Prospective clinical study of writer's cramp.

Sixty-five consecutive patients with writer's cramp were studied prospectively to evaluate clinical and demographic features, the number of writing hours per day at dystonia onset, and the existence of trigger events. Assessment of writing and drawing was done on a standardized test using categorical scales. The importance of subjective handicap and pain, of postural and action tremor, the abnormal features of hand grip and the occurrence of mirror dystonia (dystonia occurring in the dominant hand when writing with the other hand) were studied. Thirty-two patients had simple writer's cramp, wh

Agraphia in Alzheimer's disease: an independent lexical impairment.

This study was conducted to delineate the pattern of the writing impairments in 12 patients with Alzheimer type dementia. The patients performed writing tasks involving regular and irregular words and nonwords given by dictation as well as a decision test composed of printed words and pictures requiring phonologic, lexical, and semantic processing. Writing from dictation demonstrated a predominant, but nonisolated, lexical deficit. In order to better evaluate this lexical disorder, the correlation between the dictation writing scores and the decision task scores was analyzed. No significant co

Pure agraphia of kanji due to thrombosis of the Labbé vein.

A case is described of a 56 year old Japanese male with pure agraphia of kanji (the Japanese morphograms) due to haemorrhagic infarction of the left temporal lobe caused by the rare condition of cortical vein thrombosis of Labbé. Writing kanji was severely impaired without disturbed consciousness, aphasia or apraxia. On the other hand, writing kana (the Japanese syllabograms), and reading kanji and kana were almost intact. This suggests that the process of writing kanji involves a different pathway from that of reading kanji in the left temporal lobe. Pure agraphia of kanji is considered to be

[A case of pure agraphia due to left parietal lobe infarction].

We reported a case of a 63-year-old right handed man with pure agraphia due to the left parietal lobe infarction. The characteristics of agraphia in the patient were as follows. 1) The written letters were generally recognizable and well formed. 2) He succeeded in pointing to single Kana letter named by the examiner from the Japanese syllabary, but missed in pointing to Kana words. 3) Further, it took more time for the patient to point to even single Kana letter than for the control. 4) Most errors in Kana writing was substitution. Errors in Kanji writing are partial lacking or no response. Bu

Representations of Clients and Speech and Language Therapists in Assessment Reports: A Critical Analysis.

BACKGROUND: This paper examines assessment report-writing from a critical perspective, a relatively unexplored aspect of speech and language therapy (SLT) clinical practice. To date, there has been little discussion of the relationship between discourse formats, objectives of assessment reports, and the effects of these objectives on how clients and speech and language therapists (SLTs) are portrayed. Furthermore, there has been little exploration of the concepts of objectivity and scientific writing in relation to report-writing in SLT. An emphasis on a scientific, objectifying system has lon

Effects of White Noise on Academic Skills in Children with ADHD and Specific Learning Disorders: New Perspectives for Personalised Rehabilitation and Educational Intervention.

BACKGROUND/OBJECTIVES: This study examined whether exposure to white noise improves reading and writing performance in children with Specific Learning Disorder (SLD), with and without comorbid Attention-Deficit/Hyperactivity Disorder (ADHD). METHODS: Thirty children aged 8-13 years (mean age = 9.4) with SLD, 12 of whom also had ADHD, were recruited from the Centro di Riabilitazione San Raffaele Pisana (Rome). Each child completed two standardized reading and writing assessments, four weeks apart, under two auditory conditions (with vs. without white noise) in randomized order. The primary out

Cerebellar-induced apraxic agraphia: a review and three new cases.

Apraxic agraphia is a writing disorder due to a loss or lack of access to motor engrams that program the movements necessary to produce letters. Clinical and functional neuroimaging studies have demonstrated that the neural network responsible for writing includes the superior parietal region and the dorsolateral and medial premotor cortex. Recent studies of two cases with atypical lesion localisations in the left thalamus and the right cerebellum support the hypothesis that the written language network is larger than previously assumed. The aim of this study is twofold: (1) to provide a surve

Improving learning disabled students' skills at composing essays: self-instructional strategy training.

This study was conducted to determine if self-instructional strategy training would improve learning disabled students' writing. Students were taught a strategy designed to facilitate the generation, framing, and planning of argumentative essays. Training effects were investigated using a multiple-baseline across-subjects design, with multiple probes in baseline. Strategy instruction had a positive effect on students' writing performance and self-efficacy. Effects were maintained over time and transferred to a new setting and teacher. Evidence for generalization to a second genre, story writin

Modified pen grip in the treatment of Writer's Cramp.

Writer's Cramp (WC) is a focal, action-related dystonia, which induces hypertonic co-contractions and severely impairs handwriting. One behavioral treatment approach is the handwriting training developed by Mai and Marquardt (1999), [Mai, N., & Marquardt, C. (1999). Schreibtraining in der neurologischen Rehabilitation. In EKN-Materialien für die Rehabilitation. Dortmund: Borgmann] which includes among various motor exercises the use of a modified pen grip (stabilized between index and middle finger). This pen grip has proven particularly successful in clinical practice. The current study aims

Electroencephalographic spectral power in writer's cramp patients: evidence for motor cortex malfunctioning during the cramp.

We investigated cortical activation as reflected in task-related spectral power (TRPow) changes in 8 writer's cramp patients during writing on a digital board and during isometric contraction and compared them to those of 8 age-matched healthy subjects. Scalp EEG was recorded over the contralateral primary sensorimotor area (SM1(c)), and from the ipsilateral sensorimotor area (SM1(i)). The electromyogram (EMG) was recorded from the Extensor Digitorum Communis (Extensor), Flexor Digitorum Superficialis (Flexor), and First Dorsal Interosseous (FDI) muscles. Further examination of the exposure model underlying the efficacy of written emotional disclosure.

In the current study, the authors examined the effects of systematically varying the writing instructions for the written emotional disclosure procedure. College undergraduates with a trauma history and at least moderate posttraumatic stress symptoms were asked to write about (a) the same traumatic experience, (b) different traumatic experiences, or (c) nontraumatic everyday events across 3 written disclosure sessions. Results show that participants who wrote about the same traumatic experience reported significant reductions in psychological and physical symptoms at follow-up assessments comp

Cognitive Processes Influencing Written Sentence Construction in School-Aged Children With Prenatal Alcohol Exposure.

The study examined the cognitive processes involved in written sentence construction in children affected by prenatal alcohol exposure (PAE) when compared a control group of nonexposed typically developing children, and a contrast group of nonexposed children with various clinical diagnoses. Results indicated that children with PAE and those in the contrast group exhibited poorer sentence writing than the control group. Additionally, children with PAE utilized different cognitive processes to write sentences when compared to the nonexposed groups, irrespective of having a diagnosis. For childr

The mediating effect of cognitive and emotional processing on PTSD and depression symptoms reduction in women victims of IPV.

Intimate partner violence (IPV) is a serious social, physical and mental health issue. Women victims of IPV can develop short- and long-term consequences such as depression and post-traumatic stress disorder (PTSD). Where trauma has been incurred, standard psychotherapies may usefully be complemented by interventions based on expressive writing (EW). Numerous studies have explored the mechanisms underpinning improvement after writing, focusing on the cognitive and emotional processing of traumatic experiences. The aims of this study were to evaluate changes in PTSD and depression symptoms foll

For whom does it work? Moderators of the effects of written emotional disclosure in a randomized trial among women with chronic pelvic pain.

OBJECTIVE: Although written emotional disclosure has potential as a stress management intervention for people with health problems, the main (group) effects of disclosure in medical populations are limited. This study sought to identify individual difference moderators of the effects of written disclosure among women with chronic pelvic pain. METHODS: In a prospective, randomized trial, 48 women with chronic pelvic pain completed 3 individual difference measures and then wrote for 3 days about stressful consequences of their pain (disclosure) or positive events (control). Health status was as

Effects of generative artificial intelligence on cognitive effort and task performance: study protocol for a randomized controlled experiment among college students.

BACKGROUND: The advancement of generative artificial intelligence (AI) has shown great potential to enhance productivity in many cognitive tasks. However, concerns are raised that the use of generative AI may erode human cognition due to over-reliance. Conversely, others argue that generative AI holds the promise to augment human cognition by automating menial tasks and offering insights that extend one's cognitive abilities. To better understand the role of generative AI in human cognition, we study how college students use a generative AI tool to support their analytical writing in an educat

A case of musical agraphia.

Damage to the left upper parietal lobule causes pure agraphia. However, we experienced a patient who exhibited musical agraphia following such a lesion after the agraphia improved. The patient was a 53-year-old female piano teacher. After surgery, she exhibited agraphia and musical agraphia. There was no expressive amusia, receptive amusia, aphasia, agnosia or apraxia. Fifteen months post-surgery, when her agraphia had resolved, her abilities to read, write, and copy music were evaluated. She could read and write single notes and musical signs, but her ability to write a melody was seriously i
