Monosymptomatic hypochondriacal psychosis complicated by self-inflicted skin ulceration, skull defect and brain abscess.

Self-inflicted dermatoses are associated with personality disorders and psychoses, including monosymptomatic hypochondriacal psychosis (MHP), which is characterized by a delusion involving a particular hypochondriacal concern. We report an unusual case of MHP with severe mutilation of the skin complicated by a skull defect and brain abscess. The patient was a 66-year-old uneducated man who damaged his forehead repeatedly because he believed that a 'toxic root' in the forehead was the source of his general ailment. He admitted that the lesions were self-inflicted. There was no other evidence of…

Brain abscess following acute otitis media in a nasopharyngeal carcinoma survivor: a case report.

INTRODUCTION: Although rare, brain abscesses secondary to acute otitis media (AOM) may pose serious risks, particularly among adolescent and young adult (AYA) survivors of head and neck cancer. PRESENTATION OF CASE: A 29-year-old woman who survived AYA nasopharyngeal carcinoma following chemoradiotherapy developed an AOM complicated by a left temporal lobe brain abscess and motor aphasia. After undergoing neurosurgical drainage and receiving intravenous antibiotics, she achieved full recovery without mastoidectomy. DISCUSSION: Eustachian-tube dysfunction resulting from prior radiotherapy and…

Fusobacterium necrophorum Brain Abscess With Concurrent Nasopharyngeal Detection of Mycoplasma pneumoniae: A Case Report.

Fusobacterium necrophorum is a recognized cause of brain abscesses and has been classically associated with Lemierre syndrome. We report a case of a 17-year-old male patient who presented with seizures, fever, and altered mental status following treatment for sinusitis. Neuroimaging revealed a right frontal lobe abscess requiring surgical drainage. Anaerobic cultures from the abscess fluid grew F. necrophorum, while a respiratory polymerase chain reaction (PCR) panel from a nasopharyngeal swab detected Mycoplasma pneumoniae. Anaerobic cultures also yielded Paenibacillus barengoltzii from a sep…

Cryptogenic brain abscess: Right-to-left shunt in pulmonary arteriovenous malformation and hepatopulmonary syndrome.

BACKGROUND: Pulmonary arteriovenous fistula (PAVF) and hepatopulmonary syndrome (HPS) both cause right-to-left shunting, allowing pathogens to bypass the pulmonary filtration barrier and enter systemic circulation, leading to brain abscess. Despite distinct etiologies, both share the same pathological pathway-right-to-left shunt-resulting in the same devastating neurological complication. CASE PRESENTATION: Two male patients with cryptogenic brain abscesses caused by PAVM (59 years old) and HPS (52 years old), respectively, are reported. The patient with PAVM presented with fever, dyspnea, an…

Hemorrhagic brain abscess mimicking an intracranial neoplasm in invasive Klebsiella pneumoniae syndrome: illustrative case.

BACKGROUND: Brain abscess rarely presents with hemorrhagic transformation. Invasive Klebsiella pneumoniae syndrome (IKPS) is characterized by hematogenous dissemination from a primary hepatic focus, causing metastatic infections in various organs. The authors report a rare case of hemorrhagic brain abscess mimicking an intracranial neoplasm in the setting of IKPS managed with stereotactic drainage. OBSERVATIONS: A 56-year-old male with a history of intra-abdominal surgery presented with progressive right-sided hemiparesis. CT performed at the referring hospital revealed a ring-enhancing lesio…

Utility of clinical metagenomics in complex infections: Cryptococcal meningitis complicated by Nocardia brain abscess.

Nocardia farcinica brain abscess (BA) is a rare yet life-threatening infection of the central nervous system (CNS) that predominantly affects immunocompromised patients. Its nonspecific symptoms often lead to delayed diagnosis and poor outcomes. Early diagnosis and precise treatment are essential to improve the prognosis of patients. We report the rare case of a 75-year-old man with IgG4-related disease undergoing long-term methylprednisolone therapy who presented with a N. farcinica brain abscess. The patient initially presented with cryptococcal meningitis but exhibited persistent symptoms d…

Vein of Galen aneurysmal malformation associated with brain abscess: A computed tomography case report.

Vein of Galen malformation (VGM) is a rare congenital, uncommon intracerebral vascular anomaly rarely complicated with the development of brain abscess as secondary to primary infection or after endovascular treatment. We report a very rare finding of a vein of Galen aneurysm associated with a large brain abscess at the time of diagnosis. A 12-year-old boy with a high-grade fever, severe headache, and recurrent episodes of convulsions came into the radiology department of Kassala Advanced Diagnostic Center. On a Siemens 16-slice scanner, brain non-contrast enhanced computed tomography (NECT) a…

Subdural empyema, brain abscess, and superior sagittal sinus venous thrombosis secondary to Streptococcus anginosus.

Streptococcus anginosus can be frequently isolated from brain abscesses, but is a rare cause of the liver, lung, and deep tissue abscesses. In this report, we present a patient with subdural empyema, brain abscess, and superior sagittal cerebral venous thrombosis as complications of rhinosinusitis whose purulent empyema sample yielded S. anginosus. A 13-year-old female patient was referred to our pediatric intensive care unit with altered mental status, aphasia, and behavioral change. On a brain computed tomography scan, subdural empyema extending from the left frontal sinus to the frontal int…

Rapid identification of Nocardia cyriacigeorgica from a brain abscess patient using MALDI-TOF-MS.

Nocardia cyriacigeorgica (N. cyriacigeorgica) is most frequently associated with human infections, including chronic bronchitis, pulmonary disease and brain abscesses. In general, N. cyriacigeorgica causes infections in immunocompromised individuals and has been reported in clinical samples worldwide. However, the isolation and speciation of N. cyriacigeorgica in the routine diagnostic microbiology laboratory are complicated and time consuming. Recent mass spectrometry techniques such as matrix-assisted laser desorption/ionization time-of-flight-mass spectrometry (MALDI-TOF-MS) have been succe…

Hereditary hemorrhagic telangiectasia patient presenting with brain abscess due to silent pulmonary arteriovenous malformation.

Hereditary hemorrhagic telangiectasia is a rare autosomal dominant inherited disease that is usually complicated by visceral vascular malformations. Patients harboring such malformations are at increased risk of brain abscess formation, which despite advances in diagnostic and surgical methods remains a life threatening medical emergency with high mortality and morbidity rates. In the present report we describe a case of cerebral abscess due to silent pulmonary arteriovenous malformation (AVM) in a young patient previously undiagnosed for hereditary hemorrhagic telangiectasia syndrome (HHT).

Supratentorial neurenteric cyst with spontaneous repetitive intracystic hemorrhage mimicking brain abscess: a case report.

Neurenteric cyst (NC) is a benign epithelial cyst (BEC) of endodermal origin that mostly occurs in the spinal subdural space or posterior cranial fossa. A 28-year-old male presented with a left frontal lobe NC associated with spontaneous repetitive intracystic hemorrhage, which was initially diagnosed and treated as a brain abscess. He presented with headache and disorientation, without underlying diseases. A cystic tumor was suspected because of a hypointense signal on diffusion-weighted magnetic resonance imaging (MRI). One day after admission, his condition deteriorated rapidly and emergenc…

Hereditary hemorrhagic telangiectasia (Rendu-Osler-Weber disease) presenting with polymicrobial brain abscess. Case report.

A 26-year-old man presented with expressive aphasia, weakness of the right arm, and anemia but there was no family history of hereditary hemorrhagic telangiectasia. Computerized tomography (CT) of the head demonstrated an enhancing left frontal mass, which was aspirated and on culture yielded multiple organisms. Pulmonary arteriovenous fistulae identified in both lungs by chest radiography, CT, and angiography were treated with coil embolization. Treatment of pulmonary arteriovenous fistulae and prolonged surveillance are necessary to prevent future neurological complications.

[Brain scintigraphic check-ups during inflammatory CNS diseases to prove absence or presence of brain abscesses and subdural empyemas].

Methods of instrumental diagnosis, which are used to provide evidence for the absence of complications associated with inflammatory processes of the central nervous system and which often give unsatisfactory results in addition to being accompanied by major risks, are compared with long-time scintigraphic reexaminactions which involve no danger to the patient examined by this method. This comparison has been made from the point of view of workers at a department of neuroradiology of a clinic of neurosurgery. Process-like developments into abscesses or empyemata as well as mixed conditions may
