[Circulatory monitoring in very low birth weight infants using Doppler sonography].

Doppler-ultrasonography measures blood flow velocity, which is proportional to blood flow if the vessel's diameters remain constant. Perfusion of brain, kidneys and gut can thus be estimated by transcutaneous ultrasonography at specific locations on the internal-carotid artery, renal artery and upper mesenteric artery. Cardiac output can be derived from measurements taken on the ascending aorta. The procedures of measurement are easy to learn, but interpretation is more critical. Doppler-ultrasonography has been applied in persistent ductus arteriosus, asphyxia, hyperventilation, brain death,

Pulmonary interstitial emphysema in low birth weight infants: characteristics of survivors.

Twenty out of 303 ventilated low birth weight infants, hospitalized in the Beilinson Medical Center's Intensive Care Unit, during the years 1984-1986, developed pulmonary interstitial emphysema. Eighteen infants had very low birth weight (less than 1500 grams) and 17 were less than 30 weeks' gestation. The mean birth weight was 987 +/- 311 and mean gestational age 27.7 +/- 2.5. Eight infants survived the pulmonary disease. There were no significant differences in the neonatal parameters between infants who died or survived. However, the survivors had a significantly lower maximal peak inspirat…

Clinicopathological studies on pulmonary hypoplasia in very low birth weight infants.

Seven VLBW infants with pulmonary hypoplasia (P.H.) were studied retrospectively and their necropsied lungs were analyzed morphologically and histometrically in an attempt to establish the clinical and pathological criteria of P.H. Clinically they had various features as follows; 1) causes with oligohydramnios due to amniotic fluid leakage, 2) bell-shaped chest and/or small lung radiographically, 3) severe respiratory distress immediately after birth, 4) frequent complications with air leaks, lethal within several hours after birth. Pathologically they showed a series of changes; 1) remarkably

Subsequent pregnancy following labor of a very-low-birth-weight infant.

During a period of 5 years (1978-1982), 55 mothers with an average age of 27.5 +/- 5.4 years, delivered 59 infants, weighing less than 1500 g. These infants had a mean birth weight of 1160.5 +/- 263 g and a mean gestational age of 28.7 +/- 2.25 weeks (range 25-32 weeks). Subsequently 47 (79.6%) survived and 12 (20.4%) died. There was a statistical difference of both mean gestational age and of mean gestational weight between survivors or infants with neonatal death. Twenty two of 29 mothers who subsequently became pregnant, gave birth to liveborn infants, who subsequently survived (four pregna…

Controlled trial of new formulae for feeding low birth weight infants.

Over a 13-month period 85 babies were randomly allocated to one of three groups at birth. Babies in Group A were fed SMA Low Birth Weight, Group B Prematalac and Group C Preaptamil. A fourth group, the control group was fed expressed breast milk (EBM). The babies were studied when they were on full enteral feeds until weight was greater than 2.0 kg. Weight gain was greatest in Group B compared to the other three groups but there was no significant difference in gain of length or occipito-frontal circumference. The babies in Group B had a lower volume of feeds and lower energy intake than the o…

Obstetric management and intraventricular hemorrhage in very-low-birth-weight infants.

A study was conducted to determine the effect of obstetric management on the incidence of intraventricular hemorrhage in the very-low-birth-weight infant. The study covered four years and involved 488 live-born viable infants weighing between 500 and 1500 g, electronically monitored during labor and with echoencephalograms performed within the first three days of life. The overall incidence of intraventricular hemorrhage and mortality was 43 and 21%, respectively. However, both intraventricular hemorrhage and mortality, 72 and 44%, respectively, were increased for gestations less than 1000 g w

Maternal serum alpha-fetoprotein levels in low birth weight singleton pregnancies.

Maternal serum alpha fetoprotein (MSAFP) measurement between 16 and 21 weeks gestation is used to define a group of women with an increased risk of fetal abnormality, particularly open neural tube defect. The test is strongly gestation dependent and women with high MSAFP levels require sonar scan examination to define gestation, exclude twins and examine the fetus for obvious malformation or death. It has been reported that women with no primary cause for raised MSAFP have an increased incidence of low birth weight babies. Conflicting reports have separately ascribed these to premature deliver…

Fetal cardiac responding: a correlate of birth weight and neonatal behavior.

Certain intrapartum fetal heart-rate (FHR) patterns are associated with low Apgar scores and other neonatal risks. Intrapartum FHR has been the subject of extensive clinical investigation, but only occasional reference is made to the possible behavioral correlates of this phenomenon. The current investigation found that neonates classified as FHR decelerators had lower birth weights and shorter gestational ages than FHR accelerators. Decelerators exhibited higher baseline heart rate and experienced more frequent uterine contractions than accelerators. Neurobehavioral assessment of these neonat…

Obstetric care and intraventricular hemorrhage in the low birth weight infant.

A concern of the obstetrician is whether any single event or combination of events before delivery places a prematurely born infant at increased risk of developing periventricular-intraventricular hemorrhage, a form of intracranial hemorrhage in 40 to 45% of all low birth weight infants. In this three-year retrospective study, 103 infants weighing less than 1500 g and delivering on or before the 35th gestational week showed evidence of intraventricular hemorrhage by routine cranial ultrasound scan. The finding of maternal hypertension, vaginal bleeding, or preterm ruptured membranes was simila…

Ligation of patent ductus arteriosus in very low birth weight infants.

Twenty-five low birth weight, premature neonates who were refractory to aggressive medical management underwent ligation of symptomatic patent ductus arteriosus. The mean birth weight was 1,007 g and the mean gestational age 29 weeks. Six patients (24 percent) died before discharge, two from continued cardiorespiratory failure. Echocardiography showed significant improvement in left atrial and left ventricular dimensions after ligation. In the premature neonate with respiratory distress and congestive heart failure refractory to aggressive medical management, surgical ligation may be accomplis…

Maternal smoking, birth weight, infant death, and the self-selection problem.

Several aspects of the relationship between maternal smoking and birth weights of infants are discussed. No satisfactory explanation for Yerushalmy's results has been given other than that low birth weight appears to relate more to the smoker than to the smoking. Recent studies by Silverman support this position. The possibility that nicotine may induce a physiologic response that serves to alleviate bioenergetic deficiency in some individuals should not be overlooked. In this view, both smoking and low birth weight are symptoms of deficient maternal bioenergetic systems.

Digoxin therapy in low-birth-weight infants with patent ductus arteriosus.

Digoxin therapy was evaluated retrospectively in a group of 30, and prospectively in a group of 16 low-birth-weight, premature infants with cardiorespiratory symptoms due to persistent patency of the ductus arteriosus. The response to decongestive therapy was equivocal. Digoxin levels in serum varied between 1.5 and 13 ng/ml. Digoxin half-life in serum exceeded three days in four patients. Fourteen of the combined group of 46 infants had signs of toxicity of digoxin. High dosage, inadvertent overadministration, and variable clearance of digoxin, as well as special characteristics of the patien

Factors associated with low birth weight of infants delivered at term.

The singleton births in the 1958 British Perinatal Mortality Survey (Butler and Bonhan, 1963; Butler and Alberman, 1969) were used to study the aetiology of low birth weight of infants delivered at term. After exclusion of macerated stillbirths and infants with lethal congenital defects, 468 infants of 259 days or more gestation and a birth weight which was less than 2500 g were identified. The mothers and their pregnancies were compared with those of the population of 16 994 singleton births. Delivery at term of a low birth weight infant was significantly associated with maternal prepregnant …

Pneumoperitoneum in the extremely low-birth-weight infant: successful nonoperative treatment.

Pneumoperitoneum without associated pulmonary air leak occurred in two extremely preterm, low-birth-weight infants. These infants were considered too critically ill to tolerate surgical exploration. Accordingly, they were treated medically and supportively, with excellent results.

Early discharge of infants of low birth weight: a prospective study.

A total of 495 African infants of low birth weight were discharged from Harari Maternity Hospital, Salisbury, between October 1972 and September 1973. Criteria used for discharge were (a) no clinical evidence of disease, (b) satisfactory feeding by mouth (breast or bottle or both), and (c) stable temperature control under normal room conditions. Of the 495 babies 264 fulfilled these criteria when they weighed 1801-1900 g (group 1), 99 when they weighed 1901-2000 g (group 2), and 132 (group 3) when they weighed 2001-2500 g. The overall follow-up rate of those infants living in greater Salisbury…
