Updated 2026 Japanese Diagnostic Criteria for Takayasu Arteritis.

Takayasu arteritis (TAK) is a chronic, refractory large-vessel vasculitis designated as a Designated Intractable Disease in Japan. The Japanese diagnostic criteria for TAK, last revised in 2017, have been updated to the 2026 version as part of the Japanese Circulation Society 2026 guidelines on the management of large vessel vasculitis, and are presented here in English for the first time. The revision was conducted through a 3-round modified Delphi process involving a panel of 21 experts, supplemented by email discussion and an in-person meeting. The updated criteria incorporate 3 major chang…

You Never Think Enough About Giant Cell Arteritis.

PURPOSE: Diagnosis of giant cell arteritis (GCA) can sometimes be challenging, especially in the setting of a negative CRP. METHODS: We report a challenging case of GCA. RESULTS: A 73-year-old male patient presented with unilateral paracentral acute middle maculopathy (PAMM). Because the patient demonstrated homolateral internal carotid artery stenosis, and two negative CRP tests, the diagnosis of GCA could have been overlooked if diffuse aortitis had not been observed on supraortic CT-scan. The diagnosis was further confirmed with PET scan, demonstrating femoral arteritis in addition to aor…

Sustained drug-free remission in giant cell arteritis.

OBJECTIVES: The objectives of this study were to evaluate the frequency and timing of sustained drug-free remission (SDFR) in patients with GCA and to identify potential predictive factors of this outcome. METHODS: A retrospective review of all patients included in the large Spanish multicentre registry for GCA (ARTESER) with at least 2 years of follow-up was undertaken. SDFR was defined as the absence of typical signs, symptoms, or other features of active GCA for ≥12 months after discontinuation of treatment. RESULTS: We included 872 patients. Forty-seven percent had received concomitant t

Calciphylaxis Mimicking Giant Cell Arteritis: A Case Report.

INTRODUCTION: Calciphylaxis is a condition that causes vascular calcification and intimal proliferation with thrombotic occlusion of small-to-medium-sized vessels. CASE PRESENTATION: We describe a case of a 64-year-old woman who presented with a clinical picture that was suggestive of anterior arteritic ischemic optic neuropathy due to giant cell arteritis (GCA), but was found to have calciphylaxis confirmed by histologic examination. When calciphylaxis affects the blood supply to the eye, commonly it causes sudden vision loss, pallid optic disk edema, and a relative afferent pupillary defect…

Postpartum onset Takayasu's arteritis presenting with aortic dissection.

Takayasu's arteritis (TA), also known as pulseless disease and young female arteritis, is a chronic inflammatory large-vessel vasculitis (LVV). TA is pathologically characterized by arterial wall thickening, stenotic/occlusive lesions, aneurysm formation, and dissection. TA usually develops between 20 and 30 years of age. However, pregnancy and puerperium can affect the immune system, and several cases of postpartum onset or flare-up of TA have been reported. Herein, we report an extremely rare case of postpartum-onset TA complicated by aortic dissection. This is a case of Postpartum onset Tak…

Visual outcome after endovascular intervention in Takayasu arteritis.

Takayasu arteritis is a chronic multisystem vasculitis which has been known to be associated with myriad of ophthalmological manifestations. Steroids and immunomodulators are the mainstay of medical management in early stages of disease. We report the case of a 15-year-old girl with complaints of diminution of vision in both the eyes. Her ophthalmic examination suggested a diagnosis of Takayasu retinopathy with posterior ischemic optic neuropathy in her left eye and ocular ischemic syndrome in the right eye. Digital subtraction angiography revealed a significant narrowing of major vessels orig…

Giant cell arteritis manifested by chronic dry cough.

A 77-year-old man visited the hospital with a chronic cough persisting for 2.5 months accompanied with night sweats, weight loss (3.5 kg) an … nd foreign-body giant cell reaction with the elastic lamina, resulting in a diagnosis of giant cell arteritis (GCA). The cough was considered a symptom of GCA as it resolved following prednisolone administration. Cough may rarely be an initial GCA symptom. However, for chronic cough accompanied with elevated inflammatory findings but with a normal lung fiel …
