Prospective evaluation of sleep improvement after cubital tunnel decompression surgery.

BACKGROUND: Compromised sleep is a known phenomenon with compressive neuropathies such as carpal tunnel syndrome. However, the prevalence of sleep disturbance with cubital tunnel syndrome (CuTS) and the effect on sleep after ulnar nerve decompression are not well understood. We hypothesized that CuTS results in sleep disturbances and that decompression surgery would result in improvement in overall sleep quality. METHODS: Consecutive patients with electrodiagnostic-proven CuTS indicated for decompression were prospectively enrolled. Demographic data, McGowan grade, electrodiagnostic (electrom

[Chronic ulnar wrist pain in adults: diagnosis and treatment principles].

Adult patients with chronic ulnar wrist pain often seek medical advice. There are a number of diagnoses for chronic ulnar-sided wrist pain. A sound diagnostic strategy is therefore of paramount importance. Patient's history, clinical examination and high quality standard radiographs are the most important part of this strategy. Sophisticated investigations should only be performed to focus on solid clinical hypotheses. The palmar and dorsal aetiologies of ulnar wrist pain are considered as well as their treatment principles.

Randomized, prospective study comparing ulnar neurolysis in situ with submuscular transposition.

OBJECTIVE: To help clarify the optimal surgical strategy for idiopathic, symptomatic ulnar nerve compression at the elbow in terms of overall outcome and morbidity by using objective criteria. METHODS: Forty-four surgical candidates were recruited prospectively and were randomized into the neurolysis (n = 23) or transposition (n = 21) arm of the study. Preoperative and postoperative outcomes were assessed symptomatically and by performance on McGowen and Louisiana State University Medical Center grading systems at 1 month, 6 months, and 1 year. RESULTS: Both procedures were equally effective

[Compression of the ulnar nerve at the wrist due to an arthro-synovial cyst. Apropos of 2 cases].

The authors report two cases of ulnar nerve compression at the wrist due to arthrosynovial cysts arising from the hamatotriquetral joint. The absence of strict parallelism between the type of neurological deficit observed, the site of the cyst and the site of compression requires ulnar nerve release as far as its division in the pisi-unciform hiatus, where the section of the hypothenar fibrous arcade and pisi-unciform ligament is essential. Complete recovery of these 2 patients confirmed the favourable prognosis of this form of compression provided surgical treatment is performed early and rig

Ultrasound-guided hydrodissection of the subclavius muscle region in neurogenic thoracic outlet syndrome: a case report.

BACKGROUND: Thoracic outlet syndrome (TOS) is frequently underdiagnosed due to nonspecific symptoms and the absence of definitive radiographic findings. Neurogenic TOS (nTOS), the most common subtype, is typically caused by compression of the brachial plexus within anatomically constrained spaces. While muscular contributors such as the scalene and pectoralis minor muscles have been widely discussed, the role of the subclavius muscle remains underrecognized. CASE PRESENTATION: A 53-year-old male presented with an 18-month history of progressive right upper limb weakness and medial paresthesia

Compression neuropathies of the upper extremity.

Nerve compression syndromes of the upper extremity occur at predicable locations. The diagnosis of nerve compression or nerve entrapment is based on the neurologic and electrodiagnostic examinations. The anatomy, neurophysiology, and electrodiagnosis of nerve compression are discussed. Common and uncommon compression and entrapment syndromes of the upper extremity are described. Errors in diagnosis occur when the neurologic or electrodiagnostic examinations are incomplete or inaccurate.

Rehabilitation after subcutaneous transposition of the ulnar nerve: immediate versus delayed mobilization.

We studied 36 patients who had clinical signs and symptoms consistent with cubital tunnel syndrome and in whom nonoperative management failed. These patients underwent anterior subcutaneous transposition of the ulnar nerve followed by either immediate (20 patients) or delayed (16 patients) mobilization. All patients were evaluated with an outcomes assessment questionnaire, and 35 of the 36 were given repeat physical examinations. After surgery, there were no significant differences between the two groups in pain relief, weakness, or patient satisfaction (71% of the immediate mobilization group

Outcomes following modified oblique medial epicondylectomy for treatment of cubital tunnel syndrome.

PURPOSE: To quantify and define objective and patient-rated outcomes after our modification of medial epicondylectomy for the treatment of cubital tunnel syndrome. Although medial epicondylectomy has been previously studied, data are lacking regarding elbow-specific outcomes after our technique that aims to minimize complications historically associated with medical epicondylectomy. METHODS: A total of 27 subjects with clinical and electrodiagnostic evidence of cubital tunnel syndrome underwent a modified oblique medial epicondylectomy that was designed to minimize bony resection and preserve

Reverse End-to-Side Abductor Digiti Minimi-to-Recurrent Motor Branch Nerve Transfer for Severe Carpal Tunnel Syndrome: Report of Three Cases.

Severe carpal tunnel syndrome (CTS) with advanced thenar denervation has limited reconstructive options, and reliable reinnervation remains challenging. Reverse end-to-side (RETS) nerve transfers have been described in other contexts but to our knowledge, have not been applied clinically to compressive median neuropathy at the wrist. Here, we present three patients, ages 59, 68, and 72 years, with severe CTS, visible thenar atrophy, and electrodiagnostic evidence of profound median motor axon loss who underwent carpal tunnel release with abductor digiti minimi (ADM)-to-recurrent motor branch (

Simple in situ decompression for idiopathic cubital tunnel syndrome using minimal skin incision.

Cubital tunnel syndrome is one of the most frequently occurring compression neuropathy in the upper limb next to carpal tunnel syndrome. Recent minimal invasive technique has prompted us to gain clinical experience with simple in situ decompression with minimal skin incision for idiopathic cubital tunnel syndrome. Sixty six consecutive patients with cubital tunnel syndrome were treated using minimal skin incision technique. The mean age of the patients was 49.7 (range: 15-77) years and average follow up period was 23.9 months (range: 12-60 months). The severity of ulnar neuropathy was classifi

Neurological Complications Associated With SARS-CoV-2 Infection: A Single-Centre Experience.

BACKGROUND: The clinical presentation of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection can range from mild or moderate disease (80% of the cases) to severe disease (15%) requiring oxygen support, and critical disease (5%), associated with acute respiratory distress syndrome and admission to the intensive care unit (ICU). In critically ill patients, prone positioning can be used to optimize oxygenation. Although there is a favourable response to this strategy, being a life-saving measure, additional associated complications may appear, including compressive neuropathies

Prevention of complications in hand therapy.

Basic principles stress that the injury or surgery is not to be extended through complications resulting from a failure to control edema, poor wound care, improper splinting, or forceful exercise. Prevention of these problems in hand therapy is discussed. Close rapport between surgeon, patient, and therapist is most important to support and motivate the patient. If that motivation is demolished or even impaired, the entire course of surgery and therapy may be complicated or impaired.

Prevalent involvement of thenar motor fibres in vineyard workers with carpal tunnel syndrome.

OBJECTIVE: Carpal tunnel syndrome (CTS) has a high prevalence in agricultural workers, especially those engaged in vineyards. We postulated that vineyard CTS was electrophysiologically different from CTS of other subjects. We performed a retrospective cross-sectional electrophysiological study of two cohorts of consecutive patients with CTS, the first consisting of vineyard workers and the second, of other unselected types of workers, housewives and pensioners. METHODS: Thirty-three vineyard workers (mean age 46.8years, 42% women) and 205 patients with other occupations (mean age 53.7years; 6

A case report of primary synovial chondromatosis of the elbow.

This case is a 37-year-old man with diffuse primary synovial chondromatosis of the elbow, with associated ulnar nerve compression and flexion contracture at the elbow. An open synovectomy with removal of loose bodies and an ulnar nerve decompression with anterior subcutaneous transposition were performed. Postoperatively, the patient's elbow mobility and nerve compression symptoms improved steadily, which significantly improved his function. Primary synovial chondromatosis of the elbow is a rare disease that can result in pain, swelling, decreased range of motion, and mechanical symptoms. Trea

MR imaging of the elbow. An update.

MR imaging provides clinically useful information in assessing the elbow joint. Superior depiction of muscles, ligaments, and tendons as well as the ability to visualize nerves, bone marrow, and hyaline cartilage directly are advantages of MR imaging relative to conventional imaging techniques. These features of MR imaging may help to establish the cause of elbow pain by accurately depicting the presence and extent of bone and soft-tissue pathology. Ongoing improvements in surface coil design and newer pulse sequences have resulted in higher quality MR images of the elbow that can be obtained

Application of Stockholm criteria to patients with hand-arm vibration syndrome in a follow-up study.

We applied Stockholm criterion to patients with hand-arm vibration syndrome (HAVS) for assessing the therapeutic effects in comparison with the criterion of the Ministry of Labour in Japan (MLJ). Forty male patients recognized as an occupational disease were examined in two times (mean interval periods 3.0 years). The subjects were free from vibratory tasks during the mean period of 8.3 years. The clinical examinations and some laboratory tests were conducted with the same procedures by the two same occupational physicians. The criterion of MLJ was graded more severe than Stockholm one for the

Postoperative ulnar neuropathy after kidney transplantation.

Postoperative ulnar neuropathy as a result of mechanical trauma at the elbow was observed in eight patients undergoing renal transplantation. In five cases this occurred on arms that were adducted during the operation and in which an arteriovenous shunt was present. In four cases (one patient had the lesion bilaterally) the other, abducted arm was affected. Analysis of possible causes revealed no single factor responsible for the condition. Factors that most likely contributed were: pressure on the adducted arm by the combined weights of patient and surgeon, blood pressure monitoring with a cu

Compression of the deep branch of the ulnar nerve in Guyon's canal by a ganglion: two cases.

INTRODUCTION: Ulnar nerve compression at the wrist can be caused by a variety of intrinsic and extrinsic factors. Isolated compression of only the deep branch of ulnar nerve by a ganglion is very uncommon. Ultrasound examination can clearly show the cystic lesion compressing the nerves. MATERIALS AND METHODS: We present two cases of compression of deep branch of ulnar nerve by a ganglion in the Guyon's canal. Two male patients presented with history of progressive weakness and paraesthesia in the medial 1(1/2) digits of the non-dominant hand. Interestingly, both the patients noticed sudden on

Cubital tunnel release and medial epicondylectomy: effect of timing of mobilization.

The effects of early versus late range of motion (ROM) following cubital tunnel decompression and medial epicondylectomy were evaluated in a randomized prospective study. Forty-five consecutive procedures were divided into 2 groups. The early mobilization group started rehabilitation at an average of 3 days after surgery and the late mobilization group started rehabilitation at an average of 14 days after surgery. Flexion contracture of more than 5 degrees degrees was observed in 5% of the early mobilization group, compared to 52% of the late mobilization group (p < .001). On average, patients
