Preterm birth does not increase the risk of developmental dysplasia of the Hip: a systematic review and meta-analysis.

BACKGROUND: The purpose of this systematic review was to appraise the literature on the association between preterm birth and developmental … d not find preterm birth to be a significant risk factor for DDH. Data suggests that female sex and breech presentation are associated with DDH in preterm infants, but the data is scarce in the literature.

Does Screening Ultrasound Timing in Developmental Dysplasia of the Hip Need to be Adjusted for Moderate Preterm and Near-term Infants: A Prospective Study.

INTRODUCTION: An initial screening ultrasound is essential for patients at higher risk of developmental dysplasia of the hip (DDH) due to breech presentation or a family history of DDH. The American Academy of Pediatrics recommends screening ultrasounds to be performed after 6 weeks of age to reduce the rate of false positives. However, there is limited evidence regarding whether these screening ultrasounds need to be adjusted for gestational age in prematurity. The purpose of this study was to evaluate the influence of moderate preterm and near-term births on screening hip ultrasounds for hig…

Dislocation of the hip after normal sonographic screening examination: a case report and literature review.

We report the case of a five-month-old girl presenting with a subluxed left hip following normal neonatal clinical examination and serial ultrasound screening. Her only risk factor for developmental dysplasia of the hip (DDH) was breech presentation. She underwent closed reduction with successful concentric reduction. This case demonstrates that hip subluxation can occur after normal ultrasound screening, and has important clinical and medicolegal implications. Consideration should be given to further follow-up in children with overt risk factors for DDH, even after normal ultrasound examinati…

[Decision making on breech delivery: information as a way to reconcile maternal autonomy and medical responsibility].

As far as breech vaginal delivery remains an acceptable option, each case has to be evaluated in order to determine whether in that particular situation it is medically relevant. When vaginal delivery is to be envisaged, maternal consent is needed. This implies seeking medical information that allows women to express their autonomy and to be part of the decision regarding their delivery. This article concerns a physicians reflection on medical information and on connections between the obstetrician's responsibility, that of the future mother, and autonomy. Understanding information as necessar

Oligohydramnios: maternal complications and fetal outcome in 145 cases.

One hundred and forty-five cases of oligohydramnios in the second and third trimester were diagnosed by ultrasonography out of 25,000 obstetrics patients (0.58%). In this group, pregnancy complications included hypertension (22.1%) and bleeding in the second trimester (4.1%). We found a high incidence of meconium-stained amniotic fluid (29.1%), fetal distress (7.9%) and premature placental separation (4.2%). IUGR occurred in 24.5% of cases. Asphyxia during labor occurred in 11.5% and different other perinatal problems in 23.5%. Cesarean section was performed in 35.2% of these pregnancies. Seve

Antenatal diagnosis of congenital dislocation of the knee: a case report.

Congenital dislocation of the knee was diagnosed antenatally in an abdominal flat plate. This is probably the first time that this condition has been diagnosed prior to delivery. Examination of the newborn after delivery revealed dislocation of the right hip and knee. The obstetric and orthopedic implications and suggested treatment are discussed.

[Transverse fetal position syndrome--a combination of congenital skeletal deformities in the newborn infant].

We describe a pattern of connatal postural deformities observed in approximately 0.6% of otherwise healthy, mature newborn infants. It comprises unilateral flattening of the skull (dolichocephalus), ipsalateral mandibular hypoplasia and torticollis, deviation of the septum nasi, pes calcaneovalgus on one and pes adductus supinatus on the other side. The deformities occur in four distinct combinations attributable to the prenatal lie of the fetus: dorsoanterior or dorsoposterior, left or right oblique lie. The deformities respond well to manipulative therapy.

Abdominal rescue using the vacuum extractor after entrapment of the aftercoming head.

BACKGROUND: Fetal replacement into the uterus for ceasarean delivery after failed vaginal delivery has been reported for both vertex and breech presentations. Although an option, this mode of delivery frequently is the last maneuver to deliver a viable infant after other methods to allow vaginal delivery have failed. We report the adjunctive use of a vacuum extractor to facilitate abdominal rescue after entrapment of the aftercoming head during an attempted vaginal breech delivery. CASE: A multiparous woman presented at term with two fetal feet bulging through the membranes at the introitus. …

Revisiting Traditional Risk Factors: Implications for Risk Stratification in Developmental Dysplasia of the Hip.

BACKGROUND: Identifying patients at the highest risk of developmental dysplasia of the hip (DDH) is critical to the success of an efficient … mentation of DDH risk factors from 2018 to 2022 were included. Classic risk factors were defined as breech presentation, female sex, firstborn status, and family history of hip dysplasia. Ultrasounds and radiographs obtained at initial presentation were reviewed to determine Graf and IHDI classifications. Reasons for referral for DDH screening were obtained from the …

[Oligoamnios].

We have studied 34 cases of severe oligoamnios, and compared them with the published literature on the etiology, pathology diagnosis, complications and prognosis. We have mentioned the important role of ultrasound. The principal etiological features have been: intragrowth retardation, high maternal blood pressure, malformations of the fetal urinary tracts. We obtained an increased rate of: prematurity, small for dates newborns, congenital malformations, perinatal morbidity, as well as breech presentation and cesarean section.

Ultrasonography of the fetal cerebellum.

The transverse diameter of the cerebellum was measured with ultrasound in 265 normal fetuses ranging from 15 to 39 weeks gestational age and found to correlate closely with the biparietal diameter. The transverse diameter may be useful in estimating fetal age, particularly in breech presentation where extrinsic pressure may deform the skull and decrease the biparietal diameter. Failure to demonstrate the cerebellum could be a sign of Arnold-Chiari malformation or Dandy-Walker cyst.

Developmental dysplasia of the hip: diagnosis and management to 18 months.

Developmental dysplasia of the hip represents a spectrum of disease ranging from transient neonatal instability to established dislocation. It is accepted that female sex, breech presentation, and family history are risk factors for the disease. Early diagnosis by clinical examination or ultrasound imaging is emphasized, with splint treatment ideally commencing by 6 weeks of age. Treatment using the Pavlik harness is successful in up to 90% of patients. Ultrasound imaging is the gold standard for monitoring a patient during harness wear. Failed splintage or late presentation usually necessitat…
