Feasibility and safety of on table extubation after corrective surgical repair of tetralogy of Fallot in a developing country: a case series.

Fast-track extubation is an established safe practice in pediatric congenital heart disease (CHD) surgical patients. On table extubation (OTE) in acyanotic CHD surgical patients is well established with validated safety profile. This practice is not yet reported in tetralogy of Fallot (TOF) cardiac surgical repair patients in developing countries. Evidence suggests that TOF total correction patients should be extubated early, as positive pressure ventilation has a negative impact on right ventricular function and the overall increase in post-TOF repair complications such as low cardiac output

Hyperventilation during orthostatic challenge in spinal cord-injured humans.

OBJECTIVE: To determine whether arterial hypotension is related to hyperventilation during standing in individuals with complete spinal cord injury (SCI). METHODS: Sixteen individuals with SCI (thoracic 1 to thoracic level 12, ASIA grade: A) and 18 able-bodied individuals participated in this study. Individuals with SCI were divided into a group with injury level at or above thoracic (T) 7 [higher SCI (HSCI), n = 9] and a group with injury level at or below T10 (lower SCI, n = 7). Subjects carried out 6-min quiet sitting and then 6-min quiet standing using a standing frame. RESULTS: Signific

[Identification of patients with a high risk of needing prolonged mechanical ventilation after coronary surgery].

OBJECTIVES: To identify patients at greater risk of developing respiratory complications, defined as the need for mechanical ventilation (MV) longer than 48 h, following revascularization surgery. MATERIAL AND METHODS: This was a prospective analysis of 39 variables in 107 consecutive operations taking place over 9 months. We studied the association of these variables with the need for prolonged MV after surgery, by way of single variable and multivariate analysis. RESULTS: The incidence of prolonged MV was 7.7% and the 25% rate of mortality in the group of patients with this complication wa

Serial lung ultrasound scoring for predicting invasive mechanical ventilation in preterm neonates: a prospective diagnostic accuracy study.

UNLABELLED: Continuous Positive Airway Pressure (CPAP) is the first-line respiratory support for preterm neonates with respiratory distress, but 15-30% fail and require invasive mechanical ventilation (IMV). Lung ultrasound (LUS) has emerged as a radiation-free, point-of-care imaging modality with demonstrated superiority over chest radiography for diagnosing neonatal respiratory conditions, whose serial performance for predicting IMV across a broad preterm cohort is not well characterised. Objective: To evaluate the diagnostic accuracy of serial LUS scores on Days 1-3 for predicting IMV withi

How does the VPD response of isohydric and anisohydric plants depend on leaf surface particles?

Atmospheric vapour pressure deficit (VPD) is the driving force for plant transpiration. Plants have different strategies to respond to this 'atmospheric drought'. Deposited aerosols on leaf surfaces can interact with plant water relations and may influence VPD response. We studied transpiration and water use efficiency of pine, beech and sunflower by measuring sap flow, gas exchange and carbon isotopes, thereby addressing different time scales of plant/atmosphere interaction. Plants were grown (i) outdoors under rainfall exclusion (OD) and in ventilated greenhouses with (ii) ambient air (AA) o

Differentiating Lung From Chest Wall Mechanics Is Difficult Without Esophageal Manometry in Children With Acute Respiratory Distress Syndrome.

OBJECTIVES: Pediatric acute respiratory distress syndrome (PARDS) guidelines recommend limiting airway plateau pressure (Pplat) to 28 cm H 2 O, allowing for higher limits when chest wall compliance (C CW ) is poor since less of the pressure is transmitted to lung (transpulmonary pressure). Transpulmonary pressure depends on Pplat and the ratio of lung elastance to respiratory system elastance (E L /E RS ). E L /E RS measurement requires esophageal manometry, although it is not routinely available. We sought to determine if routinely available clinical data could reliably predict E L /E RS or c

Factors associated with successful transition from continuous renal replacement therapy in critically ill patients: a retrospective cohort study.

BACKGROUND: Acute kidney injury (AKI) in critically ill patients often requires continuous renal replacement therapy (CRRT), yet predictors of successful transition off CRRT remain unclear. This study aimed to identify clinical factors associated with successful and sustained CRRT discontinuation. METHODS: We retrospectively analyzed 924 adult ICU patients who received CRRT between January 2010 and December 2024. The primary outcome was successful CRRT transition, defined as discontinuation without CRRT re-initiation within 7 days or transition to intermittent hemodialysis. Multivariable logi

Exposure to Prolonged Rupture of Membranes at Term Increases the Risk for Ventilatory Support in Uninfected Neonates.

OBJECTIVE: The effects of prolonged rupture of membranes (ROMs) on perinatal outcomes are still unclear, and it remains controversial for the management of those labors. This study aims to evaluate how the exposure of pregnant women to a prolonged ROM (≥ 24 hours) affects maternal and neonatal outcomes. STUDY DESIGN: This retrospective cohort study included singleton pregnant women at term delivering between January 2019 and March 2020 in a tertiary hospital. All relevant sociodemographic, pregnancy, and perinatal variables (maternal age, prepregnancy body mass index, labor, and delivery outc

Risk factors for the occurrence of livestock-associated methicillin-resistant Staphylococcus aureus (LA-MRSA) in Danish pig herds.

Livestock-associated methicillin-resistant Staphylococcus aureus (LA-MRSA) is widespread in many European countries including Denmark, where 88% of randomly selected production herds tested positive in 2016. In the present study, we investigated herd-level risk factors for farms being classified as LA-MRSA positive (study 1), in addition to herd-level risk factors for farms changing status from LA-MRSA negative to LA-MRSA positive during a 2-year period (study 2). Risk factors previously identified in other studies were confirmed in study 1: large herd size, herd type (lower risk in herds with

Convulsions during cataract surgery under peribulbar anesthesia: a case report.

INTRODUCTION: Locoregional anesthesia techniques are increasingly used for cataract surgery. From these techniques, peribulbar anesthesia has been very successful over the retrobulbar anesthesia seen its effectiveness and safety. However, peribulbar anesthesia is not without risk. CASE PRESENTATION: A 70-year-old African man was scheduled for cataract surgery and lens implant for his right eye. His medical history included hypertension, diabetes mellitus and gall bladder surgery. There were no personal or family antecedents of allergy, epilepsy or taking food or toxic drug. No abnormalities w

Coronary air embolism during removal of a central venous catheter.

BACKGROUND: Acute air embolism has been described during central venous cannulation, but it may also occur during catheter removal in a spontaneously breathing patient. We describe an episode of acute coronary ischaemia that occurred during CV catheter removal. CASE REPORT: A 23-year-old male, multiple trauma patient was treated over 27 days in an ITU. He required a tracheostomy, two weeks of mechanical ventilation, and several surgical interventions. On the 27th day, he was scheduled to be transferred to a low-dependency area and his CVC was removed from the left subclavian vein. After five

Effects of acute changes in pulmonary wedge pressure on periodic breathing at rest in heart failure patients.

BACKGROUND: Patients with heart failure (HF) display a number of breathing abnormalities including periodic breathing (PB) at rest. Although the mechanism(s) contributing to PB remain unclear, we examined whether changes in pulmonary wedge pressure (PWP) and pulmonary vascular resistance (PVR) alter PB in patients with established HF. METHODS: We studied 12 male patients with HF (age, 50 +/- 11 years; ejection fraction, 18.3 +/- 3.8 %; New York Heart Association class, 3.2 +/- 0.4), with PB at rest, who are undergoing right heart catheterization with infusion of nitroprusside. Assessing the Utility of End-Tidal Carbon Dioxide as a Marker for Fluid Responsiveness in Cardiogenic Shock.

Background Preventing end-organ failure in patients with shock requires rapid and easily accessible measurements of fluid responsiveness. Unlike septic shock, not all patients in cardiogenic shock are preload responsive. We conducted this study to determine the discriminant power of changes in end-tidal carbon dioxide (ETCO2), systolic blood pressure (SBP), inferior vena cava (IVC) collapsibility index (IVC-CI), and venous to arterial carbon dioxide (Pv-aCO2) gap after a fluid challenge and compared it to increases in cardiac output. Methodology In a prospective, quasi-experimental design, mec

Response to a combination of oxygen and a hypnotic as treatment for obstructive sleep apnoea is predicted by a patient's therapeutic CPAP requirement.

BACKGROUND AND OBJECTIVE: Upper airway collapsibility predicts the response to several non-continuous positive airway pressure (CPAP) interventions for obstructive sleep apnoea (OSA). Measures of upper airway collapsibility cannot be easily performed in a clinical context; however, a patient's therapeutic CPAP requirement may serve as a surrogate measure of collapsibility. The present work aimed to compare the predictive use of CPAP level with detailed physiological measures of collapsibility. METHODS: Therapeutic CPAP levels and gold-standard pharyngeal collapsibility measures (passive phary

Breathing frequency and pattern are poor predictors of work of breathing in patients receiving pressure support ventilation.

OBJECTIVE: To evaluate the relationships between directly measured work of breathing (WOB) and variables of the breathing pattern commonly used at the bedside to infer WOB for intubated, spontaneously breathing patients treated with pressure support ventilation (PSV). DESIGN: In vivo measurements of the WOB were obtained on a consecutive series of adults. Breathing frequency (f), tidal volume (VT), the index of rapid, shallow breathing (f/V T), the duration of respiratory muscle contraction expressed as the ratio of inspiratory time over total respiratory cycle time (TI/TTOT), and a breathing
