post-treatment reaction to that seen in humans. Administration of azathioprine (100 mg/kg) for 5 days before and 5 days after trypanocidal chemotherapy abrogated the pathology in the cen

[Recurrent fever episodes in an African child: diagnostic difficulties of trypanosomiasis in France].

A young Angolian boy who had emigrated to France at the age of 2, presented with a long history of fever. Gambian Trypanosomiasis was diagnosed with peculiar aspects: 1) evolution of adult sickness with a long hemolymphatic period (first stage) and a subacute worsening period with neurologic deficit and somnolence (second stage); 2) a possible post-transfusional contamination: the young boy, born in South Angola, a nor-highly endemic area, was transfused at the age of 10 months with the blood of a donor who was subsequently treated for Trypanosomiasis; 3) a suppurating adenopathy; 4) a predomi…

Red blood cells' auto-agglutination as an indicator test in human trypanosomiasis.

Spontaneous red blood cells' auto-agglutination was assessed as an indicator for the diagnosis of human African trypanosomiasis. This test is easily carried out by health workers with minimum qualification. It presents a high sensitivity (0.91) and a high predictive value of a negative result (0.99). Although a positive result gives a low indication of infection, the health care workers should refer the patient to a screening centre.

Diagnosing multiple parasitic infections: trypanosomiasis, loiasis and schistosomiasis in a single case.

A case is reported of a 32-year-old traveller with loiasis, schistosomiasis and African trypanosomiasis. The patient had been working in oil exploration in Nigeria and Gabon and presented with Calabar swellings and carpal tunnel syndrome. Serology for all 3 diseases was positive but microfilariae of Loa loa and ova of schistosomiasis were not found. Treatment with diethylcarbamazine and praziquantel was given for loiasis and schistosomiasis respectively. Trypanosomes were isolated from a lymph node aspirate only after repetition of the procedure 2 months later and the patient was treated with …

Prevalence of domestic animal trypanosomiasis in the Fontem sleeping sickness focus, Cameroon.

Three hundred-fourteen domestic animals: 114 goats, 93 sheep, 67 dogs and 30 pigs in the Fontem sleeping sickness focus (Cameroon) were screened for infection with trypanosomes using parasitological and serological tests. Nannomonas was the only sub-genus detected in the animals with a prevalence of 28.3%. The card agglutination test (Testryp CATT) for trypanosomiasis showed a positivity rate of 38.2%, indicating that it allowed a better estimation of animal trypanosomiasis. The absence of trypanosomes of the sub-genus Trypanozoon seems to indicate that human African trypanosomiasis is not a z…

[Integration of African human trypanosomiasis control in a network of multipurpose health centers].

The authors relate their experience of integrating screening for African human trypanosomiasis (AHT) caused by T.b. gambiense in a network of multi-purpose health centres at Nioki, Zaire. Since 1983 the health centre nurses have been trained in the laboratory techniques for AHT screening and have been given the essential equipment (microscope, slides, coverslips). The strategy used was that of passive screening of patients who came of their own accord to the health centre: systematic palpation and aspiration of lymph nodes, and examination of wet and/or thick blood films from patients with AHT…

[Trial of pentamidine in the treatment of trypanosomiasis of sheep inoculated with Trypanosoma brucei brucei].

An experimental study was carried out using 5 Trypanosoma brucei brucei subcutaneously infected sheep. Pentamidine treatment appeared in the CSF. Two animals were injected daily for 10 days and they died with drug intoxication. Two animals received drug every 3 days for 15 days: one of them died, the other was cured after 3 weeks of set-back. This preliminary study should be completed by experimentation made over a more important period of time.

Relevance of autoantigens to autoimmunity in African trypanosomiasis: study of DNA and thyroglobulin antibodies.

In order to investigate whether the autoantibody production in the course of African trypanosomiasis is the result of a generalized polyclonal activation or if it is a specific (antigen-dependent) phenomenon we looked for the presence of autoantibodies directed against autoantigens likely to be released (DNA) or unlikely to be released (thyroglobulin) into the circulation during human T. gambiense infection. Sera from 21 patients with African trypanosomiasis were screened for anti-native DNA or anti-denatured DNA antibodies by a Farr DNA binding radioimmunoassay. In addition, antibodies to nat…

Immunologic reactions associated with anemia, thrombocytopenia, and coagulopathy in experimental African trypanosomiasis.

Rats infected with Trypanosoma brucei rhodesiense developed anemia, thrombocytopenia, and hypocomplementemia. Anemia, thrombocytopenia, and sharp reductions in parasitemia were associated with elevated titers of cold-active hemagglutinin, antibody to fibrinogen/fibrin-related products, and immunoconglutinin. Depletion of lytic complement, prolonged partial thromboplastin times, and presence of fibrin monomers in the blood occurred at the time anemia and significant elevations in precipitable immune complexes were observed. Terminally, consumption of immunologic factors coincided with accelerat
