Paradoxical Stroke via Persistent Levoatriocardinal Vein and Evaluation and Management Recommendations.

BACKGROUND: The levoatriocardinal vein (LACV) is a cardiac anomaly that may cause right-to-left shunts and paradoxical embolic stroke. To date, there are no guidelines for management of this phenomenon. CASE SUMMARY: An 80-year-old man with a history of Parkinson disease, dementia, and prostate cancer presented to the hospital with new right-sided weakness and aphasia. Computed tomography angiogram of the head and neck demonstrated occlusion of the left middle cerebral artery, and he underwent endovascular thrombectomy. Brain magnetic resonance imaging revealed acute bilateral infarcts in mul

The shadow in the darkness: Case report on adhesive intestinal obstruction secondary to ventriculoperitoneal shunt catheter in an elderly patient.

INTRODUCTION: Shunt placement is an effective therapy for hydrocephalus. Ventriculoperitoneal shunt draining excess cerebrospinal fluid connects the cerebral ventricles to the abdominal cavity. However, intestinal obstruction may ensue as an infrequent complication of the shunt. CASE PRESENTATION: A 65 years old female patient presented with abdominal pain, abdominal bloating, and ceased passage of flatus and stool for six days. She had a history of undergoing a VP shunt procedure due to midbrain obstruction and supratentorial hydrocephalus. Conservative treatment at another local hospital co

Surgically Treated de Novo Cervicomedullary Arachnoid Cyst in Symptomatic Adult Patient.

BACKGROUND: Arachnoid cysts are a relatively common finding in adult patients, especially with the advent of advanced imaging techniques. The overall incidence ranges from 1%-2%, and the majority are clinically silent. Arachnoid cysts are postulated to arise by congenital anomalies or trauma. De novo formation of arachnoid cysts has been reported but is exceptionally rare and mostly found in the pediatric population after head trauma. There have only been 2 reported cases of symptomatic de novo arachnoid cyst formation in adult patients to date, both with histories of head trauma. CASE DESCRI

Fatal Cerebral Air Embolism: A Case Series and Literature Review.

Cerebral air embolism (CAE) is an infrequently reported complication of routine medical procedures. We present two cases of CAE. The first patient was a 55-year-old male presenting with vomiting and loss of consciousness one day after his hemodialysis session. Physical exam was significant for hypotension and hypoxia with no focal neurologic deficits. Computed tomography (CT) scan of head showed gas in cerebral venous circulation. The patient did not undergo any procedures prior to presentation, and his last hemodialysis session was uneventful. Retrograde rise of venous air to the cerebral cir

Histophysiological and surface ultrastructural studies of the saccus vasculosus of Notopterus chitala (Hamilton).

We investigated the cytoarchitecture and surface ultrastructural characteristics of different cells lining the epithelium of the saccus vasculosus of Notopterus chitala (Hamilton, 1822). The saccus vasculosus, the richly vascularized reddish, sac like organ is situated on the ventral aspect of the diencephalon just behind the pituitary gland. Histologically, the saccus vasculosus consisted of several loculi lined with a heterogeneous population of a large number of specialized coronet cells and a smaller number of alternately arranged supporting cells. The loculi were densely surrounded by blo

Pharmacokinetics and biodistribution of squalene-containing emulsion adjuvant following intramuscular injection of H5N1 influenza vaccine in mice.

Squalene is a component of oil-in-water emulsion adjuvants developed for potential use in some influenza vaccines. The biodistribution of the squalene-containing emulsion adjuvant (AddaVax™) alone and as part of complete H5N1 vaccine was quantified in mechanistically and toxicologically relevant target tissues up to 336 h (14 days) following injection into quadriceps muscle. At 1 h, about 55% of the intramuscularly injected dose of squalene was detected in the local quadriceps muscles and this decreased to 26% at 48 h. Twenty-four hours after the injection, approximately 5%, 1%, and 0.6% of th

[The parasellar osteo-dural chamber and the vascular and neural elements that traverse it. An anatomical concept that would replace the cavernous sinus of classical anatomy].

On each side of the sella turcica, because of the existence of vascular and nervous elements, the two aspects of the parasellar segment of the tentorium cerebelli diverge and with the wall of the sphenoid bone form an extradural lodge. In this space are included a venous pathway (the cavernous sinus of the classics), the internal carotid artery and the three oculo-motor cranial nerves. The venous pathway is a plexus of small sized veins, mainly draining the orbital blood, and has neighborhood relations with the carotid. The morphology of the region and the relations of the internal carotid art

Vascularization of the pars distalis of the hypophysis in the toad, Bufo bufo (L.) (Amphibia, Anura). A comparative light microscopical and scanning electron microscopical study. I.

The vascularization of the pars distalis of the hypophysis of the toad, Bufo bufo (L.), was studied by the traditional method of injecting a mixture of India-ink and gelatine into the circulatory system of the head via the arteria coratis communis. Further, methyl-methacrylate corrosion casts of the brains were made; the hypothalamo-adenohypophysial region of these corrosion casts was studied with the scanning electron microscope. The results showed that the portal vessels which arise from the median eminence do not supply distinct areas in the pars distalis as is supposed by the point-to-poin

Functional and Hemodynamic Restoration After Microsurgical Resection of Compact High-Flow Temporo-Parieto-Occipital Arteriovenous Malformation.

Background/Objectives: Arteriovenous malformations (AVMs) in the dominant temporo-parieto-occipital (TPO) junction of the brain are extremely rare and very difficult to remove surgically because this area includes multiple sensory and language networks. Due to the fact that many patients present with bleeding, surgeons have to find a delicate balance between removing all of the AVM tissue and preserving the functional areas of the brain where important functions occur. This study is reporting a case demonstrating how precise clinical-radiologic correlation, detailed anatomical knowledge, and d

Non-invasive diagnosis and metabolic consequences of congenital portosystemic shunts in C57BL/6 J mice.

This study demonstrates the suitability of magnetic resonance imaging (MRI) and magnetic resonance angiography (MRA) for the imaging of congenital portosystemic shunts (PSS) in mice, a vascular abnormality in which mesenteric blood bypasses the liver and is instead drained directly to the systemic circulation. The non-invasive diagnosis performed in tandem with other experimental assessments permits further characterization of liver, whole-body and brain metabolic defects associated with PSS. Magnetic resonance measurements were performed in a 26-cm, horizontal-bore, 14.1-T magnet. MRA was obt

Evaluation of Overshunting between Low and Medium Pressure Ventriculoperitoneal Shunts in Dogs with Severe Hydrocephalus Using Frameless Stereotactic Ventricular Shunt Placement.

Hydrocephalus is a neurological disorder characterized by an abnormal accumulation of cerebrospinal fluid (CSF) within the ventricular system of the brain, leading to cerebral ventricular dilation, brain parenchyma compression, and neuronal cell loss. Surgery is an effective method of draining excessive amounts of CSF. Ventriculoperitoneal shunt (VPS) allows excess CSF to divert into the abdomen; this device is the most commonly used in the treatment of hydrocephalus both in veterinary and human patients. This study aims to describe the application of two types of VPS, low-pressure valve and m

A Rare Case of Thoracolumbar Epidural Abscess with Tuberculous Etiology in an Adolescent Male with Neurological Deficit.

INTRODUCTION: Spinal epidural abscess (SEA) refers to collection of infection of the epidural space due to various etiologies. Tuberculosis (TB) of the spine is one of the important causes of SEA. Patient with SEA usually presents with history of fever, back pain, difficulty in walking, and neurological weakness. Magnetic resonance imaging (MRI) is the initial modality for diagnosis and it can be confirmed by examination of abscess for microorganism growth. It can be treated by laminectomy and decompression which helps to drain out the pus and relive the compression on the cord. CASE REPORT:

Specific restrictions in the progression of Venezuelan equine encephalitis virus-induced disease resulting from single amino acid changes in the glycoproteins.

The pathogenesis of Venezuelan equine encephalitis virus (VEE) was examined in the mouse model using V3000, a virus derived from a molecular clone of the Trinidad donkey strain of VEE. These results were compared in parallel experiments with avirulent mutants of VEE derived by site-directed mutagenesis of the clone. Adult mice, inoculated subcutaneously in their left rear footpad with V3000, were followed in a time course study for 6 days in which 15 organs were tested for histopathological changes, for the presence of viral antigen by immunohistochemical staining, for the presence of viral nu

Artificial Intelligence for Automatic Analysis of Shunt Treatment in Presurgery and Postsurgery Computed Tomography Brain Scans of Patients With Idiopathic Normal Pressure Hydrocephalus.

BACKGROUND AND OBJECTIVES: Ventriculo-peritoneal shunt procedures can improve idiopathic normal pressure hydrocephalus (iNPH) symptoms. However, there are no automated methods that quantify the presurgery and postsurgery changes in the ventricular volume for computed tomography scans. Hence, the main goal of this research was to quantify longitudinal changes in the ventricular volume and its correlation with clinical improvement in iNPH symptoms. Furthermore, our objective was to develop an end-to-end graphical interface where surgeons can directly drag-drop a brain scan for quantified analysi

AVM Management Equipoise Survey: physician opinions regarding the management of brain arteriovenous malformations.

INTRODUCTION: Management of unruptured brain arteriovenous malformations (AVMs) is controversial, and conducting clinical trials in this area has been a challenge. We sought to determine which, if any, patient/AVM characteristics were most likely to influence clinicians' decisions regarding management and clinical trial enrollment. METHODS: We performed an online survey (Survey Monkey) of members of the American Association of Neurological Surgeons (AANS) (n=4836) and Society of NeuroInterventional Surgeons (SNIS) (n=512). Physicians were asked to rate the likelihood that various patient/AVM

Angiogenesis and brain oedema in intracranial meningiomas: influence of vascular endothelial growth factor.

The correlation between angiographic neovascularization, peritumoural brain oedema (PTBOe) and the expression of vascular endothelial growth factor (VEGF), was analysed in 30 patients with intracranial meningiomas. Pre-operative angiograms were examined for the existence of either an exclusively dural tumour blush or an additionally pial tumour supply from cerebral arteries. Furthermore the presence of macroscopic tumour-neovascularization and dysplastic changes of tumour-draining cerebral veins was evaluated. VEGF expression was investigated on histological tissue samples, using immunohistoch

Sigmoid and Transverse Sinus Thrombosis as a Complication of Chronic Suppurative Otitis Media in a Child: A Case Report and Review of Literature.

Chronic suppurative otitis media is an easily managed condition with potential for severe sinus thrombophlebitis complications secondary to cholesteatoma, if not diagnosed and treated promptly. Diagnosis relies on the culture and sensitivity of the drained suppuration, computed tomography (CT), and magnetic resonance imaging (MRI). Treatment entails a combination of all or some surgical, antibiotic, and anticoagulant use with postoperative follow-up. Our 8-year-old female patient presented with chronic left postauricular swelling with foul-smelling otorrhea and associated otalgia. More recentl

Profiling of the immune landscape in murine glioblastoma following blood brain/tumor barrier disruption with MR image-guided focused ultrasound.

PURPOSE: Glioblastoma (GB) poses formidable challenges to systemic immunotherapy approaches owing to the paucity of immune infiltration and presence of the blood brain/tumor barriers (BBB/BTB). We hypothesize that BBB/BTB disruption (BBB/BTB-D) with focused ultrasound (FUS) and microbubbles (MB) increases immune infiltration in GB. As a prelude to rational combination of FUS with ITx, we herein investigate the impact of localized BBB/BTB-D on innate and adaptive immune responses in an orthotopic murine GB model. METHODS: Mice with GL261 gliomas received i.v. MB and underwent FUS BBB/BTB-D (1.
