Myoclonic movement induced by etomidate is a common but undesirable problem during general anesthesia induction. To investigate the influence of pretreatment with low-dose ketamine on the incidence and severity of myoclonus induced by etomidate, 104 patients were randomized allocated to 1 of 2 equally sized groups (n = 52) to receive either intravenous low-dose ketamine 0.5 mg/kg (group K) or an equal volume of normal saline (group S) 1 minute before induction of anesthesia with 0.3-mg/kg etomidate. The incidence and severity of myoclonus were assessed for 2 minutes after administration of eto

Two methods for administering general anaesthesia are widely used: propofol-based total intravenous anaesthesia (propofol-TIVA) and inhalation volatile agent-based anaesthesia. Both modalities, which have been standards of care for several decades, boast a robust safety profile. Nevertheless, the potential differential effects of these anaesthetic techniques on immediate, intermediate, and extended postoperative outcomes remain a subject of inquiry. We discuss a recently published longitudinal analysis stemming from a multicentre randomised controlled trial comparing sevoflurane-based inhalati

Local anesthetic-associated cardiac toxicity following caudal epidural blockade is, fortunately, a rare event. Prompt recognition and early treatment is the key to successful resuscitation. Early use of the lipid emulsion Intralipid in bupivacaine-induced cardiac toxicity may lead to a good outcome.

We aimed to evaluate the effect of remimazolam-based general anesthesia on cellular immune function and postoperative recovery quality in patients undergoing laparoscopic radical colorectal cancer surgery. A total of 90 patients scheduled for elective laparoscopic colorectal cancer radical surgery were randomly divided into 2 groups: the remimazolam group (Group R) and the propofol group (Group P), with 45 patients in each group. Anesthesia induction in Group R involved intravenous remimazolam, and in Group P, intravenous propofol until the loss of consciousness (modified observer's assessment

BACKGROUND AND OBJECTIVES: Although regional analgesia is considered an important component of optimal pain management, use of peripheral nerve blocks for total hip arthroplasty remains controversial. Since the obturator nerve innervates the anteromedial part of the joint capsule, we hypothesized that an obturator nerve block would decrease the opioid consumption after total hip arthroplasty.
METHODS: In this single center, prospective, triple blinded study, we randomly allocated 60 patients undergoing total hip arthroplasty under opioid-sparing total intravenous general anesthesia to a preop

OBJECTIVES: The primary aim was to compare the changes in regional cerebral oxygen saturation (rSO2) and cerebral blood flow velocity (CBFV) during sevoflurane and intravenous anesthesia when the end-tidal carbon dioxide partial pressure (PETCO2) changed in infants undergoing ventricular septal defect (VSD) repair.
DESIGN: Prospective, observational study.
SETTING: Tertiary care hospital.
PARTICIPANTS: Patients younger than 6 months with VSDs.
INTERVENTIONS: End-tidal carbon dioxide was increased by decreasing tidal volume or respiratory rate.
MEASUREMENTS AND MAIN RESULTS: The infants we

PURPOSE: Sphenoidal electrode (SE) insertion can cause pain, for which local anesthesia with lidocaine or intravenous administration of fentanyl has been advocated by different epilepsy treatment centers. Transient facial palsies have been observed after SE insertion. Their frequency of occurrence, distribution, and duration have not been well characterized, however. We hypothesized that this complication is due to the effect of local anesthesia on the peripheral branches of the seventh cranial nerve. To test this hypothesis, we compared the incidence and characteristics of facial palsy during

This paper deals with the control, registration and statistical evaluation of blood pressure variations (systolic, diastolic and average) after an i.v. and submucous injection of fixed quantity local anaesthetic solutions (0.05 ml) in white hamsters. The solutions used were: lidocaine 2% with adrenaline 1/80,000, lidocaine 3% with noradrenaline 1/25,000. The intravenous injection of all three local anaesthetic solutions has caused an increase of the arterial pressure parameters which was statistically significant (P less than 0.01). The submucous injection of lidocaine 2% solution with adrenal

A case report of a child with Pompe's disease (glycogen storage disease type II), who underwent two general anesthetics, is presented. The progressive infiltration of heart and skeletal muscle with glycogen results in a severe form of cardiomyopathy and respiratory muscle weakness. Death usually occurs by 1 year of age from respiratory insufficiency or end-stage cardiomyopathy. Consequently, there are significant problems in the anesthetic management of these patients. The patient, a female child of 6 months presented signs of cardiac failure and took treatment with olprinone and diuretic. The

Emergence delirium is a common complication after sevoflurane-anesthesia and have a serious impact on children undergoing cleft palate surgery. The aim of this study was to compare the effect of propofol and dexmedetomidine on emergence delirium in children. Ninety children aged 8 to 24 months, underwent cleft palate repair, were enrolled in the study. Children were randomly assigned to 3 groups after the induction: Group C (intravenous infusion 0.9% saline), Group P (intravenous infusion 2 mg/kg/hour propofol), and Group D (intravenous infusion 0.5 μg/kg/hour dexmedetomidine). Emergence delir

Thoracic surgical procedures present specific challenges in anaesthesiology. One lung ventilation (OLV) regularly required in this context does improve surgical exposure, but leads to characteristic pathophysiological changes: the right-to-left shunt increases, dead space ventilation rises, and hypoxic pulmonary vasoconstriction (HPV) elevates pulmonary vascular resistance. These mechanisms have a direct impact on gas exchange and haemodynamics and demand a differentiated anaesthesiological strategy. A comparison between the performance of total intravenous anaesthesia (TIVA) and balanced gene

Paroxysmal extreme pain disorder is a very rare condition featured by flushing of the lower half of the body and excruciating burning pain caused by any stimulus below the waist or in the perianal region. It may be associated with cardiovascular instability, and thus has anesthetic implications. This condition is well described in neurological literature, but to our knowledge, this is the first case report of a patient with paroxysmal extreme pain disorder requiring anesthesia for a surgical procedure.

General anaesthesia influences the oxygen supply/demand ratio of the brain and may thus be able to improve neurological outcome after carotid artery surgery. Among the anaesthetic agents, barbiturates and isoflurane produce a marked reduction of the cerebral metabolic rate and activity. But the incidence of neurological deficits in the immediate postoperative period is low, while the possible causes of any neurological deficits are multiple. Consequently, to date, no particular anaesthetic agent or management has been shown to influence the neurological results.

Cerebrospinal fluid pressure (CSFP) by lumbar puncture, systemic blood pressure (BP) and pulse rate (PR) were measured for 15 minutes during the induction phase of general anaesthesia in seven groups of six healthy female patients each. Intravenous drugs, thiopentopne 5 mg . kg-1, alfathesin 50 microliters . kg-1 and diazepam 0.5 mg . kg-1 given in 10 to 20 seconds caused a fall of CSFP and BP, whereas ketamine 2 mg . kg-1 and a three-minute induction with halothane three per cent, trichloroethylene one per cent, or methoxyflurane 0.75 per cent caused a sharp highly significant but short-lived

Dysphagia and progressive swallowing problems due to motoneuron death is one of amyotrophic lateral sclerosis (ALS) symptoms. Malnutrition and body weight loss result in immunological disturbances, fatigability and increase risk of secondary complications in ALS patients, percutaneous endoscopic gastrostomy tube (PEG) placement representing a well-recognized method for malnutrition correction and potentially increasing life expectancy. However, despite nutritional correction, occasional rapid neurological deterioration may develop after PEG placement. We have hypothesized that this decline can

A case is presented of asymptomatic cyanosis of the hands, seen four hours after dental surgery under intravenous, local and inhalational anaesthesia. Alphaprodine hydrochloride, a synthetic opiate and one of the agents used, may produce dilatation of the venous bed with passive venous pooling. Such a mechanism is thought to have occurred in this case, manifesting as cyanosis. A weakly positive intradermal skin test to alphaprodine suggests immediate type hypersensitivity as a possible underlying cause.
