An 8-year-old boy passing urine through both fecal and urinary outlets since birth is reported. Investigations showed absence of the posterior urethra. A surgical procedure for reconstruction of the posterior urethra is described. The embryological explanation is also discussed.

Five paraplegic patients with pressure ulceration complicated by incontinence related to fistula formation were treated by deliberate closure of the urethra and augmentation cystoplasty with a continent stoma. This procedure is similar in scope to urinary diversion but preserves a low-pressure reservoir and obviates the use of tubes for drainage of urine. Overall outcome was excellent. Eight hundred and forty male patients attending sexually transmitted disease (STD) clinic for urethritis were investigated. Out of them, 31.6% had gonococcal urethritis, 16.1% suffered from nongonococcal urethritis due to Ureaplasma urealyticum and in 12.6%, both the organisms were present. Though 14.62% strains of N. Gonorrhoeae were resistant to penicillin, all the strains were sensitive to spectinomycin; while all Ureaplasma strains were sensitive to tetracyclines. As the treatment differs for these two organisms, it is necessary to identify the correct etiological agent.

A 24-year-old man, who complained of tumor of the urethral orifice with hypospadias, was admitted to our Hospital on November 30, 1983. Anterior urethral valves are rare entities, especially in case they are not associated to urethral diverticulum. Because of their obstructive effects on the urinary tract, they must be searched in case of clinical evidence of lower obstructive uropathy. Three cases of anterior urethral valves without urethral diverticulum are herein reported. Differential diagnosis with urethral diverticula, endoscopic features and treatment are discussed and emphasized.

We treated 6 girls with total urinary incontinence secondary to a congenitally short urethra by a combination of extensive internal and external urethral lengthening. Of the patients 3 had hypospadias and 3 had severe epispadias. All 6 are dry during the day and 1 has nighttime wetting. Success depends upon the creation of a long neourethra and a new continence zone.

In 14 male dogs, the functional urethra was replaced by a bladder flap tube in a 2-stage procedure. In the 1st stage of the operation a bipedicle strip from the anterior bladder wall was tubularized around a 10F or 12F catheter. A case of carcinoma in urethral diverticulum is herein reported. The patient was a 58-year-old housewife presenting with the complaint of dysuria. Physical examination revealed a hen-egg-sized cystic mass at the anterior vaginal wall. Complete excision of the diverticulum was performed transvestibulary. Pathologically, it was described as papillary adenocarcinoma. The patient's postoperative course was uneventful and she was discharged. She was readmitted because of recurrence of tumor 9 months after the first operation. Forty-five cases including this case are reviewed and discussed.

In 26 patients with detrusor-sphincter dyssynergia after a spinal cord injury the resting posterior urethra at cystography was found either closed as in normal subjects or open to a varying degree. Highest intravesical pressure measured at cystometry during uninhibited detrusor contraction was statistically significantly related to the degree of dilatation of the resting posterior urethra. The effect of intravaginal electrical stimulation (IVS) on the urethral pressure profile (UPP) before and during succinylcholine blockade or spinal anaesthesia was studied in patients with stress incontinence and in patients with clinically normal urethral function. Condyloma acuminatum very seldom affects the urinary bladder, but if it does conservative treatment has been considered of little value and cystourethrectomy will eventually be required. A case with urethrovesical condyloma is presented, where repeated transurethral resections cured the patient.
