Multimodality Imaging Markers of Adverse Myocardial Remodeling in Aortic Stenosis.

Aortic stenosis (AS) causes left ventricular remodeling (hypertrophy, remodeling, fibrosis) and other cardiac changes (left atrial dilatation, pulmonary artery and right ventricular changes). These changes, and whether they are reversible (reverse remodeling), are major determinants of timing and outcome from transcatheter or surgical aortic valve replacement. Cardiac changes in response to AS afterload can either be adaptive and reversible, or maladaptive and irreversible, when they may convey residual risk after intervention. Structural and hemodynamic assessment of AS therefore needs to eva

Calcium Handling Abnormalities as a Target for Atrial Fibrillation Therapeutics: How Close to Clinical Implementation?

Atrial fibrillation (AF) is the most common cardiac arrhythmia with a substantial impact on morbidity and mortality. Antiarrhythmic drugs play a major role in rhythm-control therapy of AF. However, currently available agents exhibit limited efficacy and pronounced adverse effects, notably drug-induced proarrhythmia. Recent experimental studies have identified that Ca handling abnormalities are critical elements in AF pathophysiology with central roles in atrial ectopic activity, reentry, and atrial remodeling suggesting that Ca handling abnormalities could be promising targets for novel AF the…

Atrial functional mitral regurgitation: mechanisms and surgical implications.

The term atrial functional mitral regurgitation refers to a newly recognized disease entity in which mitral regurgitation occurs secondary to left atrial disease, without left ventricular dilatation and intrinsic mitral valve disease, typically in the setting of long-standing atrial fibrillation. Recent evidence suggests that atrial functional mitral regurgitation is associated with increased risk of death and heart failure re-hospitalization. The etiology, pathophysiology, and mechanism of atrial functional mitral regurgitation is not completely understood but they should not be regarded as t

Ensuring long-term arrhythmia freedom: what are the options for long-standing persistent atrial fibrillation?

INTRODUCTION: Long-standing persistent atrial fibrillation (LSPAF) is among the most challenging arrhythmia phenotypes to manage, due to extensive atrial remodeling, fibrosis, and the presence of complex non-pulmonary vein substrates and triggers. Conventional antiarrhythmic drug (AAD) therapy provides limited efficacy and is constrained by short- and long-term toxicity. Catheter ablation (CA) remains the cornerstone of rhythm control but offers modest single-procedure success. AREAS COVERED: This review examines current treatment strategies for LSPAF, including pharmacologic rhythm control, …

Left Atrial Function and Strain Assessed by Three-Dimensional Echocardiography as Factors Associated With Atrial Fibrillation Recurrence After Pulmonary Vein Ablation.

INTRODUCTION: Recurrence of atrial fibrillation (AF) after pulmonary vein isolation (PVI) remains a major clinical challenge. Left atrial (LA) remodeling contributes to recurrence, but its predictive value is still under investigation. This study aimed to assess LA morphology, function, and strain using three-dimensional (3D) echocardiography and determine their association with AF recurrence. METHODS AND RESULTS: This prospective, single-center study included 119 patients with paroxysmal or persistent AF who underwent radiofrequency or cryoballoon ablation. Six months after the procedure, an

Impact of gliflozins on cardiac remodeling in patients with type 2 diabetes mellitus & reduced ejection fraction heart failure: A pilot prospective study. GLISCAR study.

AIMS: Type 2 diabetes mellitus (T2DM) and heart failure are closely related entities and together determine an increased risk of mortality compared to patients suffering from only one of these diseases. Sodium-glucose co-transporter type 2 inhibitors (SGLT-2i) have shown favorable effects on cardiovascular system, particularly on heart failure. Aim of this study is to verify whether in individuals with T2DM and heart failure with reduced ejection fraction (HFrEF) treated with SGLT-2i, echocardiographic signs of favorable reverse remodeling follow longitudinal observation. METHODS: 31 subjects

[Pharmacological restoration and maintenance of sinus rhythm by antiarrhythmic agents].

Paroxysmal atrial fibrillation is defined as an atrial fibrillation that terminates spontaneously. It is desirable that atrial fibrillation should be terminated immediately after onset, and should be prevented from re-initiation to avoid atrial electrical remodeling or atrial stunning after cardioversion. Antiarrhythmic agents are used for these reasons. The important factors for pharmacological cardioversion of atrial fibrillation are thought to be prolongation of atrial refractory and suppression of conduction time in the atrium. Therefore, class Ia and Ic antiarrhythmic drugs, including bep

Cardiac contractility modulation attenuates structural and electrical remodeling in a chronic heart failure rabbit model.

BACKGROUND: Cardiac contractility modulation (CCM) is non-excitatory electrical stimulation for improving cardiac function. This study aimed to evaluate the effects of CCM on structural and electrical remodeling in a rabbit model of chronic heart failure (CHF). METHODS: Thirty rabbits were randomly divided into the sham, CHF, and CCM groups. The CHF model was induced 12 weeks after trans-aortic constriction by pressure unloading and CCM was delivered to the myocardium for 4 weeks. Corrected QT intervals, the ventricular effective refractory period, and inducibility of ventricular tachycardia

Neurocardiology: Cardiovascular Changes and Specific Brain Region Infarcts.

There are complex and dynamic reflex control networks between the heart and the brain, including cardiac and intrathoracic ganglia, spinal cord, brainstem, and central nucleus. Recent literature based on animal model and clinical trials indicates a close link between cardiac function and nervous systems. It is noteworthy that the autonomic nervous-based therapeutics has shown great potential in the management of atrial fibrillation, ventricular arrhythmia, and myocardial remodeling. However, the potential mechanisms of postoperative brain injury and cardiovascular changes, particularly heart r

Posterior left atrial epicardial adipose tissue: scope of the problem and impact of new technology.

PURPOSE OF REVIEW: Patients with persistent forms of atrial fibrillation are seeking treatments based on the promise of better restoration of sinus rhythm with newer therapies. Successful catheter ablation and maintenance of atrial fibrillation in this subgroup is negatively impacted by the presence of epicardial adipose tissue (EAT) associated with the posterior left atrium. RECENT FINDINGS: EAT is now understood to be hormonally active and promotes adverse atrial remodelling, including fibrosis and myopathy. Despite being dominantly adipose tissue, it is known to be electrically active, com

Atrial Tachycardia Utilizing Bachmann's Bundle as an Epicardial Bypass: A Case Suggestive of a Dual-Loop Mechanism.

A 70-year-old man with a history of prior ablation for atrial fibrillation presented with atrial tachycardia (AT). Electrophysiological studies revealed roof-dependent and localized-reentrant ATs. During each AT, widespread breakthrough activation was observed just anterior to the low-voltage area on the anterior wall, suggesting that Bachmann's bundle served as an epicardial bypass within the tachycardia circuits. This case highlights the potential role of Bachmann's bundle in the development of ATs in the remodeled left atrium.

Mass spectrometric proteome profiling using a deep spectral library reveals homogenization of right and left atrial proteomes in persistent atrial fibrillation patients.

AIMS: Pulmonary veins in the left atrium (LA) are well-established as a critical site for the initiation of atrial fibrillation (AF). Emerging evidence suggests that in persistent AF (persAF), AF triggers may extend beyond LA. However, the extent to which AF-associated remodelling involves the right atrium (RA) in persAF remains a subject of debate. To address this, we employed a proteomic approach aiming at investigating AF-associated remodelling in the RA relative to the LA in persAF. METHODS AND RESULTS: RA and LA samples were obtained from sinus rhythm (SR) patients, patients with persAF

Arterial hypertension and morphologic abnormalities of cardiac chambers: results from the Copenhagen General Population Study.

OBJECTIVES: In patients with arterial hypertension (AH), hypertension-mediated organ damage may be manifested by cardiac chamber enlargement and/or remodeling. Cardiac computed tomography imaging has emerged as an important method for morphological assessment of cardiac chambers. We tested the hypothesis that prevalence of cardiac chamber abnormalities is specifically related to clinical categories of AH in the general population. METHODS: We studied 4747 individuals, mean age was 60 years (range: 40-93), 46% were men, undergoing 320-detector computed tomography in the Copenhagen General Popu
