End-of-life issues in caring for patients with dementia: the case for palliative care in management of terminal dementia.

The number of people suffering with dementia is increasing in the general population and the trend is projected to continue as people live longer, especially in countries with developed economies. The most common cause of dementia (among the many other causes) is Alzheimer's dementia, which is considered a terminal illness. The disease could eventually lead to death, or death could occur as a consequence of co-morbid physical complications. The problem of end of life (EOL) care for patients suffering from dementia though spoken of and written about, does not get the attention and system suppor

[The project of a law concerning patients' rights at the end of life].

A bill about Patients' rights and the end of life was adopted unanimously by the National Assembly on November 30, 2004. Article 1 provides … t of unconscious patients can be withheld or withdrawn. Article 7 specifies that adults may draw up advance directives to indicate their wishes for their end of life and their desires regarding the withholding or withdrawal of treatment. A randomized clinical trial of adolescents with HIV/AIDS: pediatric advance care planning.

The objective of this study is to determine if pediatric advance care planning (pACP) increases adolescent/family congruence in end-of-life (EOL) treatment preferences longitudinally. Adolescents aged 14-21 years with HIV/AIDS and their families were randomized (N = 105 dyads) to three-60-minute sessions scheduled one week apart: either the pACP intervention (survey administered independently, facilitated conversation with adolescent and family present, completion of legal advance directive document with adolescent and family present) or an active control (developmental history, safety tips, n

Valued Living intervention to increase advance care planning and well-being in depressed and anxious adults with advanced cancer: Randomized trial in community oncology clinics.

BACKGROUND: Adults with advanced cancer often experience low engagement with advance care planning (ACP) and high levels of depression, anxiety, and fear of death/dying. Access to specialist palliative care is limited. This study tests Valued Living, a novel online acceptance-based group intervention designed to increase ACP and improve psychological/spiritual well-being among adults with advanced cancer screening positive for depression or anxiety. Valued Living was delivered online by social workers in community oncology settings. METHODS: Adults with advanced solid tumor cancer (N = 240) a

What is known about the care and support provided for an ageing population with lived experience of chronic viral hepatitis as they near end-of-life: A scoping review.

Ageing with a chronic hepatitis B (HBV) or hepatitis C (HCV) infection is an emerging public health priority. For people living with chronic viral hepatitis, their disease progression into old age is both underpinned by their existing blood borne virus and the potential emergence of other infectious and non-infectious conditions. These twinned pathways bring additional challenges to the care and support for people as they near end of life. This scoping review sought to examine what is known about the experiences of the end-of-life phase of an increasing population ageing with HBV and HCV in st

[Dying in the hospital from an end-stage non-oncologic disease: a decision making analysis].

BACKGROUND AND OBJECTIVE: The aim of the study was to evaluate palliative decision-making in non- oncological patients who died in an acute care hospital. PATIENTS AND METHOD: 293 patients > 64 years old were analyzed. These patients suffered from dementia (46%), end-stage congestive heart failure (31%) and end-stage chronic obstructive pulmonary disease (23%). We evaluated written information about: do not resuscitate (DNR) orders, graduation of therapeutic decisions, information provided to relatives about prognosis, total withdrawal of other drug therapy and provision of terminal care. Preparation for the end of life in patients with advanced cancer and association with communication with professional caregivers.

OBJECTIVE: Previous studies regarding patients' end of life (EOL) preparation have focused mainly on practical tasks, such as advance directives. In this study, we investigate the relational and personal aspects of EOL preparation, using a patient-completed questionnaire, and examine associations with clinician-patient communication (CPC) and other variables. METHODS: Patients with advanced cancer but with good performance status were recruited from 24 medical oncology clinics, to participate in a cluster-randomised controlled trial of early palliative care intervention. Measures included the…

Association of Physician Orders for Life-Sustaining Treatment With ICU Admission Among Patients Hospitalized Near the End of Life.

IMPORTANCE: Patients with chronic illness frequently use Physician Orders for Life-Sustaining Treatment (POLST) to document treatment limitations. OBJECTIVES: To evaluate the association between POLST order for medical interventions and intensive care unit (ICU) admission for patients hospitalized near the end of life. DESIGN, SETTING, AND PARTICIPANTS: Retrospective cohort study of patients with POLSTs and with chronic illness who died between January 1, 2010, and December 31, 2017, and were hospitalized 6 months or less before death in a 2-hospital academic health care system. The general public's perceptions of advance care planning (ACP) in the Asia-Pacific region: a systematic review.

BACKGROUND: Populations are rapidly ageing. Advance Care Planning (ACP) is an important activity to help prepare for future healthcare needs. Little is known of the perceptions of the general public of the Asia-Pacific region in relation to these activities. Within the context of the Asia-Pacific region, this review aimed to explore awareness of, attitudes towards, experiences of, and the needs, wishes, and/or expectations related to ACP/Advance Directives (AD) of the general public. METHODS: The systematic review protocol was registered with PROSPERO (December 17, 2023): CRD42023491109). PubM…

Habenular and interpeduncularis nuclei: shared components in multiple-function networks.

STUDY AIMS: Advance Directives (AD) are central to Advance Care Planning (ACP), allowing patients to document medical preferences in legally binding documents. Despite broad public awareness and support, only one-fifth of the Swiss population has completed an AD. Consequently, the roadmap for ACP implementation by the ACP National Task Force calls for the development of digital applications to promote ACP in Switzerland. This study details the development process for the mobile application "iDecide" for AD creation based on the Swiss Medical Association (FMH) AD template. The primary aim is to…

Factors Associated with Time to Change of Resuscitation Code (Do-Not-Resuscitate) and Time to Death after a Stroke: Palliative Aspects from a Tertiary Center.

UNLABELLED: <p>Introduction: Strokes rank among the most common acute conditions in neurology, leading to substantial increases in morbidity and mortality rates. It is essential to preserve patients' autonomy in decision-making regarding resuscitation measures to prevent unnecessary interventions and safeguard their right to self-determination. This study aimed to identify the factors associated with an active decision against post-stroke patient resuscitation. METHODS: We conducted a retrospective analysis of 139 patients from 2014 to 2021 at the University Hospital Zurich, Switzerland. All

Resuscitation of the Critically Ill Older Adult.

In 30 years, adults 65 and older will represent 20% of the US population, with increased medical comorbidities leading to higher rates of critical illness and mortality. Despite significant acute illness, presenting symptoms and vital sign abnormalities may be subtle. Resuscitative guidelines are a helpful starting point but appropriate diagnostics, bedside ultrasound, and frequent reassessments are needed to avoid procrustean care that may worsen outcomes. Baseline functional status is as important as underlying comorbid conditions when prognosticating, and the patient's personal wishes shoul

Associations between Patient Characteristics and a New, Early Do-Not-Attempt Resuscitation Order after Intracerebral Hemorrhage.

BACKGROUND: Decisions to limit care, including use of a do-not-resuscitate (DNR) order, are associated with increased risk of death after intracerebral hemorrhage (ICH). Given the value that patient surrogates place on the physician's perception of prognosis, understanding prognostic indicators that influence clinical judgment of outcomes is critical. OBJECTIVE: The purpose of this study was to understand the patient variables and comorbid illnesses associated with DNR orders placed on patients within 72 hours after ICH. DESIGN: Single-center, retrospective review of medical records of 198 c
