Use of an image intensifier to remove a challenging foreign body in a segmental bronchiole.

Aspiration of foreign bodies into the tracheobronchial tree is a common presentation in children, but less so in adults. Successful extraction depends on the type of foreign body aspirated, location of the foreign body in the tracheobronchial tree, the experience of the operator and the instrumentation available. We report on our management of a patient who presented with an aspirated foreign body.

[Bronchiolitis with IgG4-positive plasma cell infiltration in a patient with bronchial asthma].

A 49-year-old-man was admitted to our hospital because of cough, wheezing, and dyspnea. Chest CT showed bilateral bronchocentric nodules and increased bronchial thickness. And he was given a diagnosis of bronchial asthma. However, another small airway disease, such as diffuse panbronchiolitis (DPB) was also suspected, and surgical lung biopsy was performed. The biopsy specimens showed a prominent peribronchial lymphoplasmacytic and eosinophilic infiltration with IgG4-positive plasma cells. IgG4-positive plasma cells may play a role in the pathogenesis of bronchiolar diseases associated with al…

[The burden and outcomes of acute bronchiolitis among young children hospitalized in Israel].

BACKGROUND: Acute bronchiolitis (AB) is a significant indication for hospitalization during the winter period. Underlying conditions increase risk for severe manifestations. AIMS: To estimate the burden and outcomes of AB in northern Israel. METHODS: A prospective study was performed between 1.12.2005 - 31.3.2006. Previously healthy children younger than 2 years of age, hospitalized with the diagnosis of AB, in three hospitals in northern Israel, were included in this study. RESULTS: Overall, 465 children (93%) out of 500 children who were hospitalized due to AB and comprised 18% of all hos…

Levels of Th1 and Th2 cytokines in children with post-infectious bronchiolitis obliterans.

BACKGROUND: Post-infectious bronchiolitis obliterans (BO) is a chronic obstructive airway disease associated with inflammation and fibrosis of the small airways; it is more common in children who have had acute viral bronchiolitis. No previous studies have reported the immune response of BO. Unbalanced Th1/Th2 immune response might be one of the risk factors for developing this illness. AIM: To compare the production of interferon (IFN)-gamma, interleukin (IL)-4 and IL-10 in peripheral blood mononuclear cell cultures in children with BO and in healthy children. METHODS: From March 2003 to Oc…

Characterization of respiratory exposures at a microwave popcorn plant with cases of bronchiolitis obliterans.

Eight former workers from a microwave popcorn packaging plant were reported to have severe obstructive lung disease consistent with bronchiolitis obliterans. Investigations into respiratory exposures at this plant were done during August through November of 2000. Samples were collected to assess airborne particulate concentrations, particle size distributions, endotoxins, oxides of nitrogen, organic gases and vapors, and other analytes. Bulk corn and flavoring components were also analyzed for endotoxins and culturable bacteria and fungi. Workers in the microwave production areas of the plant …

Bronchiolitis obliterans organizing pneumonia associated with sulfasalazine in a patient with rheumatoid arthritis.

Pulmonary toxicity and blood dyscrasias are rare side effects of sulfasalazine. Pulmonary pathology is variable, the most common being eosinophilic pneumonia with peripheral eosinophilia, and interstitial inflammation with or without fibrosis. We here present the case of a 68-year-old female patient treated for 6 months with sulfasalazine for rheumatoid arthritis. On laboratory examination, eosinophil count was 97 x 10(3) mm(3). Thorocoscopic biopsy was performed. Histopathologic diagnosis was bronchiolitis obliterans organizing pneumonia (BOOP). This is the first case in the literature to pre…

Elevated levels of soluble adhesion molecules in sera of patients with acute bronchiolitis.

The mechanisms of migration of neutrophils into the airway lumen are crucial in the development of airway injury of acute bronchiolitis and are mediated by adhesion molecules. In this study, we have attempted to evaluate the role of serum concentrations of the soluble form of intercellular adhesion molecule-1 (sICAM-1) in the disease activity in acute bronchiolitis and in respiratory syncytial virus (RSV) infection. Circulating levels of sICAM-1 in sera from 10 normal control subjects, and from 47 hospitalized acute bronchiolitis patients at admission, and from 25 patients on the day of discha…

RSV bronchiolitis and risk of wheeze and allergic sensitisation in the first year of life.

Severe respiratory syncytial virus (RSV) infection has been hypothesised to be a risk factor for the development of allergy and asthma, but epidemiological studies in older children have been inconclusive. The current study hypothesises that the effect of RSV bronchiolitis might be most prominent during the first year after bronchiolitis. Forty-two infants had experienced RSV bronchiolitis severe enough to cause hospitalisation. For each child with RSV infection, two controls were acquired from a birth cohort and matched for date of birth and sex. All the children were followed prospectively a…

Chemokines and inflammation in the nasal passages of infants with respiratory syncytial virus bronchiolitis.

This study measured chemokines in nasal lavage fluids (NLF) from infants with respiratory syncytial virus (RSV) bronchiolitis, defined by lung hyperinflation and wheezing. Comparison was made to RSV-positive infants without bronchiolitis and RSV-negative infants with acute respiratory illnesses. RSV-positive illnesses were associated with increased epithelial shedding, increased RANTES/protein ratios, and increased IL-8/protein ratios in NLF compared to RSV-negative illnesses. Among RSV-positive infants, bronchiolitics had greater total cell counts and percentage epithelial cells in NLF than n…

[Atelectasis of the right lower lobe in association with bronchiolitis obliterans organizing pneumonia].

A 58-year-old woman with underlying rheumatoid arthritis was admitted to the hospital because of a dry cough and the presence of an abnormal shadow in the right lower lung field. Consolidation and volume loss in the right lower lobe with air bronchogram were recognized on a chest tomogram. Bronchofiberscopic examination disclosed neither stenosis nor tumors in the large bronchi. Organizing pneumonia was recognized pathologically in transbronchial lung biopsy (TBLB) specimens. The volume of the right lower lobe decreased rapidly, and new infiltration shadows appeared in the right upper and midd

Vitamin A is involved in maintenance of epithelial cells on the bronchioles and cells in the alveoli of rats.

We examined the effects of mild vitamin A deficiency and ozone (O3) exposure on the labeling index, a marker of cell proliferation, of epithelial cells on the bronchiole and cells in the alveoli of rat lungs, to assess the role of vitamin A in maintenance of epithelial cells on the distal airway and alveolus. Three-week-old rats were fed a vitamin A-depleted diet for 4 wk to induce mild vitamin A deficiency. After 2 wk rats were exposed to 16.4 mumol O3/m3 for 1 to 14 d. In vitamin A-sufficient rats, labeling indices of epithelial cells on the bronchiole and cells in the alveolus increased sig

Clinical and physiological manifestations of bronchiolitis and pneumonia. Outcome of respiratory syncytial virus.

The physiological abnormalities and clinical correlates of 32 infants consecutively hospitalized with lower respiratory tract disease from respiratory syncytial virus (RSV) were studied in an attempt to characterized the infant most at risk for the acute and long-term complications of RSV infection. Arterial oxygen saturation (Sao2) determinations were obtained daily by means of an ear oximeter. On admission all infants were hypoxemic with a mean Sao2 of 87% (range, 74% to 95%). The mean of the lowest Soa2 recorded during their hospitalization was 85.5% (range, 53% to 96%). The hypoxemia impro

Magnesium sulphate for treating acute bronchiolitis in children under two years of age.

RATIONALE: The effectiveness of magnesium sulphate for acute bronchiolitis in children under two years is unclear. There is a paucity of robust data favouring or opposing the use of magnesium sulphate in acute bronchiolitis. This is an update of a review first published in 2020. OBJECTIVES: To assess the effects of magnesium sulphate in acute bronchiolitis in children under two years of age. SEARCH METHODS: We searched CENTRAL, MEDLINE, Embase, LILACS, CINAHL, and two trial registries until 23 November 2024. We contacted trial authors and searched conference proceedings and reference lists o…

Characterization of Airway Morphology in Post-infectious versus Post-lung Transplant Bronchiolitis Obliterans.

BACKGROUND: Post-infectious bronchiolitis obliterans (PIBO) may occur following childhood infections. Subsequent dysanaptic lung growth, with differential development of the alveolar compartment over the airways, remains elusive. We performed a morphological characterization of the whole airway tree in PIBO compared to bronchiolitis obliterans syndrome (BOS) after lung transplantation. METHODS: Lungs from matched PIBO (n=5), BOS (n=5), and non-diseased donors (n=5) were evaluated using ex vivo high-resolution computed tomography (CT) scans followed by three-dimensional (3D)-airway segmentatio…

Therapeutic interventions and pulmonary function in pediatric patients with post-infectious bronchiolitis obliterans.

Post-infectious bronchiolitis obliterans (PIBO) is a rare chronic obstructive pulmonary disease affecting children after a severe respiratory infection. The primary goal of this narrative review is to synthesize evidence on pulmonary function in children with PIBO, with a focus on spirometric indices, and to evaluate the efficacy of therapies reported in the literature. It also provides an overview of the disease's epidemiology, risk factors, clinical presentation and diagnostic methods. Studies reported various spirometric parameters including forced expiratory volume in one second, forced vi…

Multi-modal analysis of vascular remodelling in human obliterative bronchiolitis after lung transplantation.

Chronic lung allograft dysfunction (CLAD) is the leading cause of late morbidity and mortality after lung transplantation. Bronchiolitis obliterans syndrome (BOS), a major CLAD phenotype, is characterized by obliterative bronchiolitis (OB). While microvascular perturbations are described in BOS, their cellular and molecular characteristics remain unclear. Intact human CLAD lungs (n=3) underwent hierarchical phase-contrast tomography (HiP-CT) for three-dimensional vascular mapping. In OB-affected airways vs. controls (n=4 each), endothelial subtypes were characterized by multiplex immunofluores…

Assessing bronchiolitis severity: a comparative analysis of two commonly used clinical scoring systems.

UNLABELLED: The purpose of this study to compare the effectiveness of the Bronchiolitis Severity Score (BSS) and the Respiratory Distress Assessment Instrument (RDAI) in determining bronchiolitis severity and predicting outcomes. Additionally, it aimed to establish optimum cutoff points for both scores. In this prospective observational study, clinical details of enrolled infants, along with assessments using both scoring tools, were recorded. The performance of these scores in predicting "severe disease," defined by respiratory support requirements other than nasal prongs, PICU admission, res…
