The occurrence of haemolytic glaucoma in phakic eyes due to ghost cells has not previously been reported. Three cases of haemolytic glaucoma occurring over two years after massive vitreous haemorrhage in patients with an intact lens iris diaphragm are described. In one case there was histological confirmation of the presence of ghost cells in the anterior chamber. The mechanism proposed for the passage of ghost cells to the anterior chamber is through a defect in the anterior hyaloid face, created as the vitreous liquefies and degenerates. A prospective controlled trial of topical 0.1% dexamethasone drops was undertaken with the purpose of determining its efficacy in the prevention of aphakic macular edema. Epikeratophakia was performed in 61 children for the correction of aphakia after the removal of unilateral congenital or traumatic cataracts; 51 patients (54 grafts) have more than 6 months follow-up. In the first 27 grafts, 8 (30%) failed, but in the last 27 grafts, only 2 (7%) failed, largely due to improved surgical and tissue handling techniques. One hundred twenty-seven trabeculectomies performed on aphakic eyes at the Bascom Palmer Eye Institute (BPEI) between 1972 and 1982 were reviewed. In a study of the ocular absorption and elimination of a topically applied non-steroidal anti-inflammatory drug (NSAID), flurbiprofen, the compound was well absorbed into rabbit ocular tissues and was highly concentrated in the rabbit cornea. In aphakic eyes, more drug penetrated to the vitreous and choroid-retina area than in normal rabbit eyes, although corneal concentrations were still high. Twenty-five eyes of 18 pediatric patients (aged 7 1/2 months to 14 years 8 months) who had had cataract extractions were examined for evidence of cystoid macular edema (CME) with fluorescein angiography. No patient showed CME when studied from five weeks to four years after surgery. One patient with Peter's anomaly, glaucoma, and chronic iridocyclitis had intense fluorescence of the aqueous and vitreous that precluded evaluation of the macula. Even if this patient is assumed to have had CME, its incidence in our population of aphakic pediatric patients was no more than 4%.

Thirty-twop patients, ranging in age from three weeks to 22 years, were fitted with an extended wear 100% silicone lens. The longest period of time a lens remained in the eye without being removed was 120 days. Most lenses were worn an average of six weeks before cleaning was required. No cases of infection or corneal edema were observed that required discontinuing use of the lens.

In order to assess the applicability and safety of cellulose acetate buterate (CAB) contact lenses for extended wear in aphakia, 139 lenses were fitted in 102 aphakic patients. These lenses were removed periodically for cleaning only. There has been an average follow-up of 24.75 months on 99 patients (135 lenses), with a success rate of 80%. No serious complications or corneal vascularization have occurred. The major problems encountered were due to lack of tint, increased tendency to accumulate mucus, and the need for periodic insertion and removal.
