Trigeminal pain under vagus nerve stimulation.

Three epilepsy patients treated by cyclic continuous vagus nerve stimulation (VNS) experienced trigeminal pain during the periods of stimulation, which was reported as toothache in the left lower jaw, ipsilateral to the side of stimulation. The symptom occurred with a latency of days to weeks following an increase in stimulation current intensity (SCI). Trigeminal pain was reversible with decrease in SCI, or subsided due to habituation. These findings show that clinically relevant effects of VNS on nociception may occur. Because of the late onset and variable form of this side effect, trigemin

Ordering and interpreting rheumatologic laboratory tests.

Many mechanical and systemic conditions can cause joint pain and synovitis. When rheumatologic illness is suspected, the initial evaluation begins with an accurate history, physical examination, and selective use of confirmatory testing, which can help avoid common pitfalls inherent in serologic evaluation. Tests for erythrocyte sedimentation rate, C-reactive protein level, rheumatoid factor, antinuclear antibodies, anticardiolipin antibodies and lupus anticoagulant, HLA-B27, uric acid level, and Lyme disease, either alone or in combination, may support certain diagnoses. Using these tests non

Breast enlargement induced by D-penicillamine.

OBJECTIVE: To report a patient with reversible breast enlargement due to D-penicillamine treatment for scleroderma. CASE SUMMARY: A 55-year-old white woman developed enlarged breasts during D-penicillamine therapy for scleroderma. The enlargement regressed following discontinuation of the drug, and planned breast reduction surgery was avoided. DISCUSSION: D-Penicillamine has been used in the treatment of various diseases including scleroderma. Breast enlargement has been previously described as a rare adverse effect of this drug. The enlargement subsided following discontinuation of the medi

Inflammatory pseudotumor causing intestinal obstruction: diagnostic and therapeutic aspects.

The authors report an unusual presentation of inflammatory pseudotumor (IPT) that caused intestinal obstruction in a 9-year-old boy, and discuss the clinicopathologic features of this rare entity with emphasis on diagnosis and treatment. There are no specific presumptive clinical and laboratory findings, including tumor markers and imaging techniques, that distinguish mesenteric IPT from other abdominal mass lesions. The most important diagnostic aid is to bear this entity in mind when a child presents with intestinal obstruction associated with an abdominal mass. Radical unnecessary surgical

Fatigue fracture of the sacrum in a child.

The authors describe a 9-year-old girl with stress fracture of the sacrum. Such fractures can cause low-back or hip pain in children but may not be evident on plain films. The appearance on scintigraphy is not diagnostic. Likewise, the findings on magnetic resonance imaging may be nonspecific or confusing, because changes in signal intensity in the marrow may not be sufficient to distinguish infection or malignancy from healing of the fracture. Computed tomography can be more specific in delineating a sclerotic fracture oriented parallel to the sacroiliac joint. Recognition of the characterist

The role of the dentist in diagnosing infection in the AIDS patient.

A case report of an AIDS patient afflicted with disseminated histoplasmosis is presented. Characteristic granulomatous-appearing lesions on the tongue were observed during the patient's initial workup, but the patient became unavailable for a scheduled biopsy. Consequently, the diagnosis of a histoplasmosis infection was established only after the performance of several time-consuming, stressful, and, at times, invasive diagnostic procedures. It is proposed that early histologic evaluation of the suspicious oral lesions would have resulted in the diagnosis, thus making more extensive diagnosti

[Magnetic resonance tomography of the feet and ankle joints].

21 patients with suspected, or known, abnormalities of the feet or ankles were examined by magnetic resonance tomography. The indications were: osteochondritis dissecans of the talus (three cases), osteomyelitis (three cases), suspected tumour recurrence (three cases), undiagnosed pain (six cases) and a further miscellaneous six cases. Although most of these problems could have been solved with other imaging methods, magnetic resonance tomography did add some valuable diagnostic information. Because of its ability to demonstrate bones, synovia, tendons, vessels and subcutaneous fascia in a mul

Pseudotumor of the supraclavicular fossa.

Minor musculoskeletal variants of the upper thoracic spine or of the ribs can cause effacement of the supraclavicular fossa, simulating a mass ("pseudotumor"). These variants may occur singly or in combination, and include unilateral prominence or asymmetry of the first rib, unilateral prominence or asymmetry of a cervical rib, and mild upper thoracic scoliosis causing prominence of the first rib on one side. The detection of variants by physical examination or more easily by review of the chest x-ray film may in selected instances obviate the need for costly diagnostic procedures and unnecess

Brucellosis of the spine. A report of 3 cases.

Three cases of spinal osteomyelitis due to brucellosis are reported, all in women. Radiological confirmation was delayed for at least three months after the first clinical manifestation, while in one patient a bone scan was positive at an early stage. In two of the three cases the diagnosis was delayed because of insufficient awareness of the disease. Spinal osteomyelitis caused by Brucella is indistinguishable radiologically from that caused by other micro-organisms and needle aspiration or exploration is frequently performed to establish the correct diagnosis. Increased awareness of brucello

The problems of massive small bowel resection and difficulties encountered in management.

Massive small bowel resection is imperative in the management of several pathological conditions and is accompanied by high operative mortality. In those that survive, serious nutritional disturbances are inevitable. Intestinal adaptation is said to occur but cannot be relied upon. Long-term parenteral nutrition is problematical and often unsuccessful. Many surgical procedures have been adopted in an effort to improve prognosis and have been found wanting. Controlled experimental studies in the use of reversed segments and a limited number of reported clinical cases using the method after mass

Laparoscopic procedures for small bowel disease.

Small bowel procedures such as placement of feeding jejunostomy, diagnosis of small bowel ischaemia and obstruction, bowel resection and lysis of adhesions can all be performed laparoscopically. Diagnostic laparoscopy can be performed with low complication rates, and can help avoid unnecessary laparotomy. The open method of trocar placement is preferred in patients with adhesions or distended bowel due to obstruction or ileus. Feeding jejunostomy can be placed by laparoscopically assisted methods, pulling the jejunum out or completely laparoscopically. The latter requires fixation of the jejun

Building blocks for the prevention of blood culture contamination: A quality improvement intervention in emergency department.

Blood culture contamination, commonly caused by skin flora, leads to false-positive results, unnecessary antibiotic use, increased costs, and antimicrobial resistance. A multimodal intervention was initiated in the emergency department of a tertiary care hospital in Western India to reduce contamination. Root cause analysis identified unstandardized collection practices and inadequate staff knowledge. Interventions included standardized blood collection kits, two-step skin antisepsis, visual posters, and repeated hands-on staff training. Post-intervention, contamination rates decreased from 9.

Chronic Recurrent Multifocal Osteomyelitis in a Pediatric Patient: An Uncommon Case With Diagnostic Challenge.

Chronic Recurrent Multifocal Osteomyelitis (CRMO) is a rare autoinflammatory bone disorder that primarily affects children and adolescents. It is characterized by recurrent episodes of bone pain with sterile inflammatory lesions, often resembling infection or malignancy. We present a case of a 12-year-old girl who initially presented with atraumatic right knee pain and was ultimately diagnosed with CRMO after imaging and histopathologic evaluation. Her clinical course was complicated by recurrent episodes, and physeal damage resulted in limb length discrepancy. Early recognition through approp

Pulmonary Infarction Secondary to Prostatitis-induced Venous Thromboembolism Mimicking Lobar Pneumonia in a Young Patient: A Diagnostic Pitfall.

Pulmonary infarction (PI) often mimics lobar pneumonia. A 23-year-old man with prostatitis presented with pleuritic pain and hemoptysis. Despite high inflammatory markers, consolidation was refractory to antibiotic treatment. Retrospectively, the initial opacities represented the hyperacute hemorrhagic phase of PI. Thoracentesis revealed bloody effusion, and contrast-enhanced computed tomography (CECT) confirmed pulmonary thromboembolism with characteristic "central lucencies." Anticoagulation therapy led to his recovery. PI should be considered in young patients with refractory pneumonia. Blo

Hepatic Ectopic Adrenal Gland in a Patient with Hepatitis C Virus Who Achieved a Sustained Virological Response.

Hepatic ectopic adrenal glands are rare and can mimic hepatocellular carcinoma (HCC). We herein report a 60-year-old woman with a history of chronic hepatitis C who achieved a sustained virological response. A 23-mm hepatic lesion was detected and initially diagnosed as HCC. However, laparoscopic resection and histopathology confirmed the presence of an ectopic adrenal gland. Given the location of the posterior liver, a biopsy is challenging. Adrenal scintigraphy may aid in the diagnosis and reduce the need for invasive procedures. As more cases are reported, improved imaging-based diagnoses m

Late secondary syphilis mimicking anal carcinoma: A case illustration.

Syphilis can closely mimic malignancy in both clinical presentation and appearance. We describe a case of a 58-year-old man presenting with painful perianal masses raising concern for anal carcinoma. Serologic testing revealed late secondary syphilis with a rapid plasma reagin titer of 1:128. Treatment with doxycycline resulted in complete clinical resolution and appropriate serologic response, obviating planned biopsy. While condyloma lata were considered as part of the differential, the patient's history, lesion morphology, and distribution were atypical, prompting initial concern for malign

Pericardial Recesses on Computed Tomography: Implications for the Pulmonologist.

The pericardium comprises a double-walled fibrous-serosal sac that encloses the heart. Reflections of the serosal layer form sinuses and recesses. With advances in multidetector computed tomography (CT) technology, pericardial recesses are frequently detected with thin-section CT. Knowledge of pericardial anatomy on imaging is crucial to avoid misinterpretation of fluid-filled pericardial sinuses and recesses as adenopathy/pericardial metastasis or aortic dissection, which can impact patient management and treatment decisions. The authors offer a comprehensive review of pericardial anatomy and

Consensus of the Brazilian association of hematology, hemotherapy and cellular therapy on patient blood management: Assessment and management of coagulation in the preoperative period.

Managing coagulation disorders and potential bleeding risks, especially in the context of anticoagulant medications, is of immense value both clinically and prior to surgery. Coagulation disorders can lead to bleeding complications, affecting patient safety and surgical outcomes. The use of Patient Blood Management protocols offers a comprehensive, evidence-based approach that effectively addresses these challenges. The problem is to find a delicate balance between preventing thromboembolic events (blood clots) and reducing the risk of bleeding. Anticoagulant medications, although crucial to p
