Left ventricular outflow tract obstruction may be dynamic, most commonly associated with hypertrophic cardiomyopathy, and, uncommonly, by congenital anomalies such as discrete subaortic stenosis. We describe a patient with Behçet's disease, presenting with a systolic murmur, fever, and syncope, in whom a diagnosis of subaortic obstruction caused by a pseudo-aneurysm dissecting the interventricular septum and associated with a complete atrioventricular block was made.

Left atrioventricular (AV) valve regurgitation is the most common complication after a atrioventricular canal defect (AVCD) repair. Despite what appears to be a less complex repair, patients with partial and transitional AV canal have higher reoperation rates for left AV valve regurgitation and left ventricular outflow tract (LVOT) obstruction. Retraction of bridging leaflets with secondary attachments to the septal crest commonly produces increased tension and flattening of the medial left AV valve leaflet and LVOT obstruction after cleft closure. We describe a novel technique of detachment a

BACKGROUND: The impact of alcohol septal ablation (ASA) on exercise hemodynamics, symptoms, and quality of life in patients with obstructive hypertrophic cardiomyopathy is poorly described according to contemporary standards. This study aimed to evaluate the effect of ASA on filling pressures at rest and during exercise, left ventricular outflow tract obstruction, symptoms, and quality of life in patients with obstructive hypertrophic cardiomyopathy.
METHODS: This prospective study enrolled 24 patients with obstructive hypertrophic cardiomyopathy referred for ASA. Right-heart catheterization

The physical findings in hypertrophic cardiomyopathy with left ventricular outflow tract obstruction are well described. In the absence of outflow tract obstruction the findings are less distinctive. There have been several reported cases in which the cardiac findings have suggested the diagnosis of mitral stenosis, In this report we describe a patient whose auscultatory and roentgenographic findings more closely mimicked mitral stenosis. The patient had a loud first heart sound, mitral opening snap and an apical presystolic murmur; left atrial enlargement was present. Noninvasive studies, inc

Between January 1967 and December 1988, 34 patients ranging in age from 3 to 18 years (mean, 14 +/- 3.6 years) underwent replacement of the aortic valve or root with their own pulmonary valve. The indication for operation was left ventricular outflow obstruction in 16 patients (47%), aortic regurgitation in 14 (41%), mixed aortic valve disease in 3 (9%), and failure of a previously implanted aortic homograft in 1 (3%). There were four early deaths, all before 1971, giving a hospital mortality of 11.8% (70% confidence interval, 6% to 20%). Surviving patients have been followed up a cumulative t

BACKGROUND: Obstructive hypertrophic cardiomyopathy (oHCM) is a complex condition characterized by left ventricular outflow tract obstruction (LVOTO) in the setting of a hypercontractile and hypertrophic left ventricle. Obstructive hypertrophic cardiomyopathy can lead to exertional angina/dyspnoea, (pre)syncope, heart failure, and sudden cardiac death. A rare but life-threatening complication of oHCM is acute left ventricular ballooning. Standard oHCM treatment is medical (negative inotropic, chronotropic, and dromotropic drugs) or septal reduction in refractory cases (surgical myomectomy or p

New transgastric echocardiographic projections, obtained by monoplane transoesophageal echocardiography are presented. Starting from the transgastric short-axis view, the probe is first advanced 3-5 cm and slightly rotated clockwise. From this projection either a tricuspid valve long-axis or a subcostal-like 4-chamber view is obtained. Advancing the probe 48-55 cm from the incisor teeth and rotating it counter clockwise with maximal anterior flexion, an apical long-axis view is obtained. The left ventricle, including the apex, the left ventricular outflow tract, the aortic valve, the proximal

A group of patients with minor aortic valve disease and inappropriately severe left ventricular hypertrophy is described. Clinical, electrocardiographic and echocardiographic assessment of the degree of left ventricular hypertrophy suggested that they had haemodynamically severe aortic stenosis but this was not borne out at cardiac catheterisation. Although a chance association between non-obstructive hypertrophic cardiomyopathy and mild aortic stenosis may have been responsible, an abnormally severe hypertrophic response to minor aortic outflow obstruction is proposed as another possible expl

BACKGROUND: The Rastelli procedure has been criticized for a high rate of left ventricular outflow tract obstruction (LVOTO) and translocation procedures have been proposed as an alternative. Infundibular resection can be performed during Rastelli to optimize the outflow tract. This study examines whether a selective policy to enlarge the outflow tract improves Rastelli outcomes across all morphological variants.
METHODS: Single institution 29-year experience in 105 patients. Patients were classified into transposition of the great arteries with ventricular septal defect, congenitally correct

Discrete subaortic stenosis (DSS) is a type of left ventricular outflow tract obstruction whereas double-chambered right ventricle is a form of right ventricular outflow tract obstruction. Both of these cardiac malformations share lots of similar characteristics which classify them as acquired developmental heart diseases despite their congenital anatomical substrate. Both of them are frequently associated to ventricular septal defects. The initial stimulus in their pathogenetic process is anatomical abnormalities or variations. Subsequently, a hemodynamic process is triggered finally leading

We report a 9 year old boy who presented late with cyanosis as a case of dextro-transposition of great arteries, intact ventricular septum and left ventricular outflow tract obstruction (D-TGA/IVS/LVOTO). Arterial switch operation (ASO) with resection of sub-neo aortic membrane and repair of mitral valve were done for the naturally trained LV. On the second postoperative day, the newly discovered right ventricular outflow tract obstruction (RVOTO) was relieved and mitral valve replacement (MVR) was done for significant mixed stenotic/regurgitant mitral valve disease, and intraoperative extra-c

Three patients with patent ductus arteriosus and moderate aortic stenosis had a marked reduction in aortic valve gradient following transcatheter ductal occlusion. The hemodynamic effects of an aortopulmonary shunt on the severity of left ventricular outflow obstruction and the implications on intervention are discussed.

BACKGROUND: Mechanisms leading to left ventricular hypoplasia and endocardial fibroelastosis in the fetus remain unknown. Prevailing theory is that obstruction to blood flow through the left ventricle leads to elevated end-diastolic pressures, compromised myocardial perfusion, and endocardial ischemia. Fetal interventions are now being performed, based on the presumption that they would prevent such pathogenic mechanisms.
METHODS: Forty first-trimester fetal sheep (mean gestational age, 53 days) were studied. Severe fetal left ventricular outflow obstruction was created by banding the ascendi

BACKGROUND: DDD pacing has been advocated as an effective treatment for drug refractory obstructive hypertrophic cardiomyopathy. This study reports the outcome of pacing in 56 patients with refractory symptoms referred to four tertiary centres.
METHODS: Core data on symptoms, drug burden, and left ventricular outflow tract gradient were recorded. Patients underwent a temporary pacing study with optimisation of the atrioventricular (AV) delay for greatest gradient reduction without haemodynamic compromise. Patients were assessed after implantation in terms of changes in symptoms, drug load, an

Evaluation of the severity of aortic stenosis in the presence of dynamic left ventricular outflow (LVOT) obstruction is challenging. Invasive hemodynamic assessment with provocative maneuvers can be useful to differentiate of the relative components of obstruction. In patients with both dynamic LVOT obstruction and aortic stenosis, surgical myectomy and concurrent surgical aortic valve replacement is the optimal treatment strategy; combined alcohol septal ablation and transcatheter aortic valve replacement should only be considered for very high surgical risk patients.

In patients with severe mitral regurgitation (MR), mitral valve repair is the preferred and recommended treatment; thus, precise diagnosis of the mechanism, location, and the specific scallop causing MR is decisive and paramount. Occasionally, mitral valve repair is inadequate (residual MR), or result in mitral stenosis, or is associated with systolic anterior motion (SAM) of the mitral valve and left ventricular outflow tract obstruction. Two- and three-dimensional Transesophageal echocardiography (2D and 3D TEE) can precisely diagnose the mechanisms, location and scallops causing MR, identif

OBJECTIVE: Anatomic alterations of the mitral valve such as increased mitral leaflet area, length and laxity, and anterior displacement of the papillary muscles in hypertrophic obstructive cardiomyopathy predispose patients to residual systolic anterior motion and persistence of outflow obstruction and mitral regurgitation after septal myectomy. We investigate the long-term results of combined anterior mitral leaflet retention plasty and septal myectomy in children with hypertrophic obstructive cardiomyopathy.
METHODS AND RESULTS: Anterior mitral leaflet retention plasty and subaortic septal

Eleven patients with dextro-transposition of the great vessels (TGV) and subpulmonic (left ventriclar outflow) obstruction documented by cardiac catheterization and angiography were studied by echocardiography. Echocardiographic features were which characterized the outflow obstruction included a) prolonged diastolic apposition of the mitral valve with the ventricular septum (10 patients), b) smaller width of the pulmonary artery as compared to the aortic root (10 cases), and c) narrowing of the subpulmonic area demonstrated by beam anglation studies through the planes of the mitral valve and

BACKGROUND: Perioperative dynamic left ventricular outflow obstruction associated with systolic anterior motion of the mitral valve is well recognized as a cause for unexplained sudden hypotension in perioperative settings, even without underlying heart diseases such as hypertrophic obstructive cardiomyopathy. We treated a patient who experienced sudden hypoxemia without severe hypotension during emergence from anesthesia after an uneventful laparoscopic cholecystectomy.
CASE PRESENTATION: A 65-year-old female patient with a history of hypertension presented a sudden decrease in oxygen satura
