Palpatory diagnosis of plagiocephaly.

INTRODUCTION: The term plagiocephaly, from the Greek plagios (oblique) and kephalê (head), means distortion of the head, and refers clinically to cranial asymmetry. Cranial Osteopathy, since it was first proposed, has focussed upon the diagnosis and treatment of birth trauma and cranial asymmetries, and consequently specific therapy for plagiocephalic deformities has been described. Osteopathic manipulation also has been proposed as a treatment for torticollis, a condition associated with plagiocephaly. For these reasons, we decided to look at the mechanics of the occipital bone and the adjace

Unusual presentations of craniovertebral junction tuberculosis: a report of 2 cases and literature review.

BACKGROUND: CVJ tuberculosis is a described entity requiring challenging ways of management. Severe neck pain, causing restricted neck movements and torticollis, is a characteristic presentation of neurologically asymptomatic suboccipital Pott's disease. CASE DESCRIPTION: Two patients with unusual CVJ tuberculosis form the basis for the present communication. The first patient presented with tubercular otitis media, causing progressive erosion of the petrous part of temporal bone, and destruction of the occipital condyle, along with the lateral mass of atlas, leading to CVJ instability. This

Craniovertebral junction tuberculosis: a review of 29 cases.

PURPOSE: The purpose of this work was to describe the various imaging findings in craniovertebral tuberculosis and the importance of imaging in treatment in these patients. METHOD: A retrospective review of MR and CT scans in 29 patients with craniovertebral tuberculosis was performed. The images were reviewed, paying special attention to both bony (skull base, atlas, and axis) and soft tissue involvement in addition to atlantoaxial dislocation, lateral subluxation of the dens, and compression of the spinal cord. RESULTS: Suboccipital pain with neck stiffness was the most common presenting s

Pleomorphic T-cell lymphoma with chondropathia tuberosa; a case report and review.

Pleomorphic T-cell-lymphoma (anaplastic IgA-plasma cell tumor) belongs to the group of malignant non-Hodgkin's lymphomas (NHL). The histological and immunophenotypical subtypes differ with genetic and environmental etiologic factors. Lymphomas arise from the clonal proliferation of precursor cells within lymphoid organs with acquired chromosomal abnormalities. Approximately 5% of all primary malignant bone tumors are NHL, the majority of diffuse large B-cell type. Our case history can be regarded as the first published in English language reporting on a pleomorphic T-cell-lymphoma imitating a

Craniovertebral junction database analysis: incidence, classification, presentation, and treatment algorithms.

INTRODUCTION: The initial treatment of bony lesions at the craniocervical junction consisted of posterior decompression with enlargement of the foramen and removal of the posterior arch of the atlas and axis vertebra. This was regardless of the site of compression. Needless to say, the results were poor when an irreducible ventral lesion at the craniocervical junction caused compression of the cervicomedullary area. It was predictable that an adverse outcome would occur in approximately 35-40% of patients treated with dorsal decompression with a fixed anterior abnormality at the craniovertebra

Os Odontoideum in Children: Treatment Outcomes and Neurological Risk Factors.

BACKGROUND: Treatment outcomes and risk factors for neurological deficits in pediatric patients with an os odontoideum are unclear. METHODS: We reviewed the data for 102 children with os odontoideum who were managed at 11 centers between 2000 and 2016 and had a minimum duration of follow-up of 2 years. Thirty-one children had nonoperative treatment, and 71 underwent instrumented posterior cervical spinal arthrodesis for the treatment of C1-C2 instability. Nonoperative treatment consisted of observation (n = 29) or immobilization with a cervical collar (n = 1) or halo body jacket (n = 1). Surg

Patterns of C-2 fracture in the elderly: comparison of etiology, treatment, and mortality among specific fracture types.

OBJECTIVE Previous studies have focused on Type II odontoid fractures and have failed to report on the effect of other C-2 fracture types on treatment and outcome. The purpose of this study was to compare patient characteristics, cause of injury, predisposing factors to fracture, treatments, and mortality rates among C-2 fracture types in a cohort of elderly patients 70 years of age and older. METHODS A retrospective cohort study design was used. Patients who sustained a C-2 fracture between 2002 and 2011 and who were admitted to the authors' Level 1 trauma center were identified using the Dis

Surgical management of the fractures of axis body: indications and surgical strategy.

PURPOSE: The axis body fractures are relatively uncommon and have a variety of presentations. Surgical management to them has been only reported as case reports or included as a minor part of clinical management. The objective of this study is to summarize the indications for surgery and report the clinical outcome of surgical treatment based on different fracture patterns. METHODS: A retrospective analysis of 28 consecutive patients presenting with the axis body fractures was undertaken. The indications for surgical treatment were defined as: (1) fractures associated with instability of adja

Anterior fixation of the axis.

BACKGROUND: Although anterior fixation of the axis is not commonly performed, plate fixation of C2 is an important technique for treating select upper cervical traumatic injuries and is also useful in the surgical management of spondylosis. OBJECTIVE: To report the technique and outcomes of C2 anterior plate fixation for a series of patients in which the majority presented with symptomatic degenerative spondylosis. METHODS: Forty-six consecutive patients underwent single or multilevel fusions over a 7-year period; 30 of these had advanced degenerative disease manifested by myelopathy or defo

[Pathomorphology, diagnosis and treatment of "hangman's fractures"].

In a retrospective study diagnostic procedures and outcome of treatment were evaluated in 33 patients with traumatic spondylolisthesis of the axis (Hangman's fracture) who were treated in our department between 1986 and 1999. There were 18 men and 15 women with an average age of 41 (17-88) years. The cause of the injury was in 23 cases an accident as a driver of a car, in 2 cases an accident with a bike, 2 patients had an accident as a pedestrian. 5 patients fell on their head and 1 patient was hit by a beam. In 15 cases there were relevant accompanying injuries. To assess the stability of the

[Atloaxoid dislocation in chronic inflammatory rheumatism. Results of 16 occipitoaxoid arthrodeses].

Sixteen cases of spontaneous dislocation of the atlanto-axial joint have been reviewed, fifteen in rheumatoid arthritis and one in ankylosing spondylitis. On four occasions there was some neurological impairment, but in eleven there was only pain and stiffness. The only case of ankylosing spondylitis was operated on after failure of conservative treatment. Three patients died between one and eight months after operation, two of them having had severe neurological impairment before operation. In the thirteen other cases, the average follow up was four years. All had satisfactory fusion after gr

Hangman's fracture caused by suspected child abuse. A case report.

This report highlights the difficulties associated with diagnosing cervical spine injuries in children especially as the history and mechanism of injury may often be unclear and the normal variations in roentgenographic appearance may be confusing. As far as we are aware this is only the second case of traumatic Hangman's fracture in a child under the age of 3 years and the only case where there is a strong probability of child abuse. A female child aged 23 months was admitted with a 5-day history of irritability and general malaise. Her father reported noticing that she was reluctant to move

Whole-axis versus odontoid process CBCT radiomics for forensic sex estimation: a retrospective paired-ROI model-development study.

BACKGROUND: The second cervical vertebra (C2, axis) is included in head-and-neck cone-beam computed tomography (CBCT) and may provide a target for forensic sex estimation. Whether odontoid-only segmentation preserves whole-C2 radiomic information remains unclear. OBJECTIVE: To develop and internally validate paired CBCT radiomics models using whole-C2 and odontoid-only regions of interest (ROIs), and to compare classification performance while describing feature-level reproducibility for each ROI. METHODS: This retrospective single-device study included 140 adult CBCT examinations (68 female

Do the newly proposed realignment targets for C2 and T1 slope bridge the gap between radiographic and clinical success in corrective surgery for adult cervical deformity?

OBJECTIVE: Surgical correction of cervical deformity (CD) has been associated with superior alignment and functional outcomes. It has not yet been determined whether baseline or postoperative T1 slope (T1S) and C2 slope (C2S) correlate with health-related quality-of-life (HRQoL) metrics and radiographic complications, such as distal junctional kyphosis (DJK) and distal junctional failure (DJF). The objective of this study was to determine the impact of T1S and C2S deformity severity on HRQoL metrics and DJF development in patients with CD who underwent a cervical fusion procedure. METHODS: Al

Cervical Spine pain as a presenting complaint in metastatic pancreatic cancer: a case report.

A 48 year-old female presented to her primary care physician with a two-month history of neck pain with negative cervical spine x-rays. During that office visit, the patient was noted to be tachycardic with EKG revealing ST depressions, which led to hospital admission. Acute coronary syndrome was ruled out, however, persistent neck pain warranted inpatient MRI of the cervical spine, which revealed a cervical spine lesion. Extensive investigation and biopsy ultimately confirmed stage IV pancreatic adenocarcinoma with metastases to the bone, liver, and likely lung. In the literature, the finding
