Endocrine Comorbidities in Patients with Psoriatic Arthritis: A Population-based Case-controlled Study.

OBJECTIVE: To investigate endocrine comorbidities in patients with psoriatic arthritis (PsA). METHODS: A retrospective, cross-sectional study was performed with the database of Clalit Health Services, the largest healthcare provider in Israel, between 2002 and 2014. Patients with PsA were identified and matched by age and sex to healthy controls. The following morbidities were analyzed: hypo/hyperthyroidism, hypo/hyperparathyroidism, hyperprolactinemia, Cushing disease, Addison disease, diabetes insipidus, diabetes mellitus (DM), pituitary adenoma, acromegaly, and osteoporosis. Descriptive st…

Disorders of pigmentation.

Skin color is highly individual and the variations are controlled by numerous genes. The different skin colors result from the size and numb … tion only rarely reflects a primary genetic disorder but is most often from acquired diseases as in Addison disease, secondary hemochromatosis or primary biliary cirrhosis. Treatment of pigmentation disorders are based on a diagnosis which sometimes allow a specific intervention. Cosmetically acceptable results are difficult to obtain.

Sexual dimorphisms in endocrinopathies: Their impact on the evolution of metabolic diseases.

Sexual dimorphisms, the biological differences between males and females, are well-documented across various endocrine disorders. These dimorphisms not only influence the presentation and progression of endocrinopathies but also play a critical role in the development and evolution of metabolic diseases. This review explores the intricate relationship between sexual dimorphisms and endocrinopathies such as Diabetes Mellitus, Thyroid disorder, Cushing's syndrome, Addison's disease, polycystic ovary syndrome (PCOS), Hypogonadism in males and acromegaly, and their subsequent effects on metabolic

Woman with a Blackened Tongue: A Case Report.

UNLABELLED: Primary adrenal insufficiency is a rare disease characterized by deficient production of glucocorticoids with or without a deficiency of mineralocorticoids and adrenal androgens.1 This disease can present as a range of symptoms provoking a visit to the emergency department (ED). In this case, we present a 40-year-old female who reported tongue and gingival discoloration, unintentional weight loss, and skin hyperpigmentation. The patient was evaluated by an endocrinologist in the ED, received a dose of steroids in the ED, and discharged home with a prescription for steroids. This ca

[Polyglandular type I autoimmune syndrome].

Two HLA-identical sisters have developed the full picture of type I polyglandular autoimmune syndrome over a period of 12 years. Both girls have hypoparathyroidism and Addison's disease. One of them additionally developed diabetes mellitus, hypergonadotropic hypogonadism and hypothyroidism. Autoantibodies to the adrenal, parathyroid and thyroid glands are present in both patients, as well as antinuclear antibodies. HLA associations have been described recently for the type I polyglandular autoimmune syndrome, but this could not be confirmed in the present two cases. Although we assume that the

The clinical significance of urinary 17-ketosteroid assays.

The urinary 17-ketosteroids are a group of compounds derived from complex steroids produced by the adrenal cortex, testis, or ovary. The method of determining the amount excreted has been simplified so that it is available for routine diagnostic purposes. Usually the amount is increased in diseases in which there is hyperfunction of the adrenal cortex due to tumor or hyperplasia, and decreased in lesions that impair the function of the adrenal cortex. Other conditions such as myxedema, eunuchism, gout, and arthritis may alter the excretion of the 17-ketosteroids. Low levels are also found in t

Gastric Perforation Necessitating Splenectomy.

Gastric perforations can rarely cause splenic erosion from irritation of gastric contents spilling into the peritoneum. This case describes a 51-year-old female on chronic steroid use due to Addison's disease who presents with a gastric perforation involving the splenic hilum. Computed tomography showed a posterior gastric perforation with splenic erosion of a foreign body. Due to her abdominal exam, surgery for abdominal peritonitis was performed. The patient was found to have a pill that had eroded into the splenic hilum after passing through a gastric perforation. This required concomitant

Delay in Diagnosis of Two Siblings with Severe Ocular Problems and Autoimmune Polyglandular Syndrome.

Autoimmune polyendocrine syndrome type 1 (APS1) is a scarce polyendocrinopathy with autosomal recessive inheritance results from defects in the human autoimmune regulatory (AIRE) gene. In addition to three major manifestations of APS1 including mucocutaneous candidiasis, hypoparathyroidism, and Addison's disease, ophthalmic problems such as keratoconjunctivitis, dry eye, iridocyclitis, and cataract can be seen in these patients. In this article, we introduced two siblings presented with nail dystrophia, severe photophobia, and keratitis since early childhood which genetic examination revealed

Adrenal failure due to bilateral metastases as the sole manifestation of relapsing lung carcinoma. Report of two cases.

We describe two patients with lung carcinoma in whom adrenal glands were the sole site of tumor relapse, revealed by the appearance of Addison's disease. Both patients showed bilateral adrenal masses on US and/or CAT scans and received hormone replacement therapy, with rapid improvement of their general conditions. One of them, with small-cell carcinoma, could also be treated with further chemotherapy and achieved a second remission. Therefore, we stress that patients with lung carcinoma should be periodically screened for adrenal deposits by US or CAT and undergo prophylactic steroid maintena

[Acute adreno-cortical insufficiency. Eight case-reports (author's transl)].

Four female and four male adults, with acute adrenocortical insufficiency were admitted to a medical intensive care unit over a period of five years (1975-1980). Three of them were under treatment for Addison's disease. The clinical picture combines fever with signs of shock and dehydration. Increased pigmentation of the skin and laboratory data suggest the diagnosis, which is established by the serum cortisol assay. The etiology was tuberculosis in four patients. Cortical retraction was found in three others. One case followed interruption of non-substitutive corticotherapy. After 48 hours of

Genetic heterogeneity in autoimmune polyglandular failure.

Five cases with candidiasis-endocrinopathy syndrome are presented. They are clustered in three distantly related families with two pairs or affected sibs and one single case. All cases have idiopathic hypoparathyroidism, four have Addison's disease while chronic mucocutaneous candidiasis has appeared in three cases. The mode of presentation argues for an autosomal recessive inheritance. Varying degrees of defective cellular mediated immunity were found. HLA typings revealed no evidence for HLA linkage. These findings discriminate the syndrome as a distinct entity compared to other forms of aut

New Approach to Hyponatremia: High Prevalence of Cerebral/Renal Salt Wasting, Identification of Natriuretic Protein That Causes Salt Wasting.

Our understanding of hyponatremic conditions has undergone major alterations. There is a tendency to treat all patients with hyponatremia because of common subtle symptoms that include unsteady gait that lead to increased falls and bone fractures and can progress to mental confusion, irritability, seizures, coma and even death. We describe a new approach that is superior to the ineffectual volume approach. Determination of fractional excretion (FE) of urate has simplified the diagnosis of a reset osmostat, Addison's disease, edematous causes such as congestive heart failure, cirrhosis and neph

A case of Histoplasma capsulatum causing granulomatous liver disease and Addisonian crisis.

A 56-year-old man with persistently elevated liver enzyme levels, fatigue, lethargy and a 9.0 kg weight loss over six months underwent a percutaneous liver biopsy that demonstrated multiple granulomas. Screening serologies were positive for histoplasmosis, and he was started on itraconazole treatment. He returned to hospital the same night with coffee-ground emesis and in Addisonian crisis requiring parenteral steroids and intensive care unit support. An abdominal computed tomography scan revealed bilaterally enlarged, nonenhancing adrenal glands suggestive of infarcts, presumed secondary to h

Genitourinary tuberculosis complicated with adrenal involvement and primary adrenocortical insufficiency: a case report.

Genitourinary tuberculosis (TB) rarely involves the adrenal gland. A 67-year-old man presented with progressively hyperpigmented skin and an enlarged mass over both testes. Early morning plasma cortisol concentration was low and adrenocorticotropic hormone (ACTH) concentration was high. A rapid ACTH stimulation test revealed the absence of plasma cortisol response and confirmed a diagnosis of primary adrenocortical insufficiency. An abdomen computed tomography (CT) scan disclosed enlargement of the right adrenal gland and punctuate calcification over the left one. This is compatible with tuber

Primary biliary cirrhosis and type II autoimmune polyglandular syndrome.

A 45-year-old female was diagnosed with Hashimoto's thyroiditis in 1976 and Addison's disease in 1979. At that time, her antimitochondrial antibody (AMA) level was elevated at 1:32. She subsequently developed premature ovarian failure and type I diabetes mellitus. In 1996, she became jaundiced with a cholestatic enzyme pattern. AMA was positive at a titre of 1:256. A liver biopsy confirmed the diagnosis of primary biliary cirrhosis (PBC). She underwent a liver transplantation in January 1998. This is the first report of PBC in association with type II autoimmune polyglandular syndrome. The ass

Subsequent Type 2 Diabetes in Patients with Autoimmune Disease.

Immunological data show that type 2 diabetes (T2D) manifests autoimmune features. We wanted to test the association epidemiologically by ass … ed after 27 AIds; the highest SIRs were noted for chorea minor (8.00), lupoid hepatitis (5.75), and Addison disease (2.63). T2D was increased after 27 of 32 AIds but we were unable to control for factors such as obesity and smoking. However, the clearly increased risks for T2D in most types of AId patients, and in reverse order increased risks for AId after T2D, …

Pseudo-Addisonian Crisis With Hyponatremia and Hyperkalaemia in a Late Pregnant Bitch.

CASE PRESENTATION: A 3-year-old pregnant Golden Retriever bitch was presented 52 days after ovulation for nausea, hyporexia, regurgitation, polyuria, and polydipsia. Reproductive ultrasound demonstrated several viable foetuses, and the bitch was treated conservatively. Two days later, the bitch was hospitalized due to continued hyporexia, vomiting, abdominal distention, and an acute onset of haematochezia. Blood work showed regenerative mild anaemia and neutrophilia with a left shift, metabolic acidosis, hypocalcaemia, hyponatremia, hypochloraemia, hyperkalaemia, and Na:K ratio of 24:1, raisin
