["Coronary arteriographic findings in the patients of arteriosclerotic heart disease combined with cardiac arrhythmias or conduction defects" (author's transl)].

Correlation between cine-coronary arteriographic findings and various cardiac arrhythmias or conduction defects were studied in 40 patients … vere and also, the number of the coronary artery involved in stenosis was increased. Interestingly, atrioventricular node arteries were not opacified in 5 of 10 cases. There were 2 cases of complete A-V block where the atrioventricular node artery was not visualized at all both either from right or left coronary artery opacification. It seemed to be hard to draw a clea …

[Successful selective electrical ablation of the retrograde pathway in atrioventricular nodal reentry tachycardia associated with syncope].

A 55-year-old man was admitted to the intensive care unit on account of repeatedly occurring syncopes which developed at the peak of physical exertion. The attack was reproduced by exercise on a bicycle ergometer: the patient developed paroxysmal tachycardia with a narrow QRS and a frequency of 160/min leading after 20 sec. to severe hypotension and loss of consciousness. The same tachycardia caused by programmed atrial stimulation caused a drop of tension in the recumbent position by 30 mmHg and after more detailed analysis during electrophysiological examination it was evaluated as atriovent

Fetal Supraventricular Tachycardia: Histologic Evidence of Accessory Pathways Due to Incomplete Annulus Fibrosus Formation.

BACKGROUND: Atrioventricular (AV) reentrant tachycardia is a common type of supraventricular tachycardia (SVT) that occurs in the fetus and neonate. Although many tachycardias resolve within several weeks of birth or respond to medical management, disruptions in the cardiac annulus fibrosus and development of additional accessory pathways may lead to refractory dysrhythmia resulting in fetal hydrops and ultimately, fetal death. OBJECTIVES: While accessory pathways have been well documented anatomically in adult and childhood tachyarrhythmias, there are no reports of the histology of these pat

[Radiofrequency catheter ablation of atrial flutter and atrial fibrillation].

Radiofrequency catheter ablation is now considered as a curative approach in patients with typical atrial flutter. Typical atrial flutter is due to a macrore-entrant circuit within the right atrium and it can be eliminated by a linear lesion in the isthmus between the tricuspid annulus and the vena cava inferior. The electrophysiological criterion of a bidirectional isthmus block has been shown to reduce the recurrence rate of atrial flutter after catheter ablation, thus achieving long-term cure of typical atrial flutter. Acute success rates of 85 to 90% and recurrence rates of 10 to 15% have

Evidence of three clinical subgroups in patients with dual atrioventricular nodal pathways.

We attempted to test the hypothesis that dual atrioventricular (A-V) nodal pathways with second-degree atrioventricular block (2nd A-V block) present as a different clinical entity from those with A-V nodal reentranttachycardia (AVNRT). By evaluation with Holter monitoring (2.9 +/- 2.5 recordings/patient) and 12-lead electrocardiogram (11.9 +/- 11.6), 177 patients with dual A-V nodal pathways could be divided into three subgroups. Thirty-two patients had 2nd A-V block only (2nd A-V block group), 57 had AVNRT only (AVNRT group), 88 had neither 2nd A-V block nor AVNRT (silent group), and none ha

Triple atrioventricular nodal pathways can be masked.

In summary, CL modulates the dispersion in refractoriness of triple nodal pathways by causing greater changes in refractoriness in the slower pathways and thus determines the feasibility of manifestation.

Catheter ablation for the treatment of paroxysmal supraventricular tachycardia.

Radiofrequency catheter ablation has evolved into a front-line curative therapy for patients who have paroxysmal supraventricular tachycardia secondary to Wolff-Parkinson-White syndrome. AV nodal reentrant tachycardia, and atrial tachycardia. In patients with accessory pathways, cure rates exceed 90% in almost all anatomic locations. Equally high success rates are noted in patients with atriofascicular pathways and the permanent form of junctional reciprocating tachycardia. Complications secondary to catheter ablation of accessory pathways occur in 1% to 3% of patients and include cardiac perf

[Therapy of AV nodal reentry tachycardia with catheter ablation of the fast retrograde pathway].

AV-nodal reentrant tachycardia (AVNRT) is a common cause of recurrent supraventricular tachycardia. Currently, catheter ablation of either slow or fast pathway are nonpharmacologic options for the treatment of patients with AVNRT. Radiofrequency (RF) catheter ablation of the fast pathway was attempted in 35 patients (aged 46.7 +/- 15 years; 12 m, 23 f) with recurrent AVNRT. RF energy (25-50 watt, 30-90 s) was delivered to the anterior right atrial septum. The catheter was placed posterior to the largest His bundle deflection. AV conduction was monitored during continuous pacing of the high rig

Radiofrequency ablation of slow pathway in patients with atrioventricular nodal reentrant tachycardia. Do arrhythmia recurrences correlate with persistent slow pathway conduction or site of successful ablation?

BACKGROUND: Residual slow pathway conduction in the form of persistent jump in the atrioventricular (AV) conduction time or atrial echo beats is a common finding after successful radiofrequency (RF) ablation of the slow pathway in patients with AV nodal reentrant tachycardia (AVNRT). Sites of successful RF ablation of the slow pathway may be located anteriorly in the tricuspid annulus (cephalad to the coronary sinus os) or posteriorly (at, within, or caudal to the coronary sinus os). The aim of this study was to investigate whether arrhythmia recurrences correlate with persistent slow pathway

Radiofrequency catheter ablation for AV nodal reentry: a technique for rapid transection of the slow AV nodal pathway.

Selective radiofrequency (RF) catheter ablation of the slow AV nodal pathway has shed new light on the anatomy and physiology of the atrioventricular junction. The recording of "slow pathway potentials" facilitates localization of the slow pathway and has led to a concept of multiple pathway components with atrial insertion sites covering a potentially broad region surrounding the coronary sinus os. The critical area for complete interruption of the slow pathway may be larger than lesion size produced by ablation at a single site, resulting in multiple RF applications with lengthy sessions and

The long R-P' tachycardias.

The family of tachycardias that are called long R-P' tachycardias represent a unique group of tachycardias which have been notably refractory to pharmacologic therapy in the past. On the surface electrocardiogram, the rhythms may be indistinguishable. It is only with careful electrophysiological evaluation in many cases that these rhythms can be sorted out. The differential diagnosis in these rhythms is important because with incessant tachycardia, ventricular dysfunction may be produced. In many of the instances of long R-P' tachycardias definitive and directed ablation of the tachycardia can

Influence of racemic higenamine on the sinus node.

The aim of this study was to explore the mechanism of racemic higenamine in the treatment of sick sinus syndrome (SSS). A total of 40 New Zealand rabbits were randomly divided into normal sinus node and damaged sinus node (SND) groups, and each group was randomly divided into treatment and control groups (n=10). The SND model was established by formaldehyde wet dressing of the sinus node area. The treatment groups were administered an intravenous infusion of 0.04 mg/kg racemic higenamine via the marginal ear vein within 5 min. The electrophysiological indicators of sinoatrial function, includi

Use of DDDRP pacing device in prevention and treatment of tachy-brady syndrome after Mustard procedure.

A 13-year-old male patient, who underwent Mustard operation for a very complex congenital heart disease (CHD), after palliation presented a decrease of the sinus node function, developing a tachy-brady syndrome and a mild dysfunction of atrioventricular (AV) conduction. He was successfully treated using a DDDRP pacemaker, which ensured a suitable atrial rhythm and was able to interrupt supraventricular tachycardia episodes. Until now, hospitalization related to episodes of heart failure or symptomatic arrhythmia, has not been necessary.

Radiofrequency catheter ablation for atrial fibrillation.

Until recently, catheter-based radiofrequency ablation for atrial fibrillation was limited to palliative approaches of either atrioventricular node ablation or modification. It is now recognized that at least a proportion of patients with paroxysmal atrial fibrillation may be suitable for curative ablation of an underlying single arrhythmogenic focus. With the intense interest in this area, a catheter-based cure involving endocardial linear lesion creation for patients with chronic or paroxysmal atrial fibrillation may not be far in the future.

[Lyme carditis].

BACKGROUND: Lyme carditis is a rare manifestation of systemic Lyme disease, typically occurring one to two months after infection. The most common feature is conduction system disturbance, often involving the atrioventricular (AV) node. CASE PRESENTATION: A young woman was admitted to the emergency department after experiencing palpitations. She was haemodynamically stable and had no respiratory distress. An ECG revealed an atrioventricular block with a PR interval of 380 ms. Her medical history included a recent tick bite. Potential causes of conduction disturbances were ruled out and blood

[The radiofrequency ablation of Mahaim and Kent fibers during the repair of Ebstein's anomaly].

A young man 20 years old was referred to the Institute with Ebstein anomaly. He had experienced palpitations since the age of eight. Those episodes occurred several times a year. During the electrophysiologic study a regular antidromic reciprocating tachycardia was induced using a right atriofascicular connection (Mahaim). The atrial insertion of the Mahaim fibers was in the right posterolateral atrium. Also another orthodromic supraventricular tachycardia was induced with anterograde conduction over atrioventricular node and retrograde conduction over to canceled atrioventricular right poster…

Verapamil effects in AV node reentry tachycardia with intermittent supra-Hisian AV block.

The electrophysiologic details of two patients with atrioventricular (AV) node reentry tachycardia with intermittent 2:1 supra-Hisian block are presented. Both patients had clear evidence for atrial arrhythmias as well, emphasizing the need for a careful diagnostic analysis. Evidence supporting a diagnosis of AV node reentry tachycardia included: (1) short ventriculoatrial (VA) coupling intervals, (2) normal retrograde sequence of atrial activation, (3) dependence on critical AV node conduction times for initiation of tachycardia by atrial pacing, (4) ability to pace and capture the atria or v

Common Supraventricular and Ventricular Arrhythmias in Children.

The most common pediatric arrhythmias are tachycardias, and the most common type is supraventricular tachycardia, originating from or above the atrioventricular node and HIS bundle. Ventricular tachycardias are less common but more dangerous. Supraventricular tachycardias usually cause a narrow complex tachycardia unless there is a basal bundle branch block or rate-dependent aberration. A wide QRS tachycardia should be treated as ventricular tachycardias unless proven to be an supraventricular tachycardia with aberration. Diagnosis of both tachyarrhythmia types depends mainly on 12-lead electr…
