BACKGROUND: Several methods for the treatment of acquired flexible flatfoot have been described.
PATIENTS AND METHODS: We followed the outcome of calcaneocuboid distraction arthrodesis with lengthening of the lateral column prospectively in 20 patients (20 feet). The mean age of the patients was 55 (30-66) years and 16 were women. The lateral column lengthening was combined with percutaneous lengthening of the Achilles tendon and augmentation of the posterior tibial tendon in all patients. Fibular hemimelia is the congenital absence of the fibula and it is the most common congenital absence of long bone of the extremities. Shortening of the extremity is obvious at birth with leg-length discrepancy. On plain radiograph of the leg and foot, significant fibular deficiency (hypoplasia) or absence of the fibula can be seen. A simple, light distraction device was developed for digital lengthening. Materials for this device are one piece of 1.5-mm K-wire, two five-hole miniplates, one one-hole miniplate, two bolts and nuts, and a nonabsorbable suture or surgical wire. Seven children were treated surgically as a result of a pathologic fracture through a simple bone cyst in the subtrochanteric region of the proximal femur. Average age at surgery was 10.6 years. Six children were treated primarily. One child was operated for a refracture through a persistent cyst and malunion of a previous fracture that had been treated nonoperatively. AIMS: To compare the structural durability of hydroxyapatite-tricalcium phosphate (HATCP) to autologous iliac crest bone graft in calcaneal lengthening osteotomy (CLO) for pes planovalgus in childhood.
PATIENTS AND METHODS: We present the interim results of ten patients (HATCP, n = 6 and autograft, n = 5) with a mean age of 11.5 years (8.2 to 14.2) from a randomised controlled non-inferiority trial with six months follow-up. The primary outcome was the stability of the osteotomy as measured by radiostereometric analysis. A non-inferiority margin of ≤ 2 mm osteotomy compression was set.

In immature long bones, radical excision of malignant tumours of the metaphysis may necessitate sacrifice of the adjacent epiphysis. To preserve the adjacent joint while allowing a safe margin of excision, we used physeal distraction before removing the tumour. From July 1984 to August 1992, we operated on 20 patients by this method. After a mean follow-up of 54 months there was no local recurrence in the epiphyseal region. Three patients had developed pulmonary metastases.

Axial force applied during epiphyseal distraction has been measured close to skeletal maturity in patients having leg lengthening, in a rabbit model, and in vitro from an amputation specimen. Six dog's femurs were lengthened until complete peroneal palsy was present. Ten-24 days later MRIs of the spinal cords were normal. Between 1987 and 1991, 209 patients were operated by the Ilizarov method in the Department of Orthopaedic and Traumatologic Surgery of the University of São Paulo Medical School.
