A Burdizzo castrator was evaluated for the neutering of dogs. Congenital absence of both vasa deferentia is not an infrequent cause of sterility. Between April 1975 and December 1981, 11 men out of a total of 749 presenting with infertility were diagnosed as having congenital absence of both vasa deferentia. Subsequent clinical investigations showed that FSH levels were within the normal range (2-10 mIU/ml), blood karyotype (XY) was normal, and testicular histology demonstrated normal spermatogenesis. Contractile responses to norepinephrine (NE) of strips of vasa deferentia from prostatic end isolated from rats with genetic hypertension and deoxycorticosterone (DOC)-salt induced hypertension were examined. Maximum responses to NE of vasa deferentia from all hypertensive and corresponding normotensive control rats were biphasic. We performed microsurgical vasovasostomy on 25 outpatients using spermatic cord block anesthesia, an infrapubic incision and the 1 1/2-layer anastomosis technique. Vasitis nodosa is a benign reactive condition characterized by nodular thickening of the vas deferens which usually develops following vasectomy. In addition to chronic inflammation, fibrosis and sperm granulomas, an exuberant proliferation of ductules resembling invasive adenocarcinoma is seen. Because of both external and individual factors, nowadays more men seek clarification and consultation about their fertility. This article reviews the current diagnostic work-up of subfertile men and the evidence based treatment options. Also testicular biopsy and cryopreservation of sperm or testicular tissue are addressed. Finally, the issues of vasectomy reversal and aplasia of the vas deferens are discussed. The paper emphasizes practical issues and practicability in office urology.

Laparoscopy was utilized in the evaluation of 27 nonpalpable testes in 21 patients prior to exploration and necessary definitive surgery. Laparoscopic prediction of the location of the nonpalpable testes was accurate in all cases. Laparoscopic findings can help in determining the appropriate technique of orchiopexy expected to yield better testicular salvage. It can also prevent unnecessary exploration of children with anorchia as evidenced by blind-ending vas deferens and spermatic vessels.

A case of autohemicastration in a 19-year-old patient with schizophrenia is reported. Orthotopic replantation of the testis under local hypothermia was performed. One pair of arterial, two pairs of venous vessels and vas deferens ends were anastomosed micro-surgically end-to-end. Surgical reinnervation of the testis was carried out. Peculiarities the operation technique, expediency of local hypothermia and surgical reinnervation of the testis in replantation are discussed.

In a retrospective review of the case notes of 510 infertile men, testicular failure of various types was the aetiologic factor in 61% of cases, chronic epididymitis in 32% and varicocoeles in 25%. Five patients had bilateral absence of the vas deferens. It is hoped that improved health-care delivery will benefit the group of patients with preventable and potentially curable aetiologic factors like varicocoeles, chronic epididymitis and testicular torsion.

PURPOSE: We reviewed the histological findings of cord structures excised at inguinal exploration after diagnostic laparoscopy in patients with a nonpalpable testis with the vas deferens and spermatic vessels exiting the internal inguinal ring in the absence of a patent processus vaginalis.
