The surgical anatomy of Kent bundles based on electrophysiological mapping and surgical exploration.

The development of the procedure for interruption of a Kent bundle for the correction of the tachyarrhythmias associated with the Wolff-Parkinson-White syndrome has been hampered by the lack of anatomic descriptions of the pathways. Only 21 descriptions are given in enough detail for guidance to the surgeon. Our experience with our first 100 patients operated upon for serious dysrhythmias associated with Kent bundles has supplemented this information. In our first 40 patients the Kent was divided in 22 of 39, one having elective and four having forced interruption of the His bundle, with succe

Trade-off between elimination of premature ventricular complexes and loss of synchronized left ventricular pacing improved cardiac function in a patient with heart failure.

Synchronized left ventricular pacing (sLVP) via adaptive cardiac resynchronization therapy (aCRT) algorithm might enhance the efficacy of CRT. A 71-year-old female was admitted with the diagnosis of heart failure. Electrocardiograms revealed left bundle branch block with QRS width of 144 ms, and frequent premature ventricular complexes (PVCs). A CRT device was implanted, and sLVP via a CRT algorithm was provided. Frequent PVCs arising from the His-bundle suppressed effective sLVP percentage. Radiofrequency application at the His-bundle successfully eliminated the PVCs, but induced complete atr

Is Left Bundle Branch Pacing Feasible in Patients With Ventricular Septal Defect?

His bundle typically passes through the central fibrous body of the atrioventricular node and then enters the membranous portion of the interventricular septum, where it branches into the left and right bundle branches. The feasibility and safety of left bundle branch area pacing (LBBAP) in patients with perimembranous ventricular septal defect (pmVSD) have not been reported.

Effects of hyperkalaemia on sinus nodal function in dogs: sino-ventricular conduction.

The effects of hyperkalaemia on electrograms recorded from the sinus node, crista terminalis, Bachmann bundle, right and left atria and His bundle were studied in anaesthetised dogs. Increasing hyperkalaemia up to about 8.5 mol X litre-1 produced: 1) a gradual prolongation of the sino-crista terminalis interval, and 2) a shift in sinus pacemaker location. Hyperkalaemia between 8.5 and 10.0 mmol X litre-1 produced arrest of most of the atria but persistence of electrical activity of the sinus node, crista terminalis, Bachmann bundle, His bundle and ventricles and sustained sino-ventricular cond

[Historical development of diagnosis of bradyarrhythmias : The early years of clinical electrophysiology in Germany].

The introduction of His bundle electrography by Benjamin Scherlag (New York) in 1969, together with programmed stimulation of the heart by Philip Coumel (Paris) in 1967, and Hein Wellens (Amsterdam) in 1972, were decisive turning points on the way to invasive electrophysiology and the development of an independent, now distinctly interventional subspecialty of cardiology. The main topic of the 1970s was bradycardic arrhythmias, promoted by pacemaker therapy, which had been introduced just over 10 years earlier. The recording of the potentials of the bundle of His and other recording locations …

The Continued Search for Physiological Pacing: Where Are We Now?

Cardiac pacing is an effective treatment for patients with bradycardia due to sinus node dysfunction or atrioventricular block. Despite decades of technological advances, the optimal ventricular pacing site to mimic normal human ventricular physiology and best hemodynamic response remains elusive. Beginning with atrial synchronous right ventricular (RV) apical pacing, the search has continued through alternate RV pacing sites, minimizing RV pacing, biventricular pacing, left ventricular (LV) pacing, and His-bundle pacing. Understanding the deleterious effects of long-term RV apical pacing in v

Safety and performance of a novel ICD lead for left bundle branch area pacing: Results from the ASCEND CSP trial.

BACKGROUND: Use of implantable cardioverter defibrillator (ICD) leads for left bundle branch area pacing (LBBAP) has relied on adaptation of conventional leads not designed for conduction system pacing (CSP). OBJECTIVE: This study aimed to evaluate the safety and effectiveness of a purpose-built CSP ICD lead designed for implantation within the LBBA. METHODS: The ASCEND CSP Investigational Device Exemption (IDE) trial is a prospective, multicenter, global, single arm, pivotal trial evaluating the UltiSynq CSP ICD lead (Model LDA1122Q) in patients undergoing de novo ICD or cardiac resynchroni

Heterogeneity in clinical judgment of septal lead position and capture type in left bundle branch area pacing.

BACKGROUND: Determining capture type and septal lead location during left bundle branch area pacing (LBBAP) relies on criteria obtained during implantation. However, during follow-up, the interpretation of left bundle branch (LBB) capture largely depends on QRS morphology, which is not so straightforward in LBBAP. OBJECTIVE: This study aimed to investigate the inter- and intraobserver agreement, as well as the accuracy of clinical judgment of the electrocardiogram (ECG) in determining LBB-capture and septal lead position in patients undergoing LBBAP implantation. In addition, the role of vect

Controversy: in heart failure patients with a reduced ejection fraction and left bundle branch block, conduction system pacing can be a valid alternative to biventricular pacing-pro and contra.

For patients with heart failure (HF) and bundle branch block, cardiac resynchronization therapy (CRT) by biventricular pacing (BiVP) has been found effective and has been widely used for around 20 years. The effects of BiVP are well documented in a row of large randomized controlled trials (RCTs) with long-term follow-up to include prolonged survival, less HF hospitalizations, and better quality of life for the patients. More recently, conduction system pacing (CSP) as His bundle pacing or left bundle branch area pacing has been introduced for CRT and shown to in best cases establish a normal

Verapamil in idiopathic ventricular tachycardia of right bundle branch block morphology: observations during electrophysiologic and exercise testing.

Electrophysiologic studies before and after administration of verapamil were performed in three young patients with recurrent sustained ventricular tachycardia (VT) of right bundle branch block morphology. VT was not provoked by maximal treadmill testing in any patient. Electrophysiologic findings at induction of VT suggested reentry in the first patient and triggered automaticity in the second. Findings were inconclusive in the third patient. Intravenous verapamil terminated the VT in all the three cases. Oral verapamil prevented laboratory induction of sustained VT in the latter two patients

Serial electrophysiologic testing with a permanent pacemaker after acute myocardial infarction.

Electrophysiologic testing in patients after acute myocardial infarction may predict subsequent sudden death. We used a permanent pacemaker for repetitive electrophysiologic testing in a patient after acute infarction. Programmed, twice-threshold ventricular extrastimulation at two weeks after anteroseptal infarction failed to induce ventricular tachycardia. Subsequent programmed extrastimulation induced sustained ventricular tachycardia on four occasions (6, 16, 29 and 65 weeks) after infarction. As yet (20 months), spontaneous ventricular tachycardia has not occurred. Firm conclusions cannot

Exercise induced "Mobitz type II" second degree AV block in a patient with chronic bifascicular block (right bundle branch block and left anterior hemiblock).

A 55 year old healthy man with chronic bifascicular block (right bundle branch block and left anterior hemiblock) had a near syncopal episode. A treadmill test showed exercise induced Mobitz type II AV block manifested clinically by paradoxical slowing of the heart rate and decreased blood pressure. True His Purkinje block and pseudo AV block due to His extrasystoles were documented by an electrophysiological study.

[Reciprocating tachycardia and anterograde conduction by a nodoventricular pathway].

Two patients were investigated for paroxysmal regular tachycardia with left bundle branch block centrifugation. A right-sided nodo-ventricular accessory pathway was demonstrated in both cases at electrophysiological investigation with His bundle recording and atrial and ventricular programd pacing techniques. However, the function of this pathway was different in the two cases. In the first case, there were no signs of an accessory pathway on the surface ECG in sinus rhythm but it could be unmasked by simple right atrial pacing at the same rhythm (widening of the QRS and shortening of HV from

Diagnosis of trifasicular damage following tetralogy of fallot and ventricular septal defect repair.

We evaluated the usefulness of the surface electrocardiogram to predict the presence of postoperative trifascicular damage. We used the pre and postoperative electrocardiograms and postoperative His bundle electrograms from 50 patients an average of 3.8 years following tetralogy of Fallot (TF) and ventricular septal defect (VSD) repair. Right bundle branch block (RBBB) and RBBB with left anterior hemiblock (LAH) developed in 88% and 18% respectively, and 22% had transient postoperative complete heart block (CHB). Mean P-A and A-H conduction times were 16.6 and 84.9 msec and were not different

[Right bundle-branch block depending on the cardiac frequency].

Two cases with paroxysmal tachycardia are described. A transitory right bundle branch block was induced, lasting from twenty to forty minutes, with the medicament controlling of the paroxysms. The transitory bundle branch block is explained by an existing lesion of one of the His bundle branches. A brief literature survey is presented and the experience in paroxysms treatment is shared.

His Pacing vs Biventricular Pacing for Cardiac Resynchronization Therapy: Long-Term Follow-Up From the His-Alternative I Trial.

BACKGROUND: The His-Alternative I (A Randomized Trial of His Pacing Versus Biventricular Pacing in Symptomatic Heart Failure Patients With Left Bundle Branch Block) trial was the first randomized European trial to compare cardiac resynchronization therapy (CRT) delivered by His bundle pacing (His-CRT) vs CRT delivered by conventional biventricular pacing (BiV-CRT). OBJECTIVES: The goal of this study was to compare long-term lead performance, survival, and echocardiographic response between His-CRT and BiV-CRT. "Three-level shock" in ablation-refractory paroxysmal atrial fibrillation.

His-bundle ablation is a very effective method to control drug-refractory supraventricular arrhythmias. We present a 69-year-old woman with paroxysmal atrial fibrillation, in whom several attempts of unipolar and bipolar His-bundle ablation were ineffective. Ultimately a stable 2/1 atrioventricular block was induced when three successive shocks of 400 W were applied. The first shock was given in that position with the largest His-potential deflection. The other two shocks were applied at catheter positions more distal and more proximal from the first one, respectively. We suppose that the init
