Prevalence of extended-spectrum beta-lactamases in South America.

In South American countries, the class A extended-spectrum beta-lactamases (ESBLs) so far recognised belong to the CTX-M, Pseudomonas Extended Resistance (PER), SHV and TEM families. ESBL rates in South America are among the highest in the world, probably due to multiple factors. SHV- and TEM-type ESBLs have been frequently encountered, but CTX-M is endemic and widely dominant. PER-type ESBLs seem to be restricted to the southern 'cone' of South America. Community-acquired ESBLs are starting to appear.

Cervical cancer screening programs in Latin America and the Caribbean.

Latin America and the Caribbean (LAC) have a significant burden of cervical cancer. Prophylactic human papillomavirus (HPV) vaccines are an opportunity for primary prevention and new screening methods, such as new HPV DNA testing, are promising alternatives to cytology screening that should be analyzed in the context of regional preventive programs. Cytology-based screening programs have not fulfilled their expectations and coverage does not sufficiently explain the lack of impact on screening in LAC. While improved evaluation of screening programs is necessary to increase the impact of screen

On the origin of HIV-1 subtype C in South America.

OBJECTIVE: Our aim was to investigate the monophyletic status of the HIV-1C that circulates in South America and its phylogenetic relationships with other HIV-1C populations around the world in order to shed light on its the geographic origins as well as the place of introduction in the continent. METHODS: Fifty-one sequences from South America and 46 from non-South American countries, including samples from Africa and Asia, were obtained from the Los Alamos National Laboratory. The data analyzed corresponded to the entire protease and two-thirds of the polymerase domain from the reverse tran

International health, the early cold war and Latin America.

This article offers a panoramic vision of the development of international health in Latin America during the late 1940s and the 1950s, when a series of bilateral and multilateral institutions, such as the World Health Organization and UNICEF, were founded and reshaped. The language, policies, and activities of these new institutional actors were heavily influenced by the context of the early Cold War between the era's superpowers: the United States and the Soviet Union. Vertical campaigns against yaws and malaria--implemented under the leadership of Fred L. Soper, director of the Pan American

Regional analysis on the occurrence of oral clefts in South America.

The aim of this work was to search for unequal birth prevalence rates (BPRs) of cleft lip +/- cleft palate (CL/P), and cleft palate only (CPO), among different geographic areas in South America, and to analyze phenotypic characteristics and associated risk factors in each identified cluster. Included were 5,128 CL/P cases, 1,745 CPO cases, and 3,712 controls (like-sexed, non-malformed liveborn infant, born immediately after a malformed one, in the same hospital), over 4,199,630 consecutive births. They were ascertained between 1967 and 2004, in 190 maternity hospitals of the ECLAMC (Estudio Co

A history of biological disasters of animal origin in North America.

This paper examines past occurrences in North America relevant to the possibility of biological disasters with animal origins. With respect to naturally occurring animal disease outbreaks, North America, while not as adversely affected by epizootics as other regions, has had its fair share of such outbreaks of both 'traditional' and emerging animal diseases. The traditional category includes such diseases as anthrax, classical swine fever, bluetongue, brucellosis, foot and mouth disease, and the family of equine encephalomyelitis viruses. The emerging diseases include relatively more recent cu

Simuliidae and the transmission and control of human Onchocerciasis in Latin America.

Factors that affect the propensity of a simuliid species to act as a host to Onchocerca volvulus and to naturally transmit this filarial worm in nature are discussed. Presence or absence of a cibarial armature is believed to be a major factor that has been previously overlooked and this is considered in relation to the choice of control methods currently advocated for onchocerciasis. The current epidemiological studies, transmission dynamics and relevant control measures are discussed for each onchocerciasis focus in Latin America.

Prevalence of fractures among the Thalassemia syndromes in North America.

Historically, fractures are cited as a frequent problem in patients with Thalassemia prior to optimization of transfusion and chelation regimens. The aim of this study was to determine the prevalence of fractures in a contemporary sample of North American patients with Thalassemia. The North American Thalassemia Clinical Research Network (TCRN) database registry was used to gather historical data on 702 patients with common alpha and beta-Thalassemia diagnoses including Thalassemia Major (TM), Intermedia (TI), E/Beta, homozygous alpha Thalassemia (AT), Hemoglobin H disease (HbH) and HbH with C

Epidemiology of heartworm: what is happening in South America and Mexico?

Canine heartworm has been known to exist in the Americas for more than 150 years (1847), and the first South American report was published approximately 30 years later (1875). The first human case was reported in 1887 and the first feline case in 1921. Coincidently, these two reports were from Brazil, where most surveys of the disease during the past two decades have been conducted in dogs. Feline cases are seldom found in the South American literature, although feline heartworm disease has been reported in Venezuela and Brazil. The prevalence in Mexico seems to be stable over the past 10 year…

Congestive heart failure in Latin America: the next epidemic.

Coronary artery disease is the main cause of congestive heart failure (CHF) in all populations. Latin American countries (LAC) are undergoing the first phase of an epidemic of coronary artery disease that probably will lead to an increased incidence of CHF. The progressive implementation of successful interventions, such as early reperfusion and rehabilitation programs, should increase the survival of patients with acute myocardial infarction and the population at high risk of CHF. The increasing prevalence of risk factors, such as diabetes, hypertension, and obesity, and the ageing of the pop

West Nile virus and North America: an unfolding story.

Before the introduction of the West Nile virus (WNV) into the United States of America (USA) in 1999, conditions in North America were ideal for an arboviral epidemic. Such factors as the large, susceptible and non-immune animal and human populations, the presence of competent vectors, increasing international travel and commerce, existing methods for rapid dissemination and an ill-prepared animal and public health infrastructure all combined to create the essential elements for a severe animal and public health crisis--the 'perfect microbial storm'. The introduction of WNV into New York City

Community self-help and the homeless poor in Latin America.

Finding realistic housing solutions that are able to respond to the realities of poverty in the developing, or newly industrializing, world are frequently distinct from those suited to the developed world due to levels of poverty and differing welfare regimes. This requires a different understanding of the concept of housing and shelter for developing and developed countries. Population increase and emerging habitation patterns in parts of Latin America have required that policy-makers review traditional 'top down' approaches to the way the homeless poor are treated and how self-help or 'botto

Marketing to America's youth: evidence from corporate documents.

OBJECTIVE: To evaluate the claim that the tobacco industry does not market its products to youth. DESIGN: The data for this study come from tobacco industry documents collected from the tobacco industry's document websites, presently linked at http://www.tobaccoarchives.com. The websites were searched using "request for production" (RFP) codes, specified keyword searches, and serendipitous terms identified in document citations found with RFP and keyword searches. RESULTS: Industry documents show that the cigarette manufacturers carefully monitored the smoking habits of teenagers over the pa

Chronic wasting disease in deer and elk in North America.

Chronic wasting disease (CWD) has emerged as an important disease of wildlife in North America. The disease is a unique member of the transmissible spongiform encephalopathies (TSEs) or prion diseases, which naturally affect only a few species. Of the TSEs, CWD is the only one found in free-ranging species. However, interest in CWD has recently grown, by association with the better-known TSEs such as variant Creutzfeldt-Jakob disease of humans and bovine spongiform encephalopathy. Knowledge of the geographic distribution of CWD, though still limited, has greatly improved since the mid-1990s as

Survival of the extremely preterm infant in North America in the 1990s.

Survival of extremely premature infants has been significantly higher in the last decade than previously, and may well have improved during this time. The majority of infants greater than or equal to 25 weeks' gestation survive today. Survival of infants 23 and 24 weeks' gestation is significantly lower, but is by no means negligible. Reports of survival of infants less than 23 weeks or less than 500-g birth weight are not unique. Moreover, the maximum survival of infants less than or equal to 25 weeks possible with current state-of-the-art care is not known. Currently available data do not al

Living arrangement transitions among America's older adults.

This analysis describes the relationship between age and transitions from four living arrangements: living alone, living with spouse only, living with a child, and living with a spouse and child. Data from the National Survey of Families and Households, collected in 1987-88 and 1992-93, are used to calculate destination-specific hazard rates by age and then construct multiple-decrement life tables. Living alone or with a spouse are the most stable living arrangements during the early stages of later life, whereas for the oldest-old, living with a child is the most stable living arrangement. Th

Medical ethics and the executing process in the United States of America.

The article focuses on the ethical and moral issues raised by the participation of physicians in the execution process in the United States of America. Discussion centres on two main areas. Firstly, participation in the actual execution, particularly where the method is lethal injection; and secondly psychiatric assessment and treatment of inmates who are deemed not competent in law for execution. It is argued that as an execution is a harm, participation runs counter to the ethics of the medical participation and cannot be justified even on the basis of relieving pain. Treatment of incompeten
