Molecular biomarkers in cardiac hypertrophy.

Cardiac hypertrophy is characterized by an increase in myocyte size in the absence of cell division. This condition is thought to be an adaptive response to cardiac wall stress resulting from the enhanced cardiac afterload. The pathogenesis of heart dysfunction, which is one of the primary causes of morbidity and mortality in elderly people, is often associated with myocardial remodelling caused by cardiac hypertrophy. In order to well understand the potential mechanisms, we described the molecules involved in the development and progression of myocardial hypertrophy. Increasing evidence has i

[Left ventricular insufficiency: definitions and mechanisms].

Left ventricular failure is defined as an inability to maintain, under resting conditions, a cardiac output sufficient for the oxygen requirement. It is accompanied by a rise in trans-mural ventricular filling pressure and/or a reduction in systolic volume. It may be linked to an alteration in the systolic performance and/or an alteration in the characteristics of the left ventricle. The compensatory mechanisms are represented by the noradrenergic stimulation; a rise in the muscular mass of the left ventricle and the associated changes in its geometry the heterometric regulation of Frank Starl

[Ortner syndrome as a cause of sudden dysphonia, a little-known etiology].

CLINICAL CASE: A 77-year-old woman was admitted with symptoms of heart failure and sudden dysphonia. Imaging tests revealed a pericardial effusion and the existence of severe cardiomegaly at the expense of an aneurysmal left atrium. Pericardiocentesis was performed, with improvement in the clinical picture of heart failure, although residual dysphonia persisted upon discharge. DISCUSSION: Ortner syndrome or cardiovocal syndrome consists of paralysis of the left recurrent laryngeal nerve caused by a cardiovascular condition, usually and originally described by left atrial dilation in relation …

Managing the Giant: A Novel Approach in Orthotopic Heart Transplantation With Extreme Left Atrial Enlargement.

This case report highlights an innovative surgical approach in orthotopic heart transplantation (OHT) for a patient with the largest recorded giant left atrium, measuring a biplanar volume of 1906 mL. The patient, a 63-year-old man with severe heart failure and a history of mitral and aortic valve replacements, presented with atrial fibrillation, moderate systolic dysfunction, and severe cardiomegaly. Despite optimized medical therapy, his condition warranted heart transplantation. The procedure involved a novel surgical technique utilizing Teflon reinforcement and a bovine pericardial patch t…

Tachycardia-Induced Cardiomyopathy Secondary to Iatrogenic Chronic Hyperthyroidism.

This case describes a middle-aged woman found to have tachycardia-induced cardiomyopathy secondary to chronic liothyronine overuse. Despite a non-specific presenting complaint, she was found clinically to have heart failure and evidence of fluid overload. ECG showed that she was in atrial flutter. A chest X-ray showed marked cardiomegaly and bilateral pleural effusions. Imaging of the abdomen showed a dilated inferior vena cava and evidence of systemic congestion. Trans-thoracic echocardiography showed a left ventricular ejection fraction of under 30%. Thyroid function tests showed a severely …

Ruptured aortic aneurysm in an adolescent with systemic lupus erythematosus.

Systemic lupus erythematosus (SLE) is a chronic, multisystem, autoimmune disease. Cardiovascular involvement is frequent; however, aneurysm and/or aortic dissection are rare entities with fatal evolution. The objective is to describe a rare and fatal complication of SLE in pediatrics and review the literature. We present the case of a 16-year-old girl with SLE with multisystem involvement without cardiovascular disease at diagnosis. She consulted for severe chest pain. Chest X-ray showed a dilated aortic arch with no cardiomegaly. The presence of a dissection was suspected, and an angiotomogra…

Dual-processing theory helps to explain delay in diagnosis of Stanford type A aortic dissection.

A woman in her 70s presented with chest pain, which was initially thought to be an acute coronary syndrome but subsequently felt to be pericarditis. Chest radiography and echocardiography demonstrated striking cardiomegaly and marked biatrial dilatation, likely secondary to undiagnosed restrictive cardiomyopathy. The patient remained well on the ward for some days with only mild discomfort and stable haemodynamics. CT of the thorax went on to unexpectedly demonstrate a Stanford type A aortic dissection. The patient was promptly transferred for emergent surgery but sadly died intraoperatively.D…

Systemic Infection by Angiostrongylus vasorum in a Fennec (Vulpeszerda) in an Italian Zoological Garden.

This paper reported a case of a metastrongyloid nematode Angiostrongylus vasorum infection in a fennec (Vulpes zerda) kept in a zoo in central Italy. The fennec had shown paralysis of the hind limbs, anorexia, weakness and respiratory signs before death. Cardiomegaly and granulomatous pneumonia were the major anatomopathological findings. Inflammatory lesions associated with parasitic larvae were observed in the lungs, brain, liver, heart, spinal cord and kidney of the fennec at histology. A. vasorum diagnosis was confirmed by both morphological and molecular identification of adult worms reco…

Adaptations and Diversity of Antarctic Fishes: A Genomic Perspective.

Antarctic notothenioid fishes are the classic example of vertebrate adaptive radiation in a marine environment. Notothenioids diversified from a single common ancestor ∼22 Mya to between 120 and 140 species today, and they represent ∼90% of fish biomass on the continental shelf of Antarctica. As they diversified in the cold Southern Ocean, notothenioids evolved numerous traits, including osteopenia, anemia, cardiomegaly, dyslipidemia, and aglomerular kidneys, that are beneficial or tolerated in their environment but are pathological in humans. Thus, notothenioids are models for understanding a…

Clinical and radiological findings for the new multisystem inflammatory syndrome in children associated with COVID-19.

The World Health Organization defines the multisystem inflammatory syndrome in children (MIS-C) as a new syndrome reported in patients aged … e elevated markers of inflammation and heart dysfunction. The most common radiological findings are cardiomegaly, pleural effusion, signs of heart failure, ascites, and inflammatory changes in the right iliac fossa. In the context of the current COVID-19 pandemic, radiologists need to know the clinical, laboratory, and radiological characteristics of this synd …

Ebstein's anomaly with 'reversible' functional pulmonary atresia.

We present the case of an infant with prenatal diagnosis, at 32 weeks gestation, of Ebstein's anomaly without anterograde flow from right ventricular to pulmonary atresia (PA)-functional PA with flow reversal in the ductus arteriosus. Prostaglandin E1 was started after birth. Chest X-ray showed severe cardiomegaly and echocardiogram confirmed Ebstein's anomaly with a thickened non-opening pulmonary valve without anterograde flow but with mild regurgitation. Multidisciplinary team decision was to progressively reduce prostaglandins and have an expectant attitude. Peripheral oxygen saturation ab…

Unilateral right pulmonary artery agenesis and congenital cystic adenomatoid malformation of the right lung with Ortner's syndrome.

We report a 2.5-year-old girl who presented with hoarseness of voice since 3 months of age and failure to thrive. Chest X-ray showed cardiomegaly with a deviation of the trachea and mediastinum to the right side. Two-dimensional echocardiography showed decreased flow across the right pulmonary artery, a small atrial septal defect (ASD) with a right-to-left shunt, and a dilated right atrium and right ventricle with severe tricuspid regurgitation suggestive of severe pulmonary hypertension. A silent large patent ductus arteriosus was also seen. Multiple detector computerized tomography aortogram…
