Crossing-Borders: Experiences With International Transports on Extracorporeal Membrane Oxygenation: Special Considerations and Challenges.

Extracorporeal membrane oxygenation (ECMO) is a crucial support for patients with severe cardiac or respiratory failure, but its availabilit … l, with 63% supported by venoarterial ECMO. Transport distances averaged 1,200 km, using fixed-wing aircraft 89% of the time. Hospital survival for those transported to Karolinska was 82%, and 36% of transports experienced complications, though no deaths occurred during transport. This study highlights the safety and effectiveness of international ECMO tra …

Noise and light exposure and cardiovascular outcomes: A review of evidence, potential mechanisms and implications.

Cardiovascular diseases (CVD) are the leading cause of global non-communicable disease-related deaths. In recent years there has been increa … search. Future studies should investigate not only road traffic noise but also consider railway and aircraft noise. Additionally, research on light pollution should include younger individuals too. In the future, incorporating individual assessments of noise and light pollution exposure, along with the identification of particularly vulnerable groups, coul …

Neural Networks Based on Synthesized Training Data for the Automated Detection of Chemical Plumes in Passive Infrared Multispectral Images.

Automated detection of volatile organic compounds in the atmosphere can be achieved by applying pattern recognition analysis to passive infrared (IR) multispectral remote sensing data. However, obtaining analyte-active training data through field experiments is time-consuming and expensive. To address this issue, methodology has been developed for simulating radiance profiles acquired using a multispectral IR line-scanner mounted in a downward-looking position on a fixed-wing aircraft. The simulation strategy used Planck's radiation law and a radiometric model along with the laboratory spectru…

Mortality from neurodegenerative diseases in a cohort of US flight attendants.

BACKGROUND: Concern exists about the potential chronic neurological effects among aircrew of exposure to chemical contaminants from engine oil in aircraft cabin air. We evaluated mortality from neurodegenerative diseases among 11,311 former US flight attendants. METHODS: Vital status was ascertained through 2007, and life table analyses were conducted to obtain standardized mortality ratios (SMRs). RESULTS: Amyotrophic lateral sclerosis (ALS) mortality was over twice as high in the cohort as in the US general population, based on nine observed ALS deaths. There was no clear pattern in risk w…

Latent tuberculosis infection among sailors and civilians aboard U.S.S. Ronald Reagan--United States, January-July 2006.

Crews aboard ships live and work in crowded, enclosed spaces. Historically, large tuberculosis (TB) outbreaks and extensive transmission of Mycobacterium tuberculosis have occurred on U.S. Navy ships. On July 13, 2006, smear- and culture-positive, cavitary, pulmonary TB was diagnosed in a sailor aboard the aircraft carrier U.S.S. Ronald Reagan; the patient, aged 32 years, had a negative human immunodeficiency virus test. The M. tuberculosis strain cultured was susceptible to all first-line TB medications. The sailor was born in the Philippines, had latent tuberculosis infection (LTBI) diagnose…

Central retinal artery occlusion by proxy: a cause for sudden blindness in an airline passenger.

The use of gas media in ophthalmologic procedures is relatively commonplace. Scleral buckle and pneumatic retinopexy procedures using air-gas mediums are a widely accepted treatment for retinal detachment. We present a patient who had a scleral buckle with pneumatic retinopexy performed and subsequently flew in a commercial airliner 2 wk later. The patient experienced sudden blindness due to central retinal artery occlusion brought about by expansion of the air bubble when the aircraft reached cruise altitude and a cabin pressure of 8000 ft. The intraocular pressure exceeded the central artery…

Invasions by insect vectors of human disease.

Nonindigenous vectors that arrive, establish, and spread in new areas have fomented throughout recorded history epidemics of human diseases … ns Complex, and, most recently, Aedes albopictus have been facilitated by worldwide ship transport. B-scan ultrasonic measurement of the lumbar spinal canal as a predictor of industrial back pain complaints and extended work loss.

B-scan ultrasonic measurements of lumbar spinal canal diameter were examined as predictors of industrial back pain complaints and extended work loss. Baseline data were collected on 3,020 Washington State aircraft manufacturing workers, and over a mean 3.7-year follow-up period 352 subjects reported industrial back pain complaints. Mean canal measurements of subjects with industrial back pain complaints were smaller at all spinal levels than in subjects without complaints. The mean differences between the groups, however, were extremely small (0.07 mm to 0.51 mm), and not all levels were stati…

Cataract surgery and intraocular lenses in military aviators.

We reviewed the medical records of 23 military aviators who were evaluated by the United States Air Force School of Aerospace Medicine (USAFSAM) after cataract extraction with intraocular lens implantation between 1979 and 1990. The 23 subjects were male Caucasians, with a mean age of 43 years. Of the subjects, 21 were pilots, and, of these, 8 were qualified in high-performance aircraft. There were a total of 28 operated eyes, 24 of which had received extracapsular cataract extractions (ECCE) with posterior chamber lenses. The best-corrected, postoperative vision was 20/20 or better in all eye…

Annoyance and health risk caused by military low-altitude flight noise.

Effects of noise of low-flying military jet aircraft were investigated from demoscopic and epidemiological points of view. Areas with different low-altitude flight noise exposure were compared with one another as to subjective annoyance, casual blood pressure and ear symptoms. With the same energy equivalent sound pressure level (Leq), the subjective disturbance caused by military low-altitude flight noise was essentially greater than that due to ordinary flight noise (in the neighbourhood of civil airports). A comparison of several areas revealed that frequencies of ear symptoms (tinnitus las…

The Case of James Leininger: An American Case of the Reincarnation Type.

Numerous cases of young children who report memories of previous lives have been studied over the last 50 years. Though such cases are more easily found in cultures that have a general belief in reincarnation, they occur in the West as well. This article describes the case of James Leininger, an American child who at age two began having intense nightmares of a plane crash. He then described being an American pilot who was killed when his plane was shot down by the Japanese. He gave details that included the name of an American aircraft carrier, the first and last name of a friend who was on t…

Cervical C2 osteoma, unusual presentation and transoral approach for complete excision.

BACKGROUND CONTEXT: Osteoid osteomas are benign tumors of the bone with 10% occurring in the spine. The authors discuss a case of a 70-year-old lady with an unusual presentation of a cervical C2 osteoma. The presentation was with mild dysphagia and pressure symptoms behind the left ear similar to earache experienced on an aircraft at high altitude with no hearing loss. PURPOSE: The authors postulate that with the appropriate preoperative imaging, operation, and postoperative care, patients with C2 body osteomas can make a good recovery. STUDY DESIGN: This is the case of a patient with a C2 o…

Airplane crash. Traumatologic findings in cases of extreme body disintegration.

A one-engined light aircraft struck the ground at high terminal velocity, producing a large impact crater. The captain and the five passengers were killed, their bodies being dismembered into several hundreds of mostly small tissue pieces. Besides identification, the pathologic investigation dealt with the morphologic features of high-speed impact trauma. In detail, the following peculiarities were found: detachment of the skin with typically sharp-edged margins and sometimes striaelike tears due to overstretching, intracutaneous hemorrhages according to the pleats of the clothing, a strikingl…

Jet lag and motion sickness.

Jet lag. Present day aircraft operating round northern and southern latitudes cross time zones at almost the same rate as the earth rotates, and it is these rapid transmeridian transitions that lead to the syndrome commonly referred to as jet lag. On arrival at their destination, individuals find themselves out of synchrony with the social and time cues of their new environment and, until they adapt, may experience symptoms such as malaise, gastrointestinal disturbance, loss of appetite, tiredness during the day and poor sleep. The severity and exact nature of the problems vary with the direct…
