This paper reviews the typical features of psoriatic arthritis, with emphasis on the clinical manifestations in children. The concepts of seronegative spondyloarthropathy and enthesopathy are described. Recent information is presented on genetics, possible pathophysiologic mechanisms, and treatment.

Musculoskeletal emergencies include tendon rupture, carpal tunnel syndrome, rupture of synovial cyst (" pseudothrombophlebitis " syndrome), subluxations in the cervical spine, cricoarytenoid arthritis and septic arthritis. Ocular complications can be serious emergencies, as can vasculitis and rheumatoid nodules. Drug reactions represent an additional group of urgent problems. While rheumatoid arthritis is characteristically an indolent disease, failure to recognize its urgent complications can cause significant morbidity and mortality.

Reactive arthritis is arthritis in which, although the nature of the responsible infection is known or suspected upon serological grounds, attempts at recovering the pathogen from the synovial fluid have failed. One of the main pathogenetic problems is the multiplicity of etiologic agents. Some are exogenous and may be related to the articular tropism of certain microorganisms, to immunologic depression due to an antecedent or coincident infection, and to successive reinfections by the same pathogen or by others which may promote an exacerbation of the disease. Others are endogenous and attent…

Aspirin remains the cornerstone of treatment for juvenile arthritis, although tolmetin may be more effective in controlling the fever of systemic-onset disease. Tolmetin and naproxen appear to be more effective in HLA-B27 positive males with pauciarticular disease. When the nonsteroidal anti-inflammatory drugs fail, gold, penicillamine and, occasionally, immunosuppressive agents may be used in the polyarticular forms. Prosthetic joint replacement may be beneficial for patients with extensive disease. The emotional and sexual problems of children with chronic disease require recognition and hel…

Cases of reactive arthritis generally have a favourable short-term outcome, within 1 to 6 months in 50 to 70 per cent of cases. The curable nature of the disease and its short clinical course mean that the therapeutic approach should be devoid of risks. Antibiotics, which are necessary for the mucosal lesion, are ineffective in the joint involvement. Non-steroidal anti-inflammatory agents, sometimes in combination with intra-articular steroid injections, seem to be sufficient in the simple forms. In the more chronic, polyarticular forms, gold therapy would appear to be preferable to the use of…

In an eight-week double-blind study comparing the new long-acting aspirin derivative, diflunisal, in doses up to 1 g/day with aspirin in doses up to 4 g/day in 16 patients with classical or definite rheumatoid arthritis, diflunisal was more effective in reducing the total articular index (Ritchie) and erythrocyte sedimentation rate and in increasing grip strength. Diflunisal had an earlier effect on erythrocyte sedimentation rate than antiinflammatory doses of aspirin. Patients on diflunisal experienced fewer side effects than patients on aspirin. Ten patients with rheumatoid arthritis who pre…

Naturally occurring mycoplasmal arthritis in various animals species and the ability to induce mycoplasmal arthritis experimentally have been the main reasons for searching for evidence of mycoplasmal infection in the joints of patients with rheumatoid arthritis. However, reliable reports of the isolation of mycoplasmas from synovial fluids and tissue of such patients do not exist, and measurements of specific antibodies have not proved rewarding. There is some evidence that infection by Mycoplasma pneumoniae occasionally has an arthritic sequela, although the microorganism has not been isolat…

Fifty-four patients with rheumatoid arthritis and severe osteomalacia were found to have considerable bone and general deficiency problems. In 46 of them 14 stress fractures occurred and 32 minimal trauma fractures necessitating admission to hospital. Radiological abnormalities of absorption were found in nearly 25% of the total, implying that dietary factors alone are not always responsible for osteomalacia in patients with rheumatoid arthritis. A high index of suspicion is necessary in the diagnosis of osteomalacia in patients with rheumatoid arthritis, so they may benefit from treatment.

Since both psoriasis and seronegative arthritis occur individually in childhood, they may be expected to occur together occasionally. Clinical features used to identify psoriatic arthritis as a disease sui generi have been assembled in groups of adult patients, establishing the justification for describing a distinct syndrome. A review of the experience in an active children's rheumatology service indicates that the syndrome of psoratic arthritis occurs in the pediatric-aged patient with sufficient frequency to qualify as a diagnostic term appropriate in the classification of childhood-onset a…

Perhaps the most important conclusion is that much remains to be learnt about how to use penicillamine in clinical practice. The results described have been obtained by observing groups of patients on various fixed schedules of dosage. There may be a case for using a more flexible system but this makes comparisons difficult and at this stage predetermined fixed schedules have been preferred. A total daily dose of 1500 mg is clearly excessive and probably unnecessary. There is some suggestion that administration between meals produces better and perhaps more predictable absorption. If used in t

The most common disease of the temporomandibular joint (TMJ) is osteoarthritis. Rheumatoid arthritis and psoriatic arthritis may also involve this joint. Other diseases that may occasionally affect the TMJ include familial Mediterranean fever, systemic lupus erythematosus, gout, Sjögren's syndrome and infectious arthritis. Many cases of TMJ syndrome are labeled as idiopathic facial pain syndrome, a category that probably represents a number of different entities. The role of dental malocclusion has been greatly overemphasized in the past.

In the first 4 years (1974 to 1978) of operation of the Children's Hospital of Eastern Ontario, in Ottawa, 50 children were treated for septic arthritis. The neonatal group (birth to 6 months) of three patients had the most severe involvement and the worst prognosis. The early childhood group (6 months to 4 years) of 15 patients was characterized by sepsis due to Hemophilus influenzae. The 32 older children (4 to 16 years), more susceptible to staphylococcal infections, commonly presented with atypical and unusual symptoms. The erythrocyte sedimentation rate was the most reliable laboratory in…
