Moyamoya disease (MMD) is a chronic, progressive cerebrovascular disorder characterized by steno-occlusive changes in the intracranial internal carotid arteries and the development of fragile collateral networks. Imaging plays a pivotal role in diagnosis, disease staging, and management, yet the expanding range of available imaging modalities has resulted in heterogeneous evidence that remains difficult to synthesize. This scoping review aimed to systematically map and critically appraise imaging-based diagnostic approaches used in MMD, summarizing their diagnostic performance, clinical utilit

BACKGROUND: Stereoelectroencephalography (SEEG) is a procedure used to localize the epileptogenic zone in patients with medically refractory epilepsy, involving the stereotactic implantation of electrodes into brain parenchyma. Magnetic Resonance Imaging (MRI), Digital Subtraction Angiography, and Computed Tomography have been used preoperatively to prevent Intracranial Hemorrhage (ICH) by identifying electrode-vessel conflicts (EVC's) on planned electrode trajectories. There is variation in the use of Digital Subtraction Angiography and non-invasive sequences for vascular planning. Digital Su

Carotid cavernous fistula (CCF) is a condition with abnormal flow from the carotid artery to the cavernous sinus. The characteristic clinical findings are ophthalmic monoplegia, ptosis, exophthalmos, chemosis, Horner's syndrome, facial pain, loss of sensation in the distribution of the trigeminal nerve, and headache. Digital subtraction angiography (DSA) is the gold standard modality for evaluating CCF. An 18-year-old patient came with decreased bilateral visual acuity after a traffic accident. CT angiography was performed on the patient and found a dilated right superior ophthalmic vein. CCF

A 22-year-old man suffered severe sudden onset head and neck pain after being pushed from behind during an assault. Physical examination was normal. Cervical MRI demonstrated an intradural hematoma, anterior to the cord, between C2-4. Subsequent contrast enhanced MR angiography and digital subtraction vertebral angiography confirmed that the cause of the hemorrhage was a fusiform (presumed dissecting) pseudoaneurysm of the artery of the cervical enlargement at its junction with the anterior spinal artery. The aneurysm was managed conservatively. Follow-up angiography demonstrated that the aneu

Renovascular hypertension (RVH) is a common cause of secondary hypertension. Noninvasive tests such as computed tomography angiography (CTA) or magnetic resonance angiography have a high specificity and sensitivity for the diagnosis. Digital subtraction angiography (DSA) is, however, the gold standard with which RVH can be reliably diagnosed. We report the case of a 30-year-old Chinese man with right ureter malformation and stenosis of the entire right renal artery. We discuss how to distinguish between inconsistencies in CTA results and DSA findings. Interobserver variability is an important

The authors report on a 38-year-old woman with a dislocated hangman fracture associated with unilateral vertebral artery (VA) occlusion. The patient presented with a mild central cord syndrome, as well as anterior subluxation of the C-2 vertebral body upon C-3, bilateral neural arch fractures, and a unilateral locked facet joint. Digital subtraction angiography revealed occlusion of the right VA, with the posterior cerebral circulation entirely dependent on the left VA. Intraoperative angiography demonstrated that complete reduction of the dislocation would have caused severe stenosis of the l

Although anterior circulation transient ischemic attacks (TIAs) tend to be more common in patients with extra- cranial carotid arterial disease than in those with intracranial carotid or middle cerebral arterial disease, the authors recently encountered 4 patients with both recurrent, stereotypical TIAs as well as isolated stenosis of their petrous internal carotid artery (ICA). While the gold standard for establishing the diagnosis of intracranial large-artery disease has always been conventional angiography, magnetic resonance angiography changes, confirmed with intra-arterial digital subtra

A 41-years-old woman presented with left-sided headache, diplopia, and ptosis of the left eyelid. Cerebral computed tomography angiography revealed a left persistent primitive trigeminal artery cavernous sinus fistula and a dissecting aneurysm in the V4 segment of the left vertebral artery. Digital subtraction angiography confirmed the diagnosis. The fistula was successfully occluded using dual-microcatheter coil embolization combined with Onyx-18 injection, assisted by balloon protection of the internal carotid artery. The vertebral artery dissecting aneurysm was managed conservatively by adm

Contrast-induced encephalopathy (CIE) is a rare complication of the intravascular application of a contrast agent. CIE can be manifested by headache, cortical blindness, consciousness disorders, seizures, or focal neurological deficit. Neurological symptoms are typically transient with temporary abnormal findings on a brain scan. Urgent neuroimaging is important to obtain the correct diagnosis, especially in cases that require an acute management and treatment. We present a case of CIE after a digital subtraction angiography of the vertebral arteries in the patient with a symptomatic pre-occlu

A 50-year-old man with a history of left-sided retrosigmoid craniotomy for vestibular schwannoma (VS) resection 19 years prior presented with severe headache and left cerebellopontine angle subarachnoid hemorrhage (SAH). Digital subtraction angiography demonstrated a dissected, nonfunctional left posterior inferior cerebellar artery with direct fistulization at the left transverse sinus (Video 1). The lesion was treated with endovascular Onyx embolization. The patient recovered without neurologic deficit. Five additional cases of new dural arteriovenous fistula arising after VS resection have

A 31-year-old man presented with dull headache and memory disturbance lasting for one week. Computed tomographic scans revealed acute hydrocephalus. The cerebrospinal fluid contained 53 leukocytes/mm(3), with a mononuclear preponderance and no erythrocytes. Magnetic resonance imaging revealed hydrocephalus and leptomeningeal enhancement. Magnetic resonance angiography and digital subtraction angiography showed supraclinoid occlusion of the right internal carotid artery, which resembled unilateral moyamoya disease. Neuroendoscopic biopsy of a lesion in the septum pellucidum revealed noncaseatin

We report a case of bilateral aplasia of the internal carotid arteries. A 59-year-old man was admitted with generalized convulsions. Computed tomography and magnetic resonance imaging of the head showed normal findings. Three-dimensional computed tomography, angiography, and intra-arterial digital subtraction angiograms showed bilateral aplasia of the internal carotid arteries. The bilateral ophthalmic arteries were filled from the hypoplastic carotid arteries, and the anterior and middle cerebral arteries were filled from the basilar artery via the posterior communicating arteries. Digital subtraction angiography (DSA) and conventional film angiography (CFA) were compared prospectively in 50 patients with trauma of the extremities. Each patient underwent both procedures with comparable imaging parameters and injections. Three angiographers reviewed the angiograms for the presence of seven angiographic signs of arterial injury. The two modalities showed pseudoaneurysms, arteriovenous fistulas, vessel displacement, occlusion, and focal narrowing equally well. CFA was superior to DSA in delineating intimal dissection; DSA was better for demonstrating extravasation. Clinical

Perforating arteries are thin and long vessels which originate from the main cerebral arteries. Subarachnoidal hemorrhage from a perforator aneurysm is rare. Here, we report on a 70-year-old woman who presented with severe subarachnoid hemorrhage from a fusiform lateral anterior (A1) artery perforator aneurysm. Unexpectedly, digital subtraction angiography was non-diagnostic. MR imaging, however, demonstrated occlusion of the aneurysm secondary to thrombosis. Surgery was performed to remove the associated hematoma, and histopathological examination verified intraaneurysmal thrombosis.

An 8-year-old boy from Xuan-Han County, Sichuan Province, China presented with 6 months headache, nausea and vomiting. A computed tomography scan revealed multiple cerebral hemorrhages. Etiology of the cerebral hemorrhages was investigated by digital subtraction angiography, and cerebral aneurysm was entertained. Twenty-six days after his admission to the hospital, a flatworm emerged from the right eye of the patient. The worm was identified as Fasciola hepatica, based on the morphological characteristics of the worm.

Popliteal artery entrapment syndrome is a rare but potentially limb threatening peripheral vascular disease occurring predominantly in young adults. We report a case of a 17-year-old boy who presented with intermittent claudication on the right side. Digital subtraction angiography revealed bilateral, focal narrowing of the popliteal arteries. Magnetic resonance imaging displayed compression of the arteries by the medial head of the gastrocnemius muscles. The mechanism, presentation, imaging findings, and management of this rare disease are discussed.

The case of a sixty-five-year-old man with discrete coronary aneurysms, associated with coronary artery ectasia, stenoses, and total occlusion of the left anterior descending artery, that resulted in an acute anterior myocardial infarction, is presented. Aortography and digital subtraction angiography revealed marked aortic and iliac elongation, tortuosity, and aneurysmal dilatation as well as dilatated femoral and popliteal arteries with aneurysmal dilatation in places. It is considered that the case reflects the broad spectrum of atherosclerosis manifestations.

Aneurysms of the coronary arteries may cause angina, myocardial infarction or sudden death due to thrombosis, embolization or rupture. The patient described herein may represent the first reported case of a large coronary artery aneurysm compressing the upper caval vein, and thereby producing a significant inflow gradient. The anatomical details were demonstrated by digital subtraction angiography (DSA). The patient successfully underwent resection of the aneurysm and reconstruction of the coronary artery with a saphenous vein bypass graft.

Pulmonary Arteriovenous Fistula (PAVF) is a rare type of pulmonary vascular malformation, resulting in right-to-left shunting (RLS) due to an abnormal connection between the pulmonary artery and the pulmonary vein. Approximately 80%-90% of congenital PAVF cases are associated with hereditary hemorrhagic telangiectasia (HHT), which can cause complications including hypoxemia, stroke and brain abscesses. Its clinical manifestations are associated with lesion size, blood flow volume, number of lesions, patient age and hormonal status, with a notably elevated risk of complications and rupture duri
