Antifungal therapy: lessons learned over the past 27 years.

As principal investigator of the National Institute of Allergy and Infectious Diseases-sponsored Mycoses Study Group for the past 27 years, I have been fortunate to play a role in the many advances in the field of clinical mycology and antifungal therapy. For the Finland lecture, I will briefly discuss the development of the Mycoses Study Group, provide an overview of the currently available antifungal agents, and describe advances and lessons related to the treatment and management of cryptococcal meningitis (the most common form of fungal meningitis), 3 important endemic mycoses (namely, bla

Candida albicans rvs161Δ and rvs167Δ Endocytosis Mutants Are Defective in Invasion into the Oral Cavity.

Invasive growth in tissues by the human fungal pathogen Candida albicans is promoted by a switch from budding to hyphal morphogenesis that is stimulated by multiple environmental factors that can vary at different sites of infection. To identify genes that promote invasive growth in the oral cavity to cause oropharyngeal candidiasis (OPC), we first identified C. albicans mutants that failed to invade agar medium. Analysis of nine severely defective mutants in a mouse model of OPC revealed that the strongest defects were seen for the rvs161Δ and rvs167Δ mutants, which lack amphiphysin proteins

Osh2 mediates Candida species resistance to miltefosine by regulating zymosterol accumulation.

Invasive candidiasis poses a growing threat to global public health, compounded by the scarcity of effective antifungal treatments. Miltefosine exhibits broad-spectrum antifungal activity, yet its mechanisms of antifungal action and the development of resistance remain poorly understood. Here, we first generated miltefosine-resistant strains of Candida glabrata through stepwise exposure to increasing drug concentrations. Whole-genome sequencing revealed that nonsense mutations in the OSH2 gene (193C > T and 3177C > A) were key drivers of resistance. Functional validation in Candida albicans co

Molecular genotyping of Candida albicans isolated from different sites may trace the etiological infection routes: Observation in China.

BACKGROUND: Invasive candidiasis is a growing concern worldwide, especially in immunocompromised patients, including ICU patients. OBJECTIVES: As Candida albicans is the leading cause of candidaemia, it is important to investigate the evolution of C. albicans in patients with candidaemia. METHODS: We analysed 238 strains of C. albicans isolated from different body sites. Antifungal susceptibility testing, CAI loci genotyping and multilocus sequence typing (MLST) of all isolates were performed. The relationships among the total isolates that differed in sequence at only one of the seven house

Comparison of invasive fungal diseases between patients with acute myeloid leukemia receiving posaconazole prophylaxis and those not receiving prophylaxis: A single-center, observational, case-control study in South Korea.

Posaconazole prophylaxis is effective in decreasing the incidence of invasive fungal diseases (IFDs) in patients with acute myeloid leukemia (AML). However, the use of antifungal prophylaxis varies in real-life practice, and only a small number of studies have compared the incidence of IFDs between those receiving posaconazole prophylaxis and those without prophylaxis. We compared the clinical characteristics and outcomes of IFDs between patients with AML who received posaconazole prophylaxis and those without antifungal prophylaxis. We reviewed the medical records of adult AML patients who und

Synthesis of Altissimacoumarin D and Other Prenylated Coumarins and Their Ability to Reverse the Multidrug Resistance Phenotype in Candida albicans.

Azoles are the main antifungal agents employed in clinical practice to treat invasive candidiasis. Nonetheless, their efficacy is limited by fungal resistance mechanisms, mainly the overexpression of efflux pumps. Consequently, candidiasis has a worrisome death rate of 75%. One potential strategy to overcome efflux-mediated resistance is to inhibit this process. Ailanthus altissima is a Chinese tree that produces several active substances, including altissimacoumarin D. Due to the low yield of its extraction and the need to search for new drugs to treat candidiasis, this study aimed to synthes

Autophagy-Inducing Factor Atg1 Is Required for Virulence in the Pathogenic Fungus Candida glabrata.

Candida glabrata is one of the leading causes of candidiasis and serious invasive infections in hosts with weakened immune systems. C. glabrata is a haploid budding yeast that resides in healthy hosts. Little is known about the mechanisms of C. glabrata virulence. Autophagy is a 'self-eating' process developed in eukaryotes to recycle molecules for adaptation to various environments. Autophagy is speculated to play a role in pathogen virulence by supplying sources of essential proteins for survival in severe host environments. Here, we investigated the effects of defective autophagy on C. glab

A Novel Small Molecule Inhibitor of Candida albicans Biofilm Formation, Filamentation and Virulence with Low Potential for the Development of Resistance.

BACKGROUND/OBJECTIVES: Candida albicans is the principal causative agent of candidiasis, the most common fungal infection in humans. Candidiasis represents the third-to-fourth most frequent nosocomial infection worldwide, as this normal commensal of humans causes opportunistic infections in an expanding population of immune- and medically-compromised patients. These infections are frequently associated with biofilm formation, which complicates treatment and contributes to unacceptably high mortality rates. METHODS: To address the pressing need for new antifungals we have performed a high cont

Posaconazole in the management of refractory invasive fungal infections.

The rising incidence of invasive fungal infections due to the expanding population of immunocompromised hosts and the increasing prevalence of fungal resistance has led to the need for novel antifungal agents. Posaconazole, a new member of the triazole class has demonstrated in vitro activity against a broad spectrum of fungi and clinical activity against various fungal pathogens, including Aspergillus spp., Candida spp., zygomycetes, and Fusarium spp. To date, posaconazole has been approved for prophylaxis of invasive fungal infections in stem cell transplant recipients with acute graft versu

Successful management of Candida auris external ventricular shunt infection: a case report.

BACKGROUND: Candida auris ventriculitis is a rare but serious condition associated with high mortality rates. The available literature on its management is limited, and there are few documented successful treatment strategies or care plans. CASE PRESENTATION: We report the case of a 34-year-old male patient from South Asia with Candida auris infection of an external ventricular shunt. The patient was initially treated with a combination of injectable amphotericin B liposomal and caspofungin for 21 days. This was followed by a 15-day course of injectable amphotericin liposomal and voriconazole

Case Series of End-Stage Liver Disease Patients with Severe Coccidioidomycosis.

Liver disease causes relative compromise of the host immune system through multiple overlapping mechanisms and is an established risk factor for invasive fungal diseases including candidiasis and cryptococcosis. This immunologic derangement also leads to rapid progression of disease with resultant increases in morbidity and mortality. We describe severe coccidioidomycosis cases in the setting of liver dysfunction. Collaborative multi-center epidemiologic studies should be performed to determine the incidence of severe coccidioidomycosis in patients with concurrent liver disease.

[Expression of intoxication syndrome in patients with destructive pulmonary tuberculosis discharging Candida fungi].

The severity of intoxication in tuberculous patients with destructive forms was quantified as the number of middle-size molecules. Diagnostic criteria for Candida infection are provided in the range from Candida carriage to invasive candidiasis basing on enzyme immunoassay and bacteriological examinations findings. Candida carriage is shown to be of little relevance to the intoxication progress and severity, whereas mucosal dissemination on the bronchial tree and subsequent development of the mycotic lesion affect intoxication symptoms aggravating tuberculosis course.

Disseminated histoplasmosis, invasive pulmonary aspergillosis, and other opportunistic infections in a homosexual patient with acquired immune deficiency syndrome.

A homosexual man with the acquired immune deficiency syndrome had an unusually wide array of opportunistic infections. Despite antibiotic treatment over a period of two and a half years, the patient died. Perianal herpetic ulcers, oral candidiasis, cytomegalovirus infection, and disseminated infections with both Histoplasma capsulatum and Mycobacterium avium-intracellulare were diagnosed during illness. An autopsy revealed invasive pulmonary aspergillosis and a cerebellar lesion caused by cytomegalovirus. The latter was probably responsible for the patient's gait disturbance.

A synthetic peptide vaccine induces protective immune responses against Candida albicans infection in immunocompromised mice.

Invasive fungal infections, such as those caused by Candida species, predominantly affects hospitalized and immunocompromised patients. Current mortality rates are expected to rise as drug-resistant fungal species increase and the pool of immunocompromised individuals grows. Today, antifungal treatments show limited effectiveness, underscoring the need for new safe and effective antifungal vaccines. This study investigates the efficacy of a protective immunization strategy with DC stimulated by a synthetic peptide, 3P-KLH, containing epitopes from three Candida albicans proteins (Als3, Hwp1 an

The potential of bacteriocins in invasive fungal infections: Antifungal activities and intestinal protection.

Invasive fungal infections are life-threatening complications in immunocompromised and critically ill patients. Invasive candidiasis may arise from endogenous fungal reservoirs, particularly under conditions of intestinal barrier disruption. Although conventional antifungal agents remain the therapeutic foundation, their clinical use is increasingly challenged by the emergence of resistant fungal pathogens, such as Candida auris. Bacteriocins are ribosomally synthesized antimicrobial peptides produced by microorganisms. Beyond their well-established applications in the food industry, bacterioc
