Transfusion therapy in autoimmune hemolytic anemia.

Autoimmune hemolytic anemias include warm and cold reactive autoimmune hemolytic anemias (WAIHA and CAD), paroxysmal cold hemoglobinuria (PCH), and drug-induced hemolytic anemias. If autoimmune hemolytic anemia is suspected, the clinical history should be communicated to the transfusion service, because specific tests to confirm these diagnoses are not routinely performed. This includes evaluation of the patient's RBCs for bound IgG and complement (direct antiglobulin test) and evaluation of the patient's serum for antibodies with RBC specificity. For warm or cold autoimmune hemolytic anemia, …

[Causes of anemia after kidney transplantation].

In this paper possible causes of anemia in patients after kidney transplantation were investigated. Anemia developed in 29 patients 14 to 22 months after kidney transplantation. At the time when these patients became anemic fourteen had normal graft function while the others 15 suffered chronic graft failure due to chronic rejection in 11 patients combined with cyclosporine nephrotoxicity in other four patients. Anemia in patients with normal graft function was caused by iron (in 8), folic acid deficiency (in 16) and vitamin B12 deficiency (in 2 patients). In this patients group anemia improve…

[Genetic counseling and sickle cell anemia].

In sickle cell anaemia genetic counselling benefits from the easy diagnosis and, as early as the foetal stage, from the heterozygosity (AS) and the major syndromes. Its main objective is to inform as many as possible of the subjects concerned and in particular couples with genetic risk. Counselling gives them all the elements they require to decide freely about their offsprings. It is part of the management of sickle cell anaemia patients and is often associated with the neonatal diagnosis. The ever increasing number of justified requests for a prenatal diagnosis and the early time of these re

[Cardiovascular pathology in sickle cell anemia].

The author, in the light of 14 cardiovascular pathology cases among patients affected by sickle cell disease, itemizes the following data: Cardiovascular pathology in sickle cell disease can be a preoccupation not only among homozygotes but also among heterozygotes. The clinical polymorphism of this pathology concern not only the common anemia heart disease and the type of sickle cell heart disease what looks like a rheumatic heart disease, but also includes cases of cor pulmonale among young patients, sickle cell cardiomyopathy, and myocardial infarction among young. The sickle cell disease a…

Aplastic anemia associated with antithyroid drugs.

Prognosis in aplastic anemia is usually linked to the degree of hypoplasia in the bone marrow and pancytopenia in the blood. The authors were, therefore, intrigued when a patient with methimazole-associated aplastic anemia who satisfied criteria for severe disease recovered rapidly and completely once her drug was withdrawn. Review of the English language literature revealed ten fully documented cases of aplastic anemia associated with use of the antithyroid drugs methimazole, carbimazole, and propylthiouracil. Analysis of the ten and of an eleventh case presented here indicated that the disor…

[Autoimmune hemolytic anemia and CREST syndrome].

There are few studies addressing the frequency and etiology of anemia in progressive systemic sclerosis. Anemia is present in about 25% of these patients. Among the implicated causes are ferropenia, generally associated with gastrointestinal hemorrhage, and renal failure. In a considerable number of patients, the specific etiology of anemia remained unknown. We report a 59-year-old female with autoimmune hemolytic anemia and incomplete CREST syndrome. We emphasize the rarity of autoimmune hemolysis as a cause of anemia in systemic sclerosis, and we discuss its significance as indirectly suppor…

Carboplatin-associated thrombotic microangiopathic hemolytic anemia.

Thrombotic microangiopathic hemolytic anemia has been associated with several chemotherapeutic agents. The authors describe a patient who developed this syndrome while receiving carboplatin, an analog of cisplatin. The clinical course was marked by encephalopathy and multifocal neurologic deficits. Progressive brainstem dysfunction culminated in coma and respiratory arrest. Pathologic examination revealed widespread microvascular thrombosis, particularly severe in the heart, kidney, and brain. Although the pathogenesis of chemotherapy-related thrombotic microangiopathy remains unclear, an elev…

[Autoimmune hemolytic anemia and myelodysplastic syndromes].

The association between autoimmune haemolytic anaemia (AHA) and myelodysplastic syndromes (MDS) was found in seven out of 156 patients with SMD who received several transfusions as supportive therapy. Three patients were diagnosed of refractory anaemia (RA), three more of refractory anaemia with excess of blasts (RAEB) and one of refractory anaemia with ring sideroblasts (RARS). All patients showed a positive direct antiglobulin test (DAT) and the presence of anti-red blood cell IgG type antibodies, both in serum and eluate. Clinically, three patients showed signs of low grade haemolysis. It i

Thiamine-responsive anemia in DIDMOAD syndrome.

Two children with the DIDMOAD syndrome (diabetes insipidus, diabetes mellitus, optic atrophy, deafness) developed a megaloblastic and sideroblastic anemia, neutropenia, and borderline thrombocytopenia. Plasma thiamine concentration was low in one patient and normal in the other; in both children, thiamine pyrophosphate in erythrocytes and thiamine pyrophosphokinase activity were lower than the lowest values observed in control subjects. A month after institution of treatment with thiamine, the hematologic findings had returned to normal and the insulin requirements had decreased. Withdrawal of…

Death from drug-induced hemolytic anemia.

Fatal hemolytic anemia occurred in a 71-year-old man after trimethoprim-sulfamethoxazole was given for presumed cystitis. Administration of this combination has previously caused multiple hematologic reactions by affecting folic acid metabolism. Megaloblastic anemia and neutropenia have been produced by both of these agents, while sulfamethoxazole alone has induced acute hemolytic anemia in patients with hereditary deficiency of glucose-6-phosphate dehydrogenase. Although hematologic complications of trimethoprim-sulfamethoxazole treatment usually follow long-term or high-dose therapy, acute r…

[Cerebrovascular complications of sickle-cell anemia].

Cerebrovascular disorders are frequent in sickle-cell anemia. They occur mainly in homozygous children. Traditionally, they were believed to result from an arteriolar sickle-cell thrombosis, the "sludge" phenomenon, which is generally responsible for sickle cell anemia disorders. It is now well known that several other kinds of cerebral damages are involved such as moya-moya syndrome, cerebral hemorrhage, subdural hematoma, extra-dural hematoma and cerebral thrombophlebitis. In this disease, the identification of these different processes cannot be always made by a simple clinical examination.…

Tocainide-induced reversible agranulocytosis and anemia.

Tocainide is an effective oral antiarrhythmic agent. We report a 77-year-old man who developed agranulocytosis and anemia while receiving tocainide therapy. These hematologic abnormalities were detected on routine evaluation six weeks after beginning tocainide therapy. The absolute granulocyte count decreased to 50/mm3 (0.05 X 10(9)/L). The anemia was mild; hemoglobin count, 10.9 g/dL (109 g/L). These abnormalities were associated with local and stromal adipocytic bone marrow damage, and decreased production of red blood cells and granulocytes. The platelet count was not affected. The patient …

Lymphocyte surface phenotypes in pernicious anemia.

Because an immunological defect of an unknown nature is thought to be a factor in the pathogenesis of pernicious anemia, we studied lymphocyte surface phenotypes in 40 patients and compared them with 113 healthy controls. The only significant difference that emerged was in their slightly decreased number and proportion of surface immunoglobulin (lambda chain) -bearing cells. Of specific interest, the numbers of OKT4+ and OKT8+ lymphocytes and the T4+/T8+ ratios were not significantly different from control values. A sizable minority of patients had increased ratios, while a smaller number had …
