Outcomes of reintroducing anti-tuberculosis drugs following cutaneous adverse drug reactions.

BACKGROUND: Data regarding outcomes of tuberculosis (TB) associated cutaneous adverse drug reactions (CADR) are limited. The re-introduction of first-line anti-tuberculosis drugs after CADR is controversial and management poorly defined. METHODS: We retrospectively reviewed the records of 298 patients with CADR admitted to a tertiary dermatology ward in Cape Town, South Africa. RESULTS: TB-associated CADR was diagnosed in 65 of 298 patients. Of these, 60/65 (92%) were human immunodeficiency virus (HIV) infected (median CD4 count 107 cells/mm (3)). Anti-tuberculosis drugs were reintroduced in

Prevalence of Hepatitis B and C Among Healthcare Workers in a Tertiary Care Center in Monrovia Liberia.

BACKGROUND: Viral hepatitis is a leading cause of death worldwide, with a higher mortality rate than that from human immunodeficiency virus (HIV), tuberculosis (TB) or malaria. In order to meet the WHO's goal of eliminating hepatitis B and C by 2030, there is a dire need to establish baseline prevalence rates and increase public health awareness of this detrimental disease, especially in low and middle-income countries (LMICs) where establishing prevalence rates of hepatitis B (HBV) and hepatitis (HCV) continues to be a problem. OBJECTIVE: To establish baseline rates of hepatitis B and C amon

[Glycocorticosteroid administration prevents fulminant hepatic failure occurrence in patients with chronic hepatitis B of severe degree].

OBJECTIVE: To prevent chronic severe hepatitis, even more fulminant hepatic failure (FHF) occurrence in patients with chronic hepatitis B of severe degree using steroid. METHODS: 120 patients were randomized into conventional supporting treatment and steroid treatment groups. The latter, 62 patients were given intravenously hydrocortisone sodium succinate at the dose of 150 mg to approximately 200 mg everyday plus support care. RESULTS: The rate of deteriorating to chronic severe hepatitis in steroid treatment group was significantly lower than that of conventional group (22% vs 48%, x(2) =7

Infections in British clinical laboratories, 1982-3.

During 1982-3, 31 specific and 12 uncharacterized infections were reported from 30 of 240 laboratories, representing 29 223 person-years of experience. Thirteen cases of hepatitis included 10 of type B or non-A, non-B hepatitis of probable occupational origin (attributable incidence 34.2 per 100 000 person years) affecting haematology, biochemistry, and postmortem workers. Of nine cases of tuberculosis, three were probably acquired in the laboratory (attributable incidence 10.3 per 100 000 person years) and affected microbiology, morbid anatomy, and postmortem staff. Microbiology staff also ac

Changes in cortisol metabolism following rifampicin therapy.

A patient with Addison's disease required increased corticosteroid dosage whilst receiving rifampicin. The pharmacological half-life of cortisol was reduced, but returned to normal when rifampicin was stopped. Cortisolproduction rates in four patients with pulmonary tuberculosis rose during treatment with rifampicin, as did urinary D-glucaric-acid excretion, an index of liver microsomal-enzyme activity. The alteration of the corticosteroid requirement in the patient with Addison's disease and the elevation of the cortisol-production rates were attributed to increased cortisol catabolism follow

Same Pathogen, Different Manifestations: A Case of Extrapulmonary Tuberculosis.

Tuberculosis (TB) remains the most prevalent contagious disease worldwide and a significant cause of morbidity, ranking as the second most deadly disease globally. The transmission of the disease occurs through aerosols via the respiratory route, predominantly affecting pulmonary tissue. However, the pathogen can disseminate and infect any organ within the body. Up to 15% of patients exhibit extrapulmonary involvement. The case involves a 59-year-old male who presented to the emergency department complaining of abdominal pain and subfebrile episodes, without any other significant symptoms or f

Management of rheumatoid arthritis: consensus recommendations from the Hong Kong Society of Rheumatology.

Given the recent availability of novel biologic agents for the treatment of rheumatoid arthritis (RA), the Hong Kong Society of Rheumatology has developed consensus recommendations on the management of RA, which aim at providing guidance to local physicians on appropriate, literature-based management of this condition, specifically on the indications and monitoring of the biologic disease-modifying anti-rheumatic drugs (DMARDs). The recommendations were developed using the European League Against Rheumatism (EULAR) recommendations for the management of early arthritis as a guide, along with lo

[Clinical epidemiology and treatment of tuberculosis in elderly patients].

In developed countries the incidence of tuberculosis is higher in patients aged 65 and over than in any other age group, with the exception of HIV positive subjects. This high incidence is a consequence of the very high rate of infection in our countries in the first part of the century, and of the diminished efficiency of the aging immune system. In this age group, most cases of tuberculosis are reactivations of dormant mycobacteria. However, the possibility of a newly acquired infection must be kept in mind, especially in institutionalized patients. The clinical presentation is often insidio

Peliosis hepatis in a patient with marasmus.

Peliosis hepatitis is a rare disorder previously seen in tuberculosis and malignant disorders and now seen with anabolic steroid use or after renal transplantation. We report the first case of peliosis hepatis in a patient with marasmus and no previously reported predisposing condition. Of interest, the peliosis hepatis resolved rapidly (over 2-3 wk) as determined by computed tomography scan and the patient presented with a cholestatic enzyme pattern that resolved with development of the lesion.

A Case of Acute Liver Failure in a Patient on Isoniazid Prophylaxis for Latent Tuberculosis.

Isoniazid (INH) is widely used for latent Mycobacterium tuberculosis despite the known risk of liver injury, with severe hepatitis occurring in up to 1% of patients. We report a patient who presented with two weeks of anorexia, nausea, and jaundice following six months of INH monotherapy for latent tuberculosis (TB). After other causes of liver injury were ruled out, she underwent a liver biopsy showing submassive necrosis, hepatocellular dropout, and lobular inflammation with no evidence of fibrosis. She was also found to have acute portal hypertension. She was diagnosed with drug-induced liv

A proline deletion in IFNAR1 impairs IFN-signaling and underlies increased resistance to tuberculosis in humans.

Type I interferons (IFN), best known for their anti-viral functions, have been shown to impair host resistance to intracellular bacteria in mice. However, the precise role of type I IFN signaling in bacterial infection in humans is unclear. Here, we show that genetic variation in the human IFNAR1 gene is associated with decreased susceptibility to tuberculosis and an increased risk of viral hepatitis in Chinese populations. Receptor mutagenesis and cell signaling studies establish that the IFNAR1 mutation corresponding to a proline deletion in the hinge region of the membrane-proximal domain o

Design, synthesis and biological evaluation of novel non-nucleoside HIV-1 reverse transcriptase inhibitors with broad-spectrum chemotherapeutic properties.

Acquired immunodeficiency syndrome (AIDS) results from infection by the retrovirus, human immunodeficiency virus (HIV). HIV is the most significant risk factor for many opportunistic infections like tuberculosis, hepatitis, bacterial infections, etc. In this paper, we designed aminopyrimidinimino isatin lead compound as a novel non-nucleoside reverse transcriptase inhibitor with broad-spectrum chemotherapeutic properties for the effective treatment of AIDS and AIDS-related opportunistic infections. Compound 1-ethyl-6-fluoro-1,4-dihydro-4-oxo-7[[N(4)-[3'-(4'-amino-5'-trimethoxybenzylpyrimidin-2

Rare cause of extrapulmonary tuberculosis in an immunocompetent elderly patient.

Tubercular hepatic abscesses in the geriatric population are rare and often misdiagnosed, frequently leading to delayed or presumptive treatment based on tissue biopsy findings. Early and accurate diagnosis using advanced diagnostic tools is crucial for improving prognosis. We present the case of a 65-year-old immunocompetent male who reported a three-month history of fever and one month of right hypochondrial pain. Diagnosis of a tubercular hepatic abscess was confirmed using a cartridge-based nucleic acid amplification test (CBNAAT), which isolated Mycobacterium tuberculosis sensitive to rif

Granulomatous hepatitis.

According to the literature on the subject, hepatic granulomas are present in 2.4%-10% of the liver tissue specimens examined in general hospitals. The pathogenesis and pathological importance of these lesions remain unclear in many cases. Hepatic granulomas have been associated with a wide variety of diseases, among which sarcoidosis, followed by tuberculosis, figure most prominently. The etiology remains undetermined in less than or equal to 36% of reported cases. In some instances the lesions are found by chance, and in other cases they are associated with a prolonged course of fever of und
